• Application of optical coherence tomography in glaucoma suspect eyes.

    The aim of the study was to compare the optical coherence tomography (OCT) parameters of the optic nerve head (ONH) and retinal nerve fibre layer (RNFL) and to identify which measurements are best  The aim of the study was to compare the optical coherence tomography (OCT) parameters of the optic nerve head (ONH) and retinal nerve fibre layer (RNFL) and to identify which measurements are best able to differentiate between normal and glaucoma suspect eyes.   The study included 27 eyes with ocular hypertension (OHT), 33 eyes with pre-perimetric glaucoma (PG), 30 perimetrically unaffected eyes of patients with glaucoma in the fellow eye (FE) and 58 eyes of age-matched normal volunteers. All subjects underwent a complete eye examination with standard automated perimetry, optic disc photography and OCT imaging. Peripapillary 'fast RNFL thickness scans' and 'fast optic disc scans' were performed with time-domain OCT. The ONH and RNFL parameters were compared among the four study groups. The ONH and RNFL parameters were examined alone and then combined via four linear discriminant functions (LDF): LDF 1, the optimal combination of ONH parameters; LDF 2, the optimal combination of RNFL parameters; LDF 3, the optimal combination of both ONH and RNFL parameters; and LDF 4, the optimal combination of the best 11 parameters. The areas under the receiver operating curves (AUC) and the sensitivity at fixed specificity of at least 80 and 95 per cent were calculated for single parameters and LDF combinations and then compared. The best 11 parameters were selected based on their AUC values.   Comparative analysis of OCT parameters revealed statistically significant differences in all seven ONH parameters in both PG and FE groups (and only in one ONH measurement in the ocular hypertensive group) when compared with normal eyes. Most of the RNFL parameters demonstrated statistically significant differences in all of the study groups when compared with the control group. The max-min parameter (0.835), inferior quadrant (0.833) and average RNFL thickness (0.829) obtained the highest AUC values in the whole glaucoma suspect group. The rim area had the best diagnostic accuracy among the ONH parameters (AUC = 0.817). The AUC values of the four LDF were: 0.825 (LDF 1), 0.882 (LDF 2), 0.902 (LDF 3) and 0.888 (LDF 4). Statistically significant differences were found between the AUC values of the single best ONH and RNFL parameters and LDF 3 and LDF 4.   In the present study, RNFL parameters presented with better discriminatory abilities than ONH parameters in the OHT and FE groups. The ONH parameters demonstrated better diagnostic precision in differentiating between PG and normal eyes. The average RNFL thickness, max-min parameter and inferior quadrant RNFL thickness had the best abilities among single OCT measurements for discriminating between glaucoma suspect (including all ocular hypertensive, PG and FE eyes) and normal eyes. The combination of RNFL parameters only or both ONH and RNFL parameters, using linear discriminant analysis, provided the best classification results, improving the diagnostic accuracy of the instrument.

  Endotoxins have been proved to be responsible for acute anterior uveitis (AAU) in animals in a well-established experimental model of endotoxin-induced uveitis (EIU). The purpose of our study was the detection of antibodies against endotoxins of selected enterobacteria in the serum of patients with idiopathic AAU and searching for correlations between the levels of these antibodies and the presence of HLA-B27 antigen as well as characteristic signs of EIU such as bilaterality and the absence of spontaneous recurrences of the disease. Reactions of serum IgG antibodies with lipopolysaccharides (LPSs) of Escherichia coli O1, E. coli O10, E. coli O111, E. coli J5, and Klebsiella pneumoniae O3 were determined for 60 patients with idiopathic AAU and 40 healthy volunteers. The presence of HLA-B27 antigen in patients was determined. Documentation of the frequency of recurrences of AAU during a follow-up period of 8 years was collected. We have observed that the sera of patients with a first attack of AAU reacted stronger with the LPS of K. pneumoniae O3 than the sera of patients with relapse of the disease. Patients with bilateral AAU had markedly higher levels of antibodies against four of the five used LPSs than patients with one eye involved. A multiply comparison showed higher levels of IgG reacting with LPS of E. coli O111 in patients with bilateral eye inflammation admitted with the first attack of AAU comparing to controls. The incidence of recurrent form of AAU was significantly increased in HLA-B27-positive patients compared to HLA-B27-negative patients. However, we found in HLA-B27 carriers that those with the bilateral form of AAU had over three times smaller risk of recurrence and showed stronger immunization by endotoxins than patients with unilateral inflammation. Our results suggest a potential role of endotoxins in the aetiology of the nonrecurrent bilateral form of AAU. We suggest that not only HLA-B27 status but also determination of number of involved eyes may be useful to assess the risk of recurrence of the idiopathic AAU.

