• [Experiences with treatment of extended laryngo-tracheal stenosis with employment of transversal resection of trachea]

  The aim of this study is to evaluate the results obtained in Department of Otolaryngology et Oncology with transversal resection of trachea acc. to Pearson technique for postintubation stenosis.The aim of this study is to evaluate the results obtained in Department of Otolaryngology et Oncology with transversal resection of trachea acc. to Pearson technique for postintubation stenosis. METHODS: from January 2000 to January 2008 ten patients with subglottic postintubation stenosis underwent surgical treatment, using Pearsons method of operation with preservation of recurrent nerves and no postopereative tracheostomy. Mean stenosis length was 2 cm, the length of of airway resection was 1,5-4 cm. RESULTS: There was no surgical or postoperative complications, all the patients get totally health, no restenosis was observed. The results confirm that the Pearsons method of operation is an adequate treatment for subglottic stenosis.

  Fungal infections of the ear and paranasal sinus, are recognised quite rarely, but they make sometimes, for many reasons, very important diagnostic-therapy issue. Cantagion may developes near everyFungal infections of the ear and paranasal sinus, are recognised quite rarely, but they make sometimes, for many reasons, very important diagnostic-therapy issue. Cantagion may developes near every paranasal sinus. Facilities to invasion give every states, which weaken general and punctual immunity (like immunosupression, chemo-sterydotherapy, blood disease, pregnancy and HIV). After penetrated organism, course of infections, in case of type of patogens, trim of the patient and localization, can have without symptoms, sharp, chronic or fulminant shape. The hardest course with the highest mortality occur in the cases of mucormycosis and aspergillosis. Actually curiosity occur cases of fungal infections, since this times consider to be unpathogenic for humans. For treatment of sinus mycosis in majority chirurgical treatment is required. In addition or the sake of morphological differentiation fungal, in their developing cycle, treatment mycosis recommends serious difficulty, extra factor, which impeds therapy, is a must of prolongely antifungal treatment and repeated (to total elimination) remove hyphae from sinus, which stay sometimes even for week. Progress of the mycotic infections in paranasal sinus, remain bacterial infections and is often reason of bad diagnosis and incorrect treatment.

• [Chondroma of the larynx].

  Chondroid tumors of the larynx are extremely rare and their develope and symptoms are difficult to diagnosis. The importance of well radiological diagnosis prior chirurgical treatment of chondroidChondroid tumors of the larynx are extremely rare and their develope and symptoms are difficult to diagnosis. The importance of well radiological diagnosis prior chirurgical treatment of chondroid tumors of the larynx is empasised. Conservative surgical management when possible, is advocated to preserve the larynx. We presented a case of chondroma of cricoid cartilage developing in 49-year old woman with 4 year history of hoarsness, treated by surgical management - laryngofissure. During 36 months follow up no reccurency is observed. In the paper review of literarature is presented.

The incidents of undesires outcomes during tracheostomy can not be exactly predictet because interactions of the above issues. This paper consider some of difficulties observed in our department from January 2000 to July 2007. Tracheotomy is usssually a safe procedure, but complication ranging could be high in the following conditions: morbid obesy, a short and thick neck, cervical stiffness, tumors and infiltrantions of the neck, thyroid gland tumors.

• [Carcinosarcoma arising from a preexisting pleomorphic adenoma--true malignant mixed tumor of the parotid gland]

  Carcinosarcoma (true malignant mixed tumor) is rare in salivary gland neoplasms. Even rarer is a carcinosarcoma arising from a preexisting pleomorphic adenoma. Case: a 49-year-old woman was admittedCarcinosarcoma (true malignant mixed tumor) is rare in salivary gland neoplasms. Even rarer is a carcinosarcoma arising from a preexisting pleomorphic adenoma. Case: a 49-year-old woman was admitted with left neck pain. Computed tomography and USG-examination revealed a mass originating from left parotid gland. The patient undergone total parotidectomy with facial nerve preservation and then irradiation and chemioteraphy. Clinical and histologic date revealed it had developed from a preexisting pleomorphic adenoma. Conclusion: microscopic examination showed and neoplasm comprised of sarcomatous and carcinomatous cells. This tumor seemed to consist of two histogenetically different populations of cells. We also review the literature and dicuss the histogenetic origin of the carcinosarcoma of salivary gland.

  In most laryngological cancer patients chronic pain can be successfully treated with pharmacological drugs, using different combinations of analgetic drugs with adjuvant analgetics. However chronic pain, mainly cancer-related pain, what lost its physiological alerting function should be especially sufficient treated to avoid patient degradation of social and professional position, psychical disorders and performance status. The understanding of cancer-related mechanisms and ethiopathogenesis is a cornerstone of successful therapy. The aim of this study is to summarize data of mechanisms of cancer-related pain and methods of treatment of chronic, neoplasmatic "total pain", using orders of analgetic ladder and coanalgetics. Dynamic characteristic of pain in laryngological patients with malignant neoplasms localized in head and neck region require very exact, individual monitoring and treatment.

