To diagnosis of the dry eye syndrome is based on the combination of clinical symptoms and clinical tests. These clinical tests evaluate tear clearance, tear stability, ocular surface integrity, tear osmolarity and conjunctival cytology. Measurement of tear osmolarity might provide a "gold standard" of diagnosis, but a practical tear osmolarity test is not yet widely available. Measurement of tear film instability by means of a TBUT test has good overall accuracy and may be more repeatable than many other diagnostic tests. The first step in managing the disease is to identify the underlying etiology and to try to eliminate it and/or treat it. Inflammation and the interruption of the inflammatory cascade seem to be the main focus in the treatment of dry eye, giving the anti-inflammatory therapy a new critical role.

It is a prevalent, multifactorial disease that is particularly frequent in elderly patients and women, especially in menopausal and postmenopausal period. Dry eye syndrome can be episodic with transient signs and symptoms or chronic with persistent signs and symptoms and is characterized by one or more of the following symptoms: burning, itching, foreign body sensation, soreness, dryness, photophobia, redness, and reduced visual acuity. The tear film instability of dry eye syndrome, which is accompanied by increased osmolarity of the tear film, causes inflammation and structural damage to the ocular surface. There are two major etiologic categories of dry eye syndrom: aqueous-deficient and evaporative. The most frequent classification of dry eye for practical clinical use is triple classification based on the ethiology, histopathological changes and severity of the disease.

The aim of the study was to evaluate the blood serum concentration of soluble forms of vascular cell adhesion molecule (VCAM-1), intercellular adhesion molecule (ICAM-1) and endothelial leukocyte adhesion molecule-1 (ELAM-1) in patients with type 1 diabetes mellitus without and with diabetic retinopathy. MATERIALS AND METHODS: The study was performed in 75 patients with type 1 diabetes mellitus, 35 without retinopathy (group 1) and 40 with retinopathy (group 2). Soluble forms of VCAM-1, ICAM-1 and ELAM-1 were determined by enzyme-linked immunosorbent assay (ELISA). RESULTS: The serum concentration of sICAM-1 and sELAM-1 were significantly elevated and the concentration sVCAM-1 was elevated but not significantly in diabetic patients when compared with control subjects. There was a significant difference in VCAM-1 concentrations between the control group and group 2 (965.9 +/- 229.0 vs. 1283.7 +/- 387.6 ng/ml, p < 0.05) and between group 1 and group 2 (1115.0 +/- 285.5 vs. 1283.7 +/- 387.6 ng/ml, p < 0.05). There were significant differences in sICAM-1 concentrations between the control group and group 1 (p < 0.05) and between the control group and group 2 (p < 0.05). Where was no significant difference in sICAM-1 concentration between group 1 and 2 (405.2 +/- 135.9 vs. 443.1 +/- 112.7 ng/ml, p = 0.08). ELAM-1 concentration was significantly elevated in group 2 (120.5 +/- 49.3 ng/ml) when compared with the control group (51.7 +/- 18.1 ng/ml, p < 0.005) and with group 1 (81.2 +/- 27.7 ng/ml, p < 0.05). CONCLUSIONS: The correlations found between sVCAM-1, sICAM-1 and sELAM-1 and the presence of retinopathy suggest that cellular adhesion and neovascularization may be linked processes.

  Background. Fractures of the condylar process of the mandible, whether isolated or coexisting with other injuries, are often difficult to manage. Treatment outcome in patients with mechanicalBackground. Fractures of the condylar process of the mandible, whether isolated or coexisting with other injuries, are often difficult to manage. Treatment outcome in patients with mechanical injuries of the temporomandibular joints and fractures of mandibular condylar process, which is an element of this joint, depends mostly when and how treatment is initiated. The goal of our study was to assess functional abnormalities in patients following fractures of the condylar process of the mandible in relation to the treatment method used. Material and methods. We analyzed patient documentation and the results of follow-up examinations of 147 patients hospitalized for fractures of the condylar process. This material included both isolated fractures of the condylar process and those accompanied by fractures of the body of the mandible. All mandibular body fractures were managed by means of multiplate osteosynthesis. Fractures of the condylar process were treated in a conservative-orthopedic manner or surgically, depending on the degree of shortening of the mandibular ramus. Anamnesis regarding pain and a functional examination of mandibular kinetics and audible effects gave a basis for assessing the function of temporomandibular joints following treatment. The Helkimo Dysfunction Index was used to assess changes in masticatory function and disorders in the temporomandibular joint. Results. Functional disturbances of the temporomandibular were observed in 38.1% of the examined patients. The most frequently observed symptoms of dysfunction were periodic pain and crackles on mandibular abduction, subluxation, deviation of the mandible, and slight limitation of jaw opening. The intensity depended mainly on when rehabilitation was commenced, and to a less degree on the type of treatment used. No joint dysfunctions were observed in patients of developmental age who sustained isolated condylar process fractures.

