• Sonozaki syndrome: case report and review of literature.

  Sonozaki syndrome--pustulotic arthro-osteitis (PAO) is a relatively rare, chronic illness. This disease belongs to the group of psoriatic arthritis (psoriasis arthropatica, artropatia psoriatica)Sonozaki syndrome--pustulotic arthro-osteitis (PAO) is a relatively rare, chronic illness. This disease belongs to the group of psoriatic arthritis (psoriasis arthropatica, artropatia psoriatica) which in turn belongs to the group of seronegative arthritis. Sonozaki syndrome includes palmoplantar pustulosis, PPP as well as arthro-osteitis. Clinically, symmetrically localised pustulae are observed on feet and hands. Effected joints are painful, swollen with a visible inflammation. Here, we describe a case of a woman aged 55 with a diagnosis of Sonozaki syndrome and hyperthyroidism. At the moment of admission multiple changes in the form of pustulae were observed on hands and soles, filled with pus and blood of the erythemal basis. Oral and genital mucosa were free from changes. The oedema within clavicle and sternum joints was without features of the severe inflammation and tactical tenderness. In additional tests, increased BSR 36/62 was found. Bone scintigraphy-focuses of increased accumulation of MDP-Tc-99 m were found in the sternal projection of the clavicle ends at both sides, and the left-side change is bigger and more strongly saturated and can probably progress to the sternum's manubrium. As a result of the used treatment during hospitalisation, (cyclosporine 3 mg/kg and steroid external therapy upon the skin changes) the improvement of the local changes was observed as well as no progression in the joints' changes. At the moment, the patient is treated in the dermatological and rheumatological out-clinic. Early and correct diagnostics allows for efficient treatment of Sonozaki syndrome and decreases the risk of potential complications, such as the described systemic amyloidosis.

  The most common adverse effects of oral isotretinoin (>1/10) are: cheilitis, skin dryness, erythema, itching, scaling, bruising, skin atopy, conjunctivitis, dry eyes and eyelid inflammation. The aim of this work was to define an optimal dose regime of the oral isotretinoin allowing for effective reduction of sebum production and minimalization of adverse effects such as decreased skin moisturization, increase in TEWL and intensified erythema. The skin parameters were assessed using the MPA 5 diagnostic apparatus. Moisturization together with an increase in TEWL and erythema during treatment with isotretinoin seem to be crucial factors responsible for patient discomfort. The decrease in the pigment content in the skin during retinoid treatment is a favourable symptom, due to the frequent tendency of skin pigmentation disturbances in the sites after acne changes. On the basis of performed analyses it was concluded that the new therapy schema is the most advantageous form of isotretinoin dosing and it involves the use of a constant drug dose- 0.4-1.0 mg/kg body mass/day without modification during treatment.

  INTRODUCTION: Human cartilage glycoprotein-39 (HC gp-39) is a protein secreted by various cells including chondrocytes. Serum HC gp-39 has been suggested to be a marker of cartilage damage. However, inflammation involving other sites than the joints is an additional factor that increases the serum level of HC gp-39. OBJECTIVES: The aim of the study was to evaluate the usefulness of HC gp-39 determination in serum of patients with systemic lupus erythematosus (SLE) as a marker of joint involvement. PATIENTS AND METHODS: Serum HC gp-39 levels were measured in 25 patients with SLE and 22 healthy controls. SLE activity was assessed by the Systemic Lupus Erythematosus Disease Activity Index, and articular involvement by calculating the number of swollen and tender joints. The markers of inflammation (erythrocyte sedimentation rate, C-reactive protein) were determined. RESULTS: We observed an increase in HC gp-39 in SLE patients. However, there was no correlation of this parameter with disease activity, inflammatory markers (except serum gamma globulin levels), and articular involvement. CONCLUSIONS: The study suggests that increased HC gp-39 in SLE patients results mainly from inflammation and is not useful as a marker of joint involvement.

• Erythema nodosum caused by ascariasis and Chlamydophila pneumoniae pulmonary infection - a case report.

  Abstract Erythema nodosum belongs to a group of relatively common hypodermal inflammations. It occurs mainly among women, particularly young women. The etiology of the disease is not clear. MostAbstract Erythema nodosum belongs to a group of relatively common hypodermal inflammations. It occurs mainly among women, particularly young women. The etiology of the disease is not clear. Most frequently, changes appear on the surface of the frontal part of the shins. Initially, red nodules change in color to dark brown and then to yellow and green. There is neither dissolution nor cicatrization of the exanthema. Regression is frequent. We present a case of erythema nodosum caused by Ascaris lumbricoides infection as well as by an early Chlamydophila pneumoniae infection, whose etiology has rarely been described in the literature. We were not able to confirm which factor was responsible for the occurrence of the skin changes as treatment of both infections was effective and all skin changes later disappeared completely. Particular attention should be paid to the fact that precise diagnosis of a patient and the search for etiologic factors, even rare ones, are crucial to obtain good results with treatment of erythema nodosum.