  This study is aimed an objective and subjective evaluation of local changes after operations of malignant tumours of the tongue and the bottom of the oral cavity. THE MATERIAL AND METHODS: we haveThis study is aimed an objective and subjective evaluation of local changes after operations of malignant tumours of the tongue and the bottom of the oral cavity. THE MATERIAL AND METHODS: we have examined 74 patients (70 men and 4 women) treated in Department of Otolaryngology of the University School of Medical Sciences in Poznań with recognized of malignant tumour of the tongue and floor of the mouth. On the base of subjective and objective ultrasound and x-ray examination with use of contrast and stereognostic test it was evaluated movability, alteration of articulation zones, regularity of swallowing acts, sense of taste and efficiency of stereognosion. In all of the patients we found orders in swallowing, speech and taste dependen on the kind of the operation.

• [Clinical effectiveness of betahistine in monotherapy of vertigo for different etiology]

  Vertigo is common symptom in clinical practice and among cerebrovascular and otological diseases. The aim of this clinical study was to evaluate the effect of betahistine dihydrochloride (Betaserc)Vertigo is common symptom in clinical practice and among cerebrovascular and otological diseases. The aim of this clinical study was to evaluate the effect of betahistine dihydrochloride (Betaserc) on patients with long lasted dizziness. Enrolled in the study were 33 parients at the age between 32 and 80 years whom were treated 16 mg doses of betaserc three times daily et the time 12 weeks. The methods of following investigation was clinical examination, subjective and objective examination and carry out individual questionnaires once a four weeks. On the basis of experiment it was showed that a distinct clinical improvement in the group of 33 patients was in above 66% patients and medication is well tolerated and suitable for long-term treatment.

  Deep neck infection is relatively rare but potentially life threatening complication of common oropharyngeal infections. This retrospective study was aimed at analyzing the occurrence ofDeep neck infection is relatively rare but potentially life threatening complication of common oropharyngeal infections. This retrospective study was aimed at analyzing the occurrence of complications, diagnostic methods and proper management of deep neck infection. A review was conducted in 32 cases who were diagnosed as having deep neck infection from 1995 to 2005. The causes of deep neck infections were tonsillitis (16 cases), tooth diseases (6 cases), paratonsillar abscess (4 cases), parotitis (1 case), pussy lymphonodes after tonsillectomy (2 cases), pussy congenital neck cyst (1 case), chronic otitis media (1 case), parotitis (1 case), foreign body of the esophagus (1 case). All the puss bacterial cultivation were positive. All the patients were treated by different ways of chirurgical drainage and use of large dosage of antibiotics. Deep neck infection should be suspected in patients with long lasting fever and painful swelling of the neck and treatment should begin quick as possible.

• [Rare case of schwannoma of the sinonasal tract]

A long, above 2-year history of symptoms was emphasized. The magnetic resonance findings and metod of treatment modo Denker was described.

  The etiology and pathogenesis of median rhomboid glossitis have been subjects of controversial discussion for a long time. In the past it was thought to be a developmental defect involving theThe etiology and pathogenesis of median rhomboid glossitis have been subjects of controversial discussion for a long time. In the past it was thought to be a developmental defect involving the tongue. This condition was believed to have been caused by the failure of the embriologic tuberculum impair to be covered by the lateral processes of the tongue. In the recent years the possible role of Candida albicans has been stressed. It is proposed that Actinomyces like Candida induces pseudoepitheliomatous hyperplasia of the mucosa of the tongue and inflammatory hyperplasia of the underlying connecting tissue, resulting in the characteristic elevated lesion. In the paper we present a case of 63 years old female with median rhomboid glossitis and methods of its treatment. The importance of tobacco smoking and denture wearing in the etiology of median rhomboid glossitis has been evaluated.

  The aim of the study was to assess the rehabilitation results in visually impaired patients with cochlear implants. Material consisted of 9 patients (2 with prelingual deafness, 3 with perilingualThe aim of the study was to assess the rehabilitation results in visually impaired patients with cochlear implants. Material consisted of 9 patients (2 with prelingual deafness, 3 with perilingual deafness, 4 with postlingual deafness) from 244 implanted persons in Poznań. METHODS: In all the patients the rehabilitation results were evaluated using Test of Auditory Perception of Speech (TAPS) and screening test. RESULTS: In the group of patients with pre- and perilingual deafness a little bit slower progress of rehabilitation was found, according to 117 persons with prelingual deafness and 34 persons with perilingual deafness without vision disorders. Within the group of patients with postlingual deafness important progress of rehabilitation was observed. Two brothers with Usher syndrome 1-2 months after speech processor switch-on reached the highest level of test, they could communicate with family. One person after 5 months of rehabilitation could talk by phone without any problems. In all implanted visually impaired patients improvement of auditory perception of speech was observed.