  This prospective, multicenter, single-masked study evaluated the additive effect of dorzolamide hydrochloride 2% on the diurnal intraocular pressure (IOP) curve and retinal and retrobulbarThis prospective, multicenter, single-masked study evaluated the additive effect of dorzolamide hydrochloride 2% on the diurnal intraocular pressure (IOP) curve and retinal and retrobulbar hemodynamics in patients with primary open-angle glaucoma (POAG) treated with morning-dosed bimatoprost 0.03%. Eighty-nine patients (aged, 60.7 ± 11.8 years, range 33-80; 68 women) with POAG received bimatoprost dosed once in the morning for 1 month, after which dorzolamide was added twice daily for 2 months. IOP (Goldmann) and arterial blood pressure (BP) and diurnal ocular perfusion pressures (OPP) were measured every 2 hr for 24 hr. Heidelberg retina flowmetry of the retinal microcirculation was recorded four times daily in 64 patients and colour Doppler imaging of the ophthalmic and central retinal arteries was recorded five times daily in 25 patients. All measurements were taken after the two phases of treatment and compared using anova analysis with Bonferroni adjustment. Mean baseline IOP was 16.5 ± 3.4 mmHg. Mean diurnal IOP with dorzolamide adjunctive therapy (12.9 ± 2.1 mmHg) was significantly lower compared to mean IOP with bimatoprost monotherapy (13.6 ± 2.2 mmHg) (p = 0.03). Adjunctive dorzolamide therapy significantly decreased vascular resistance in the ophthalmic artery (p = 0.02). Mean diastolic BP and OPP were significantly lower after adjunctive therapy. There were no changes in retinal microcirculation between the two phases of treatment. Adjunctive dorzolamide therapy to morning-dosed bimatoprost 0.03% reduced diurnal IOP and vascular resistance in the ophthalmic artery but did not alter retinal circulation in this group of patients with POAG.

The development of PVR is a complex process involving humoral and cellular factors. Surgical treatment of PVR, which consists of removal of the fibrous membranes and restoration of physiological anatomic ocular conditions is often unsuccessful. Therefore the surgery should by backed up by local medication to inhibit new formation of proliferative lesions. Unfortunately, there is no satisfactory antiproliferative treatment available so far. Proliferative vitreoretinopathy remains a therapeutic challenge.

  Age-related macular degeneration (AMD) is a leading cause of blindness over the age of 50 in western countries. People with AMD are suffering from serious vision-related disability and their socialAge-related macular degeneration (AMD) is a leading cause of blindness over the age of 50 in western countries. People with AMD are suffering from serious vision-related disability and their social life is compromised. The aim of our study was to assess quality of life (QoL) in patients with exudative AMD. The study group was 100 patients treated for AMD, the control group were 30 age and sex matched subjects without ophthalmic disorders. Patients were treated with anti-VEGF therapy, by means of National Eye Institute Visual Function Questionnaire (NEI VFQ-25). As well as visual function, the NEI-VFQ investigates social functioning, mental health and dependency. There was statistically significant difference in QoL overall score between study group and control group. Patients with AMD obtained 51.1 (+/- 20.5 ) overall score, control group reached 83.7 (+/- 11.7) overall score, p = 0.001. Detailed analysis of study group revealed low acceptance of the disease and strong dependency. QoL in patients with AMD assessed with NEI VFQ-25, is significantly impaired. Low quality of life and difficulties in performing daily activities point at the need of formal psychological and social care.