CASE REPORT: We present case of granulomatosis disciformis chronica et progressiva Miescher with good response to systemic corticosteroids therapy.Patient 45 years old woman, with primary yellow-brown areas skin lesions, with foci well separated from surroundings on both lower legs, that occurred 5 years ago. In laboratory tests there was no abnormalities. Because of advance suggestion (after last admit in dermatological ward) of observation according to xantogranuloma necrobioticum tests for paraproteinemia were made. Immunoelectrophoresis, IgG, IgM, IgA levels, kappa light chain, lambda heavy chain; were correct, Bence-Johns protein-negative. During hospitalization in Clinic methylprednisolone in dose of 32 mg od, vascular drugs and local steroidotherapy was applied with good therapeutic response. CONCLUSION: We described case of typical clinical and histological characters of necrobiosis lipoidica. without diabetes-granulomatosis disciformis chronica et progressiva Miescher that despite of suspicion of proper diagnosis for a long time was not treat effective.

  The aim of the study was to evaluate the application of sonotherapy in enhancement venous leg ulcer healing after conservative and surgical therapy. MATERIAL AND METHODS: The 60 patients wereThe aim of the study was to evaluate the application of sonotherapy in enhancement venous leg ulcer healing after conservative and surgical therapy. MATERIAL AND METHODS: The 60 patients were included into the study. The 30 patients, who agreed on operation and were accepted by medical team to the surgical treatment, had been included to A and B group. The others, 30 patients were treated conservatively, and had been included to C and D group. The patients in group A and C were additionally treated in use of sonotherapy. The evaluated factors were to estimate the sonotherapy promotes total wound closure and causes any changes of the relative surface, longest and widest dimensions, and volume of tissue defect, pus and granulation degree. RESULTS: After study in all groups, we noticed a therapeutic effect. Treatment was more efficient in patients after surgical operation than in patients after conservative methods. Beneficial effects of sonotherapy in conservative enhancement of ulcer healing were observed. No impact of sonotherapy after surgical procedure was noticed. CONCLUSIONS: The sonotherapy is useful and efficient method only in enhancement of venous leg ulcer healing after conservative treatment. In surgically treated patients is not efficient method, because there is no accelerating impact on wound healing. As well conducted surgical operation more efficiently enhance a healing process than conservative pharmacological procedures.

  Erythema gyratum repens Gammel is a rare illness that almost always accompanies the tumour of internal organs. The case of 63 year-old women with the clinical and histological recognition of ErythemaErythema gyratum repens Gammel is a rare illness that almost always accompanies the tumour of internal organs. The case of 63 year-old women with the clinical and histological recognition of Erythema gyratum repens Gammel without the accompanying tumour will be introduced. The exanthema had the character of swelling, itchy erythemas that very quickly (in a few days) started widening concentrically, forming the typical ring-shaped pattern reminding grains. Mucous membranes were free from changes. Laboratory and pictorial examination did not show any significant deviations. According to the histopathological examination the clinical recognition turned out to be true. During the hospitalization, in spite of treatment (steroids - medium dose, anti-histamine drugs and external steroids therapy) a progression of skin changes and appearance of some new single changes were observed. At present the patient is under constant ambulatory control in order to keep the exanthemas under intensive oncological alertness.

  Common acne is the most frequently diagnosed dermatosis at the age of adolescence. It may lead to psychic disturbances and/or even depression as a result of its distinctly visible localization, veryCommon acne is the most frequently diagnosed dermatosis at the age of adolescence. It may lead to psychic disturbances and/or even depression as a result of its distinctly visible localization, very long-lasting symptoms, in sequela numerous deforming scars or vicious cicatrices. The objective of the present paper is to assess recurrences after the treatment of severe acne forms in patients treated in the Department of Dermatology in the years 1991-2000, with administration of various medications applied locally and/or generally. The evaluation of treatment results determined the highest, statistically significant difference between the patients subjected to isotretinoin-supported treatment process and the locally cured patients. In the group of locally treated patients the rate of the acne symptoms recurrence could be observed more often. In the remaining groups of patients, no statistically significant differences were reported. A little higher acne symptoms recurrence rate is apparent in the group of patients locally treated, as compared with that subjected to antibiotics-supported therapy. A statistically significant relationship between the therapy duration with the application of individual methods and the recurrence of illness has not been proven as yet. The value of p = 0,5245 proved that there existed no relationship between the diagnosis and a period of time elapsed--at the end of therapy--since the recurrence of acne disorders has been observed. The longest period of remission lasted when inversed acne had been recognized. Recurrence of the disease was observed the most rapidly in patients affected with acne conglobata. When examining the relationship between the duration of acne symptoms recurrence vs. treatment process, statistically significant differences amongst the individual applied methods were recorded. In the group A, recurrence of illness occurred at a higher rate. A statistically significant relationship between the recurrence of signs and symptoms and the initial diagnosis was excluded. A higher percentage of recurrence in the groups I, II and III has been reported as compared with group V. There is a certain relationship between the patient's age and the recurrence of the disease. It mainly referred to the elderly. Statistical analysis has proved that there existed significant relationship between the recurrence of the disease analysed and the patients' sex. A little higher rate of illness occurrence can be observed in a female group. The study revealed that recurrences in group of patients treated by locally active preparations were observed usually 6 months after the end of therapy.