  Oxidative stress, are believed to be an important factor in the pathogenesis of age related diseases e.g. age related macular degeneration (AMD) and age related cataract. The is strong evidence suggesting that antioxidants can protect against development of this diseases, how ether experimental and clinical data have often been inconsistent. In this paper authors presents the review of the literature concerning on the connections between oxidative mechanisms and AMD and protective role of the antioxidative vitamin and other antioxidative factors supplementation in this disorder. This publication tries to estimate the anti-pericyte autoantibodies role in progression of diabetic retinopathy.

  PURPOSE: The aim of the study was to evaluate association of apo E polymorphism with AMD. MATERIAL AND METHODS: The included participants of this study were: 45 patients aged 60 to 71 (mean age 66.2PURPOSE: The aim of the study was to evaluate association of apo E polymorphism with AMD. MATERIAL AND METHODS: The included participants of this study were: 45 patients aged 60 to 71 (mean age 66.2 +/- 3.6), treated because of AMD. Genomic DNA was extracted from peripheral blood leukocytes, and the subsequent analysis of apo E genotypes was performed by polymerase chain reaction (PCR). RESULTS: In our study the allele frequencies in control group of patients weresimilar to the apo E allele distribution presented by other authors in Caucasian populations. We have not found significant differences between AMD and control group patients for apo (E) genotype or allele frequency. Compared with control group (epsilon4 -13.6%, epsilon2 -2.27%), the frequency of epsilon4 allele was not significantly decreased (11.11%) and epsilon2 allele frequency (7.77%) increased in AMD patients. CONCLUSIONS: The results of our present study have not indicate a significant association between AMD development and apo E allele frequency, in our studied population of AMD patients.

  PURPOSE: Recent studies have shown a relationship between elevated levels of Hcy and vascular disease including cerebrovascular accidents and myocardial infarctions. Elevated Hcy plasma is considered as an independent risk factor for atherosclerosis and trombosis. MATERIAL AND METHODS: This study included 30 patients with diagnosed exudative form of AMD. We evaluated plasma homocysteine levels, by high performance liquid chromatography and red blood cell folic acid, plasma folic acid and plasma vitamin B12 concentration by radioassay method. RESULTS: The plasma concentration of Hcy in AMD patients had significantly increased when compared with control group. We found not significant decrease of the plasma vitamin B12, red blood cell folic acid and plasma folic acid concentration in AMD group, when compared with the control group.

  Prolonged glucocorticoids administration is the most common cause of secondary osteoporosis. It is estimated that 30% to 50% of chronic glucocorticoids users experience vertebral or hip fractures.Prolonged glucocorticoids administration is the most common cause of secondary osteoporosis. It is estimated that 30% to 50% of chronic glucocorticoids users experience vertebral or hip fractures. The highest bone loss (up to 30% in some studies) is observed in the first six months of treatment. Only a minority of patients who take chronic glucocorticoids receive optimal osteoporosis diagnosis, prevention, and/or treatment. The aim of this paper is to present the pathophysiology of glucocorticoid-induced osteoporosis, as well as some guidelines on diagnostic, preventive and therapeutic strategies for this disorder in an effort to promote the greater awareness of it.

  INTRODUCTION: The aim of this study was to evaluate the blood concentration of hepatocyte growth factor (HGF) in patients at various stages of retinopathy. We hypothesised that the high level of HGFINTRODUCTION: The aim of this study was to evaluate the blood concentration of hepatocyte growth factor (HGF) in patients at various stages of retinopathy. We hypothesised that the high level of HGF found in diabetic patients may be an important marker of retinopathy progression and that HGF level may be an index of the risk of proliferative retinopathy. MATERIAL AND METHODS: The participants in the study were 76 patients with type 1 diabetes mellitus. Of these, 35 patients were without retinopathy and formed Group 1. Of the remaining 41 patients with retinopathy, 20 patients had non-proliferative diabetic retinopathy (NPDR) and formed Group 2, while 21 patients had proliferative diabetic retinopathy (PDR) and formed Group 3. We evaluated the concentration of HGF In the peripheral blood by an enzyme-linked immunosorbent assay. RESULTS: Mean serum concentrations of HGF in the control group were significantly lower than in the type 1 diabetic patients. We found a significant increase in HGF serum concentrations in diabetic patients with PDR compared with the control group. Mean serum HGF concentrations were significantly higher in diabetic subjects with PDR than in diabetic patients without retinopathy. CONCLUSION: HGF concentration is increased in patients with type 1 diabetes mellitus with proliferative retinopathy, and concentrations increase with the progression of retinopathy, suggesting that HGF plays a role in the pathogenesis of proliferative diabetic retinopathy.