• Antiproliferative properties of mycophenolic acid on human retinal pigment epithelial cells in vitro.

  Mycophenolate mofetil (MMF) is an immunosuppressive agent used in the prophylaxis of graft rejection in transplantology. Its antyproliferative effects on lymphocyte, monocytes, vascular smooth muscleMycophenolate mofetil (MMF) is an immunosuppressive agent used in the prophylaxis of graft rejection in transplantology. Its antyproliferative effects on lymphocyte, monocytes, vascular smooth muscle cells and fibroblasts are well known, but to our knowledge there are no reports on its action on the retinal epithelial (RPE) cells in vitro. In all experiments we used mycophenolic acid (MPC), which is the biologically active form of MME Its activity was assessed on the cultures of immortalized non-transformed cells line from a human donor (ARPE19). Cells were seeded and incubated in vitro with deferent concentrations of MPC: 0.0025 pg/ml, 0.025 pg/ml, 0.25 pg/ml, 2.5 pg/ml, 25 pg/ml and 250 microg/ml. After 24 and 72 hours of incubation, proliferative activity was assessed by 5'-bromo-2'-deoxyuridine (BrdU) incorporation into cellular DNA and the amount of cell proliferation was determined using the 3-(4.5-dimethylthiazol-2yl)-2.5-diphenyltetrazolium bromide (MTT) assay. Additionally, to determine cytotoxicity of MPC, ARPE19 cells were grown to confluence and subsequently cultured in a serum-deficient medium and then, after 24 hours of incubation with different concentrations of MPC, the MTT test was performed. The BrdU assay showed the decrease of DNA synthesis activity for increasing concentrations of MPC starting with 0.025 microg/ml to 250 microg/ml. The number of RPE cells assessed with MTF test decreased after the exposition to the drug concentrations of 25 microg/ml and 250 microg/ml after 24 and 72 hours of incubation, and additionally for the concncentrations of 0.25 microg/ml and 2.5 microg/ml after 72 hours of incubation. MMF influences the proliferation of immortalized ARPE19 cells without evident cytotoxic effect.

Factors inhibiting PVR without toxic potential are of special interest in ophthalmology. The aim of the study was to investigate the antiproliferative and cytotoxic effects of a preservative-free triamcinolone acetonide (PFTA) suspension on a human retinal pigment epithelial (ARPE19) cell line in vitro. MATERIAL/METHODS: ARPE19 cells (immortal non-transformed cells from a human donor) were seeded and incubated in vitro with increasing concentrations of PFTA (0.01-1 mg/ml). After 1, 3, and 7 days, cellular proliferative activity was assessed by 5'-bromo-2'-deoxyuridine (BrdU) incorporation into cellular DNA and cell proliferation was determined using the 3-(4,5-dimethylthiazol-2yl)-2,5-diphenyltetrazolium bromide (MTT) assay. To determine cytotoxicity, ARPE19 cells were grown to confluence and subsequently cultured in serum-deficient medium to ensure a static milieu and the MTT test was performed after 24 hours of incubation with PFTA. Cell recovery after transient PFTA exposure was also compared with continuous exposure after 7 days. RESULTS: PFTA inhibits ARPE19 cell proliferation in a dose-dependent manner. Significant inhibition of cell proliferation was observed on the first day of the study at 1, 0.1, and 0.01 mg/ml PFTA and significant reductions in ARPE19 cells were noted for 1 and 0.1 mg/ml PFTA. Proliferation was resumed in all ARPE19 cultures and was dependent on the initial PFTA concentration. PFTA did not cause a cytotoxic effect. CONCLUSIONS: PFTA influences the proliferation of vital ARPE19 cells in a reversible manner without cytotoxic effect.

• Helicobacter pylori--a risk factor for the developement of the central serous chorioretinopathy.

