  • Article: Successful treatment of disseminated nocardiosis complicated by cerebral abscess with ceftriaxone and amikacin: case report.

    ABSTRACT: We report the case of an 85-year-old female patient who suffered from disseminated Nocardia asteroides infection complicated by a cerebral abscess. Treatment with amikacin for 2 weeks and ceftriaxone for 6 weeks led to complete recovery, and there was no recurrence of disease over a follow-up period of 12 months after therapy. The use of ceftriaxone in combination with amikacin might significantly shorten the duration of treatment for patients with disseminated nocardiosis. This combination of antibiotics merits further investigation with use of a larger sample of patients.

  • Article: [Endocarditis prophylaxis with amoxicillin, clindamycin or erythromycin? Serum bacteriostatic and bactericidal effect against Streptococcus viridans].

    ABSTRACT: The effectiveness of antibiotic prophylaxis against endocarditis was assessed by testing the bactericidal and bacteriostatic action in serum of 12 healthy volunteers who had taken the recommended antibiotics according to laid down guidelines. Blood was obtained from these subjects every two hours for 12 hours after oral intake of amoxicillin (3 g as a single dose), clindamycin (600 mg a single dose) or erythromycin (1.5 g plus another 0.5 g after six hours), the serum being tested against three Strep. viridans strains. Two of the three strains were "tolerant", i.e. in relation to the minimal inhibition concentration (MIC) their growth was inhibited, but--in relation to the minimal bactericidal concentration (MBC)--they were not killed. A bacteriostatic effect by amoxicillin and clindamycin was demonstrated by a micromethod against all three strains during the 12-hour period. But erythromycin did not achieve bacteriostasis in all serum samples. A bactericidal effect was demonstrated only in those samples that contained amoxicillin, and then only against the non-tolerant of the three strains. These results support the view that amoxicillin and clindamycin are effective in the prophylaxis against Strep. viridans bacteraemia because of their constant bacteriostatic effect, as measured in serum.

    Article: Single-dose ciprofloxacin at 100 versus 250 mg for treatment of uncomplicated urinary tract infections in women.

    ABSTRACT: Two single-dose regimens of ciprofloxacin, 100 and 250 mg, were compared in the treatment of uncomplicated urinary tract infections in women. Cure rates 5 days after therapy did not significantly differ, being 16 of 19 (84%) with the 100-mg dose and 17 of 19 (89%) with the 250-mg dose. Ciprofloxacin was well tolerated.

  • Article: Effect of coupling the breathing- and cycling rhythms on oxygen uptake during bicycle ergometry.

    ABSTRACT: The influence of the degree of coupling between the breathing and cycling rhythms (K) on oxygen uptake (Vo2) was examined in 30 volunteers. They cycled on an ergometer with a load equal to 50% of their work capacity 170 in two experimental runs with spontaneous breathing rhythm, and in a further two runs with acoustically triggered breathing. K was continuously ascertained. Vo2 and other respiratory parameters were measured by an automatic "breath-by-breath analysis" system. In 16 subjects, Vo2-differences between runs were correlated with the differences in K. In the majority of these subjects (12), Vo2 decreased significantly with increasing K. In 14 subjects, Vo2-and K-variations within individual runs were analyzed. Phases with higher K were regularly accompanied by a decrease in Vo2. It is concluded that coupling the breathing and cycling rhythms reduces Vo2 for a given moderate work load, although the magnitude of the Vo2-reduction varies considerably between individuals.

    Article: Ciprofloxacin-induced hematuria.

    ABSTRACT: We used ciprofloxacin, a quinolone-derivative, to treat a lung infection due to Pseudomonas aeruginosa in an adult cystic fibrosis patient. On three different occasions the use of ciprofloxacin was associated with the development of an asymptomatic hematuria with red blood cell casts. The mechanism responsible for this hematuria is presently unknown, but clinicians should be aware of this potential adverse effect of ciprofloxacin.
