  • Article: Postoperative opacification of the peripheral optic region and haptics of a hydrophilic acrylic intraocular lens: case report and clinicopathologic correlation.

    ABSTRACT: We report the first case of opacification of the BioComFold 92S (Morcher GmbH) intraocular lens. The opacification pattern is unique, presenting mainly in the periphery of the optic and the haptics of the lens, leaving the central portion of the optic clear. The clinical course of this opacification pattern is not observed typically with other hydrophilic acrylic lens designs and requires further study.

  • Article: Quality of vision following clinically successful penetrating keratoplasty.

    ABSTRACT: To evaluate visual function following clinically successful penetrating keratoplasty (PKP). Department of Ophthalmology, Ege University, School of Medicine, Izmir, Turkey. Patient group (PG) included 9 patients (12 eyes) who had clinically successful PKP in our department. The control group (CG) included 12 people (18 eyes) who had no ocular disease other than refractive errors. Those with a visual acuity level less than 20/25 were not included in the study. Contrast sensitivity levels and light threshold values of the central retina were measured; scanning-slit corneal topography-pachymetry and aberrometric analysis were performed. There were no statistical difference in terms of age (32.55 years +/- 9.25 (SD) in PG, 36.75 +/- 5.85 years in CG; P =.53), cylinder power in plus form (2.60 +/- 1.25 diopter (D) in PG, 2.79 D +/- 2.51 D in CG; P =.88), and spherical equivalent of refractive errors (-3.66 +/- 3.57 D in PG, -5.52 +/- 3.37 D in CG; P =.29) between the PG and CG. Cambridge low-contrast grating scores were 96.5 +/- 41.1 in grafted eyes and 148 +/- 27.7 in CG (P =.004). Central retinal light sensitivity was measured as 29.91 +/- 2.39 db in PG and 33.08 +/- 1.56 db in CG (P =.001). In corneal topographic analysis, mean kappa intercept was 0.69 +/- 0.37 mm in PG and 0.55 +/- 0.24 mm in CG (P =.20). Lower-order Zernike root mean squares (RMS) were 7.30 +/- 3.89 microm for PG and 8.58 +/- 3.46 microm for CG (P =.37). However, higher-order Zernike RMS were 2.15 +/- 0.78 in PG and 0.38 +/- 0.10 in CG, which is a statistically significant difference (P<.001). Even though the clinically successful PKP patients have correctable amount of spherocylindrical refractive errors with spectacle lenses, they still have reduced visual quality because of the significantly high amount of higher- order aberrations when compared with naturally occurring refractive errors.

  • Article: Conventional buckling surgery or primary vitrectomy with silicone oil tamponade in rhegmatogenous retinal detachment with multiple breaks.

    ABSTRACT: There is controversy about the most appropriate operating methods for complicated rhegmatogenous retinal detachment (RD) including multiple tears, and surgical techniques may be changed according to the preference of the surgeon. In this retrospective study, we compared the surgical results of conventional buckling surgery and vitrectomy with silicone oil tamponade for rhegmatogenous (RD) with multiple breaks. Thirty patients who underwent scleral buckling surgery (group 1) and 22 patients who underwent pars plana vitrectomy with silicone oil tamponade (group 2) as the primary surgery for rhegmatogenous RD with multiple breaks were included in this study. The follow-up period was longer than 6 months after surgery. The anatomical success rates and complications were evaluated for both groups. Retinal reattachment was achieved in 24 of 30 eyes (80%) in group 1 and in 20 of 22 eyes (90.9%) in group 2 after the initial surgery. In group 1, subretinal hemorrhage developed due to the drainage of subretinal fluid in 2 eyes (6.6%) intraoperatively. Elevated intraocular pressure (3.3%), ocular motility disturbances (13.2%), and proliferative vitreoretinopathy (3.3%) were seen in the postoperative period. In group 2, iatrogenic breaks (7.3%) and lens damage (9.09%) occurred during the operation. Macular pucker (4.5%), postoperative cataract progression (22.7%), ocular hypertension (9.09%) and PVR (9.09%) were noted postoperatively. Both surgical procedures can achieve favorable and comparable anatomic outcomes in the majority of patients in the treatment of RD with multiple breaks. Intra-and postoperative complications are different in the two procedures.

  • Article: Hydrophilic acrylic intraocular lens optic and haptics opacification in a diabetic patient: bilateral case report and clinicopathologic correlation.

    ABSTRACT: To report clinicopathologic and ultrastructural features of two opacified single-piece hydrophilic acrylic intraocular lenses (IOLs) explanted from a diabetic patient. Interventional case report with clinicopathologic correlation. A 64-year-old white female underwent phacoemulsification and implantation of a single-piece hydrophilic acrylic lens (SC60B-OUV; Medical Developmental Research, Inc., Clear Water, FL) in October 1998 in the left eye and in July 1999 in the right eye. The best-corrected visual acuity after surgery was 20/60 in the left eye and 20/50 in the right eye. The patient had a marked decrease in visual acuity in June 2000 as a result of a milky, white opalescence of both lenses. Intraocular lens explantation and exchange was performed in both eyes and the explanted IOLs were submitted to our center for detailed pathologic, histochemical, and ultrastructural evaluation. They were stained with alizarin red and the von Kossa method for calcium, and also underwent scanning electron microscopy and energy dispersive radiograph spectroscopy to ascertain the nature of the deposits leading to opacification. Documentation of calcium deposits confirmed by histochemical stains and surface analyses. Opacification of the IOL was found to be the cause of decreased visual acuity. The opacification involved both the IOL optic and the haptics in the left eye and was confined to the IOL optic in the right eye. Histochemical and ultrastructural analyses revealed that the opacity was caused by deposition of calcium and phosphate within the lens optic and haptics. There are two features that distinguish this case from those reported earlier. This is the first clinicopathologic report of lens opacification that has involved completely the lens optic and the haptics. Second, these two explanted IOLs document the first bilateral case. This process of intraoptic and haptic opacification represents dystrophic calcification of unknown cause. Diabetic patients appear to be more severely and more often affected by lens opacification. Long-term follow-up of diabetic patients implanted with this IOL design should be maintained by surgeons and manufacturers.

  • Article: Aqueous infiltration into an implantable miniaturized telescope.

    ABSTRACT: The implantable miniaturized telescope (IMT) is the first intraocular magnifying system proposed for optical correction in patients with age-related macular degeneration (ARMD). The optical component is embedded in a carrying device designed as an intraocular lens that is implanted after cataract surgery. In this study, we report findings on an IMT that was explanted because of aqueous infiltration into its optic and describe the configuration of this device and the surgical technique required for its implantation. The patient, a 75-year-old male with bilateral cataract and nonexudative ARMD, underwent phacoemulsification with implantation of an IMT in the right eye. The rigid device, weighing 46.1 mg in aqueous, has an overall diameter of 13.5 mm and requires implantation through a large limbal incision. It is fixated at the 6 to 12 o'clock meridian. Follow-up examination revealed the presence of numerous droplets inside the IMT optic. The device was explanted and sent to our center for evaluation. A large fissure was found on the carrying device. However, it was unlikely the site for aqueous infiltration. Microdefects at the level of the sealing of the optical cylinder appeared to provide the opening for the inflow of aqueous that formed droplets. Based on the findings of this report the manufacturer has modified the sealing technique to avoid this complication. Current clinical trials are now ongoing to assess the efficacy of this device in providing visual rehabilitation for ARMD patients.