MATERIAL AND METHODS: We examined 55 patients with central serous chorioretinopathy confirmed by fluorescein angiogram and 55 controls. Each patient provided venous blood sample for IgG antibodies to Helicobacter pylori by enzyme-linked immunosorbent assay technique (ELISA) and a stool specimen for Helicobacter pylori antigens. RESULTS: 44% in CSC patients were positive results of stool examine and only 29% in group comtrol. In 67% of the patients we proved the presence of the antibodies IgG--anty Helicobacter pylori and in 47% controls. The difference was statistically significant. CONCLUSIONS: Helicobacter pylori infection is statistically more frequently among the patients with CSC diagnosis than in healthy population.

• Proliferative diabetic retinopathy-The influence of diabetes control on the activation of the intraocular molecule system.

  AIMS: Demonstrate the influence of type 2 diabetes control on the degree of retinal endothelial damage (vWF, E-selectin) and local increases in the concentrations of selected adhesion moleculesAIMS: Demonstrate the influence of type 2 diabetes control on the degree of retinal endothelial damage (vWF, E-selectin) and local increases in the concentrations of selected adhesion molecules (ICAM-1, VCAM-1, E-selectin) in proliferative diabetic retinopathy (PDR). METHODS: Vitreous and serum samples were collected during vitrectomy from 19 patients with PDR and 15 patients who underwent vitrectomy for other reasons. Tests were performed using the ELISA method. RESULTS: Serum and intraocular concentrations of ICAM-1, VCAM-1, E-selectin, vWF were considerably higher in the subjects with PDR than in the controls. In the vitreous, the increase in vWF depended on the elevated levels of vWF in the serum (r=0.905, p<0.001). E-selectin correlated with diastolic blood pressure (r=0.506, p=0.045). The concentrations of vWF and E-selectin in both samples were related to the significant increases in intraocular ICAM-1, ICAM-1, VCAM-1 in the serum of PDR patients. Increased VCAM-1 level in the vitreous correlated with the concentration of HbA(1)c (r=0.59, p=0.007). CONCLUSIONS: Upon local and systemic damage to the endothelium there were significant increases in ICAM-1, VCAM-1, E-selectin. A significant positive correlation of VCAM-1 increase in the vitreous with HbA(1)c is an important argument for the influence of diabetes on immuno-inflammatory activation in the retinal microcirculation.

  Cancer-associated retinopathy (CAR) is a rare paraneoplastic syndrome usually associated with small-cell lung carcinoma and serum autoantibodies against recoverin. We report the breast cancer woman with visual impairments and electrophysiological abnormalities characteristic of CAR. Her serum contained high-titer antibodies against alpha-enolase but not against other retinal proteins. This suggests that anti-enolase antibodies could be responsible for the development of CAR symptoms.

• [The cyanoacrylate tissue adhesives in treatment diseases of the anterior eye segment]

  The cyanoacrylates were first synthesized in 1949 by H. Coover. They are used in medicine for over 50 years. The main advantage of synthetic tissue adhesives is cheap and simple production process,The cyanoacrylates were first synthesized in 1949 by H. Coover. They are used in medicine for over 50 years. The main advantage of synthetic tissue adhesives is cheap and simple production process, which gives possibility of using the cyanoacrylates in many surgical disciplines of medicine. That makes also synthetic tissue adhesives better than biological tissue glue (for example fibrin- based adhesives) which preparation is much more complicated and expensive. Nowadays the cyanoacrylates are often used in ophthalmology, mostly in treatment diseases of the anterior segment--for example corneal thinning, ulceration and perforation of cornea. In this paper possible uses of cyanoacrylate tissue adhesives in ophthalmology are presented. The method of application of the cyanoacrylates on the surface of damaged cornea is presented. On base of analysis provided in many clinical centres, the advantages, disadvantages and clinical effects of using tissue adhesives are discussed in this paper. The latest results of using cyanoacrylate tissue adhesives in anterior segment surgery, especially in closing corneal incisions made during cataract operation are presented too. The article contains also information about experimental researches relating to application tissue glue in vitreoretinal surgery and operative glaucoma treatment.

• [Influence of chronic exposure to heavy metals on eyesight].