Already, we know that this dermatosis occurs as often in male as in female, can be present in each age and has genethics background. Diagnostic and especially therapy of those skin diseases is very difficult and can make a lot of problems not only dermatologist. This paper presents the case of a 48-year-old man with diagnosed and histological confirmed Pityriasis rubra pilaris Devergie. Advantages of retinoids therapy were also discussed.

  AIM: The objective of the present paper is to assess side effects linked to the treatment of severe acne forms with administration of isotretinoin systemic and various drugs applied locally and/orAIM: The objective of the present paper is to assess side effects linked to the treatment of severe acne forms with administration of isotretinoin systemic and various drugs applied locally and/or systemic. MATERIALS AND METHODS: All the examinations were carried out on patients suffering from severe acne (n=206; 126 F and 80 M) hospitalized in Department of Dermatology, Silesian Medical School, in the years of 1991-2000, as well as amongst patients who continued their therapy in the outpatient clinic of the Dermatology Department during this period. RESULTS: When assessing the individual groups, the following conclusions have been drawn: lack of the relationship between the diagnosis and the therapeutical process in the group of patients locally treated and in the groups of sick persons cured by other methods; there exists a very significant relationship between the diagnosis and the therapeutical process in the group of patients to whom the isotretinoin preparation was administered and in the group of patients locally cured. The obtained results showed that the most frequently encountered diagnoses, i.e. acne conglobata and acne papulous-pustular were treated by the isotretinoin preparation and via local therapy, respectively. In the period of hospitalization, the value p=0.5570 testifies to lack of relationship between the number of side effects and the treatment period. All side effects occurred almost exclusively in patients cured by the isotretinoin preparation. The test value, p=0.0418 computed for all the periods, has revealed statistically significant relationships between the female/male sex and the side effects, most frequently reported in male patients. For the purpose of analysis the two (2) more often groups of symptoms were taken into consideration: dermal changes and biochemical disturbances and/or dysfunctions. The value p=0.5312 (p>0.05) excludes the existence of statistically significant relationship between the kind of a specific symptom and the therapy period. CONCLUSIONS: The performed medical examinations permitted to find out that side effects could be often recorded in a group of patients subjected to generally applied isotretinoin but it is never caused to stop therapy.

  We would like to present a selected case of 12-aged girl, with recognition of dermatomyositis (DM). At the age of 11 in the normally developing child, erythematous-oedematous changes have appeared onWe would like to present a selected case of 12-aged girl, with recognition of dermatomyositis (DM). At the age of 11 in the normally developing child, erythematous-oedematous changes have appeared on the face, particularly intensified in the vicinity of the orbital cavities (so called: pseudoglasses), as well as erythema and teleangiectasis on the dorsum of hands and small-sized diarthroidal joints (the Gottron's symptom). Subsequently, lower physical efficiency and distinctly weakness in the child's extremities occurred. In EMG (quadriceps muscle of the thigh) myogenous traits have been proven. Neurological examination revealed as follows: muscular weakness (adynamia), mainly lower limbs (grade 3 in the Lovett's scale, along with decreased loss of muscles tone), lack of the periosteal reflex near lower limbs, positive Gower's symptom and increased anterior spinal curvature. In the biochemical examinations accelerated erythrocyte sedimentation rate (ESR), and a rise of activity in muscles enzymes were stated. In the child's blood serum, we disclosed antinuclear antibodies ANA (type of granular luminescence, titre 160), to be rather evident to presence of autoimmunological process. During examinations of the musculocutaneus specimen, DM-markers have been detected. Capillaroscopy proved specific presence of numerous vessels, multiple capillary tubes, individual gemmated vessels and completely invisible dermatomyositous border. Patient was treated with per os sterid--Encorton at the initial dose of 2 mg/day, every other day during the lapse of 6 weeks to reach the normal CPK-activity, and consequently clinical picture under "on-line" surveillance, gradually reducing a specific medicine up to maintenance dose through 18 months. At present, the patient is subjected to check-up and monitoring by Neurological Outpatient and Rehabilitain Clinic for Children.