  A multitude of mechanisms have been implicated in the pathophysiology of epilepsy. Objective: To assess mean daily plasma concentrations of ACTH, cortisol, DHEAS, leu-enkephalin, and beta-endorphinA multitude of mechanisms have been implicated in the pathophysiology of epilepsy. Objective: To assess mean daily plasma concentrations of ACTH, cortisol, DHEAS, leu-enkephalin, and beta-endorphin in epileptic patients with complex partial seizures evolving to tonic-clonic in relation to frequency of seizure occurrence (groups with seizure occurrences - several per week and several per year) and duration of the disease (groups less than and more than 10 years). We decided to analyse mean daily values of beta-endorphin and leu-enkephalin because of significant differences in concentrations of these substances in blood during the day. The study was performed on 17 patients (14 males + 3 females; mean age 31.8 yrs) treated with carbamazepine (300-1800 mg/day). The control group consisted of six age-matched healthy volunteers. Blood was collected at 8 a.m., 2 p.m., 8 p.m., and 2 a.m. Intergroup analysis was performed with the use of ANOVA Kruskal-Wallis test. Mean daily concentrations of ACTH and cortisol in the blood of the patients with epilepsy were higher in comparison with those of the healthy volunteers, independently of the frequency of seizures and duration of the disease. Mean daily concentrations of beta-endorphin in the blood of the patients with epilepsy were higher in the groups of patients with more severe clinical course of disease (with more frequently occurring epilepsy seizures and longer duration of the disease) in comparison with healthy subjects. Mean daily concentrations of leu-enkephalin in the blood of the patients with epilepsy were higher in the group of patients with short duration of the disease in comparison with the group with long duration of the disease. 1. Pituitary-adrenal axis hyperactivity is observed in patients with clinically active epilepsy, independently of the frequency of seizures and duration of the disease. 2. Changes in endogenous opioid system activity are related to the clinical activity of epilepsy - beta-endorphin concentrations are connected with frequency of seizures and duration of the disease and leu-enkephalin concentrations with duration of the disease. 3. Endogenous opioid peptides might take part in the neurochemical mechanism of human epilepsy. (Pol J Endocrinol 2010; 61 (1): 103-110).

  Leptin and adiponectin are involved in the pathogenesis of several autoimmune diseases. Very little is known about adipocytokine production in autoimmune thyroid diseases. Interleukin-6 (IL-6) playsLeptin and adiponectin are involved in the pathogenesis of several autoimmune diseases. Very little is known about adipocytokine production in autoimmune thyroid diseases. Interleukin-6 (IL-6) plays an important role in the inflammatory and autoimmune processes. The aim of this study was to assess the serum levels of leptin, adiponectin, and IL-6 in postmenopausal euthyroid women with Hashimoto's thyroiditis and compared them with concentrations in control women. Ninety-eight euthyroid women with Hashimoto's thyroiditis were enrolled in the study. The diagnosis was confirmed with elevated thyroid peroxidise autoantibody (TPOAb) levels in serum and typical hypoechogenic pattern on thyroid ultrasound. The control group, matched for body mass index (BMI), consisted of 105 healthy postmenopausal euthyroid women. Serum levels of leptin, adiponectin, IL-6, thyroid-stimulating hormone (TSH), free thyroxine (fT(4)), and TPOAbs were determined. When compared with controls, the women with Hashimoto's thyroiditis were characterized by significantly elevated serum concentrations of IL-6, whereas concentrations of leptin and adiponectin were not different. Hashimoto's thyroiditis patients had significantly higher serum levels of TSH than the controls. The simple linear regression analyses of the Hashimoto's thyroiditis group and all of the studied women indicated that serum leptin levels correlated positively with BMI, waist to hip ratio (WHR), TSH, and IL-6 and negatively with adiponectin. No correlation was observed between serum adiponectin and TSH, fT(4), or TPOAbs. There were no associations between serum IL-6 levels, TPOAbs, and TSH levels; however, positive correlations between IL-6 and BMI, WHR, and leptin were observed. TSH correlated positively with leptin, age, and TPOAbs. Hashimoto's thyroiditis is characterized by an increased production of IL-6 but does not have a direct influence on leptin or adiponectin serum levels. The correlations between TSH and leptin demonstrated in this study highlight the need for future investigations. (Pol J Endocrinol 2010; 61 (1): 112-116).

  BACKGROUND: The soluble forms of vascular cell adhesion molecule-1 (sVCAM-1) and intercellular adhesion molecule-1 (sICAM-1) have been found to be increased in the blood of patients with GravesBACKGROUND: The soluble forms of vascular cell adhesion molecule-1 (sVCAM-1) and intercellular adhesion molecule-1 (sICAM-1) have been found to be increased in the blood of patients with Graves disease. The aim of this study is evaluation of the serum concentrations of soluble forms of adhesion molecules ICAM-1 and VCAM-1 in patients with thyroid-associated orbitopathy (TAO) before and after methylprednisolone treatment. MATERIAL AND METHODS: The study was performed in 40 Graves disease, hyperthyroid and euthyroid patients with a clinically active form of TAO. Serum concentrations of sVCAM-1 and sICAM-1 in TAO patients were determined by enzymelinked immunoabsorbent assay (ELISA) before and after intensive pulse methylprednisolone treatment. RESULTS: We did not find any significant changes in the studied parameters between TAO patients with hyperthyroidism and those with euthyroidism. The serum concentrations of sICAM-1 and sVCAM-1 were significantly increased in patients with TAO before methylprednisolone therapy when compared with the control group. After treatment serum concentrations of sICAM-1 and sVCAM-1 decreased significantly but were still significantly higher than for the control group. CONCLUSION: From the results obtained we can conclude that Graves orbitopathy itself but not thyroid function is probably responsible for the elevated level of the adhesion molecules studied.