  All cases of malignant tumours of the pharynx (excluding tumours of the tongue) were analyzed based on the retrospective data from the 5 clinical centers in Poland from the years 1992-2001. AAll cases of malignant tumours of the pharynx (excluding tumours of the tongue) were analyzed based on the retrospective data from the 5 clinical centers in Poland from the years 1992-2001. A considerable--over 50% increase in morbidity was found out regarding particularly tumours of the oropharynx. In the analyzed material there is a predominance of males (3:1) and the peak of morbidity significantly falls. The subjects with advanced tumours of the pharynx made 26% of the studied group. The most common histopathological diagnosis was squamous cell carcinoma and the most common site--the oropharynx, in particularly the tonsil. The data the center in Katowice, where tumours of the hypopharynx have been predominant in the last few years, may point a similar tendency in the other centers in the future.

• [Fatal necrotizing mediastinitis as a complication of peritonsillar abscess].

  In the paper showed the case of fatal necrotizing phlegmon of the neck and descending mediastinitis in 48 year old male patient with primary peritonsillar abscess. On the base of literature it wasIn the paper showed the case of fatal necrotizing phlegmon of the neck and descending mediastinitis in 48 year old male patient with primary peritonsillar abscess. On the base of literature it was showed the etiology, pathomechanism of the evolution of this heavy complication and surgical and pharmacological methods of its treatment. In spite of prompt wide incision and drainage of phlegmon of the neck and mediastinum and intensive treatment in intensive care unit conditions patient died in the image of septic shock and pus changes in the mediastinum, heart and lungs, what was showed on the base of post-mortum examination.

  We present an experience with transversal tracheal resection with end-to-end anastomosis in 3 patients with extensive subglottic stenosis because of prolonged intubation. All the patients wereWe present an experience with transversal tracheal resection with end-to-end anastomosis in 3 patients with extensive subglottic stenosis because of prolonged intubation. All the patients were treated chirurgically based on the Pearson's method. In order to attein the fixation of tracheal stumbs, poroplastic splint was applied into the lumen of the trachea and patency of airways was protected by tracheotomy.There were no major complications during treatment and time of the treatment was significally shortening.

  In this paper the authors present the dynamics of habituation of reaction and perception in tinnitus patients. All patient with subjective chronic tinnitus were diagnosed and treated using TRT methodIn this paper the authors present the dynamics of habituation of reaction and perception in tinnitus patients. All patient with subjective chronic tinnitus were diagnosed and treated using TRT method according to Jastreboff's neurophysiological model. 110 patients was estimated during last 4 years treated in ENT Rehabilitation Center in Poznań. The authors present the most frequently troubles in every day life caused by tinnitus and also the dynamics regression of complaints during period of treatment. Speed of disappearance of suffering is very important for all patients already in the beginning of cure.

• [Case of adenoid cystic carcinoma of the nose with rapid course]

  A case of adenoid cystic carcinoma in 41-year old man, localised on nasal septum was described. The course of disease was very unsatisfactory--many local recurrences, lymph node metastases and in theA case of adenoid cystic carcinoma in 41-year old man, localised on nasal septum was described. The course of disease was very unsatisfactory--many local recurrences, lymph node metastases and in the last period generalisation of the neoplasms. The period between diagnosis and the time of death was only 8 months in spite of repeated operations of the tumours local recurrences and lymph nodes metastases as well as radio- and chemotherapy.

  The introduction of surgical lasers into microsurgery of the larynx has made the resection of the posterior vocal cord with or without the arytenoid cartilage possible. This method of surgicalThe introduction of surgical lasers into microsurgery of the larynx has made the resection of the posterior vocal cord with or without the arytenoid cartilage possible. This method of surgical treatment allows one to carry out a fast, non-open larynx procedure and practically bloodless operation with minimal postoperative trauma and edema of the surrounding tissues. Since November 1990 at the Clinic of Otolaryngology of the University School of Medical Sciences 48 arytenoidectomies (40 on the right side, 8 on the left side), 19 partial chordectomies and 25 Kashima operations. All surgical treatment were performed by means of CO2 laser in patients with bilateral paralysis of the vocal cords. In all patients postoperative recovery was correct and breathing difficulties were not observed after extubation. Laryngoscopic control examinations were performed a day after operation and a wide lumen of air through the operative field was observed. At the control examination, narrowing of the lumen of the larynx was not observed, the healing process of the operation area was without granulation. Laser arytenoidectomy allows good results of the breathing and phonation function. No changes of granulation proliferation were observed at the side of the CO2 laser treatment. Lack of reaction to laser beam there was possibility to use endoscopic procedures in patients who did not undergo a tracheotomy.