  Common acne, the most frequently occurring dermatosis of adolescence may lead to mentally unstable states, even depression, due to localization, its very prolonged duration, and in some patientsCommon acne, the most frequently occurring dermatosis of adolescence may lead to mentally unstable states, even depression, due to localization, its very prolonged duration, and in some patients leaving deforming cicatrices. The objective of the present paper is to answer the following questions: which treatment procedures were used for curing patients with different types of acne who were admitted to Department of Dermatology, Silesian Medical School, Katowice, Poland, in the years between 1991 and 2000. During 1991 to 2000, 206 patients were hospitalized with the diagnosis of acne. Mean age of patients was 25.51 years. The following types of acne were recognized: 108 patients--acne punctata, 82--acne conglobata, 4--acne fulminant, 4--papulous-pustular acne, 8--inverse acne, respectively. Analyses have been performed on the basis of case records related to patients admitted to Department of Dermatology, and registered at Outpatient Clinic in the years 1991-2000. It has been proven that there existed a substantial statistical difference between the age of patients affected by acne conglobata and inverse one and those suffering from acne conglobata and acne punctata. A significant difference in the age between the group of patients locally treated and the group of those cured by Roaccutane was observed. The results obtained point out that most frequently diagnosed cases: acne conglobata and acne punctata were treated by Roaccutane and locally, respectively. Results obtained revealed that the percentage of patients treated orally by retinoids showed a rising tendency, while a relatively constant percentage of local treatment methods (strongly marked in the late 1990s) and variable rate of administered antibiotics for curing acne was observed.

  Common acne is steatorrhoeic chronic disease, to which specific is, among others, the presence of blackheads, papulopustular eruptions, purulent cysts and cicatrices. Such hormonal factors belong to elements inherent in etiology of the affection. Sebaceous glands have cell receptors on their surface for androgens. In etiopathogenesis of common/simple acne, a decisive role is played by a derivative of testosterone, i.e. 5-alpha-dihydrotestosterone (DHT). However, some experts are of opinion that there is no correlation between the increased intensity of common acne and other symptoms of hyperandrogenism. Numerous authors assume, however, that common acne-affected patients may be sometimes subjected to intense reactions caused by sebaceous glands against physiological androgens concentrations. Naturally, estrogens can inhibit release of such androgens. Under physiological conditions, natural progesterone does not conduct to intensification of the seborrhea, but the activity of sebum secretion may be triggered off by its synthetic counterparts. Hormonal etiology can be very distinctly visible in the steroid, androgenic, premenstrual, menopausal acne, as well as in juvenile acne and acne neonatorum. In case of females affected by acne, hormonal therapy should be persistently supported and consulted with dermatologists, endocrinologists and gynecologists. Antiandrogenic preparations are applied, such as: cyproterone acetate concurrently administered with estrogens and, as well as not so frequently with chlormadinone acetate (independently or during estrogenic therapy).

  Retinoids--new agents derived from vitamin A have initiated a new era in therapy of many skin diseases. They are highly effective medicines used in the treatment of severe forms of psoriasis. Also they are administered in many forms of acne, as alternative drugs do not exist. Moreover, retinoids show antineoplastic activity. They are also effective in the symptomatic treatment of the congenital keratinization disorder syndromes. The recent development of retinoids combined with the growth of expertise with regard to the biological function and mechanisms of retinoid receptors, are likely to improve the present therapeutic methods and to reveal new potentials regarding the use of retinoids in the therapy of many skin disorders.

  Infectious non-venereal diseases are divided into contagious and non-contagious skin diseases, that have sometimes different and unexplained aetiology. Some diseases caused by viruses, fungi,Infectious non-venereal diseases are divided into contagious and non-contagious skin diseases, that have sometimes different and unexplained aetiology. Some diseases caused by viruses, fungi, bacteria, Candida albicans and parasites are the contagious skin diseases. The non-contagious diseases include: contact dermatitis, erythema fixum, lichen sclerosus et atrophicus, Reiter's disease and some pre-neoplasmatic lesions. The aim of the study was to demonstrate some skin lesions commonly located on external genital organs in both sexes, that should be differentiated with skin lesions in sexually transmitted diseases.

• [Assessment of efficacy of Diane-35 in androgenetic feminine alopecia]

  The aim of the study was to carry out clinical and trichological examination (trichogram and assessment of hair loss) before and after therapy in 25 women aged 31-35 years with androgenetic alopecia.The aim of the study was to carry out clinical and trichological examination (trichogram and assessment of hair loss) before and after therapy in 25 women aged 31-35 years with androgenetic alopecia. Diane-35 was given over 6-9 months once a day in a dose of 1 coated tablet beginning the therapy on the first day of menstrual cycle. After the administration of a whole package (1 blister) the drug was paused for 7 days which resulted in the occurrence of menorrhoea. The study showed that Diane-35 used for 6-9 months decreases clearly the loss of hair, hair thinning and seborrhoea.