  The aim of the study was to investigate relationship between chronic occupational exposure to heavy metals and eyesight, with particular lead influence. The experimental group was composed of 120The aim of the study was to investigate relationship between chronic occupational exposure to heavy metals and eyesight, with particular lead influence. The experimental group was composed of 120 workers of Copper Steelwork Plant "Legnica", divided into 2 groups, based on the degree and period of exposure to heavy metals. 75 men hired in direct production with work residency of 7 to 34 years (mean 21.2) made the experimental Group A--the occupationally exposed group. Group B with 45 white-collar men with work residency of 4 to 39 years (mean 19.8) made the environmentally exposed group. All workers underwent complex ophthalmic examination with particular lens investigation following pupil dilation, lead and cadmium level in full blood determination and free erythrocyte protoporphyrin (FEP) test, as indicator of human body lead overload. Mean lead levels in both groups were considerably higher than permissible concentration in biological material (PCB) for occupationally exposed workers, and was higher for those working directly in production. Free erythrocyte protoporphyrin (FEP) level was correct in both groups. Lenticular changes were noticed in 26 (21.7%) patients, of whom 18 (69.2%) were hired in direct production. Similarly, fundus changes were shown in 17 patients, 16 (22%) from Group A; changes in field of vision in 3 patients. Furthermore, 15 men working in direct production complained of burning sensation, tearing and periodic redness in the eyes. Mean blood cadmium and lead levels, higher in occupationally exposed workers was statistically important only in case of lead. However, the correct FEP level indicates that an organism was not overload with lead. In these workers (group A) lenticular changes were observed more often and were more intense (cortex opacitates, posterior subcapsular cataract). Fundus changes noticed in both groups were of low degree and coexisting diseases may have promoted their progression. Field of vision changes noticed in 3 workers may have been caused by lead toxicity to optic nerve. Periodical conjuctival inflammatory states are caused by irritative dust and smoke therefore protective eye wear is indicated.

The problem of aphakia exists as long as cataract is treated, that is at least 45 hundreds of years. Correction of aphakia using glasses is uncomfortable for patients and even impossible in some cases. The invention of an artificial intraocular lens was a crucial event in the history of ophthalmology. Nowadays there are more indications for cataract surgery and implantation of intraocular lens, including correction of refractive errors. The first intraocular lenses were made of rigid plastic: polymethylmethacrylate (PMMA). In present times foldable intraocular lenses made of soft material: silicone, hydrogel and acrylic compounds are also produced. In this paper 4 groups of intraocular lenses: PMMA, silicone, hydrogel, acrylic and their influence on biocompatibility are discussed. This article contains also description of two types of IOL biocompatibility: uveal and capsular. Their clinical aspects, especially posterior capsule opacification are discussed. Factors affecting biocompatibility are shortly described. Some historical applications of IOL are presented. Regarding the latest experimental studies on improving IOL biocompatibility, results of IOL surface modification tests are presented. New perspective technologies of intraocular artificial lens production such as single crystal materials are also discussed.

Contact lens must be oxygen-permeable to provide nutrition for cornea which has no own vascular net. In the past decade hydrogel lenses are being replaced with silicone-hydrogels. A new generation of contact lenses does not disturb the physiology of a cornea as much as the old one. Since the introduction of silicone-hydrogel lenses much less side effects have been reported. They can also be used continuously with less risk, over longer period of time. Traditional measures such as oxygen permeability and transmissibility seem inadequate with silicone-hydrogel lenses. Therefore a new measure, an oxygen flux, has been introduced. It is more physiological and more suitable in higher oxygen transfer range. It refers to quantity of oxygen reaching the surface of a cornea.

Consequently to reduce its farther transmission large knowledge of HCV should be spread as there is still no effective means of prophylaxis and its therapy is much costly. This article describes ophthalmic symptoms of hepatitis C virus infection such a mere keratoconiunctivitis sicca to ischemic retinopathy, macular edema and ischemic neuropathy.

  BACKGROUND: Injection of lipopolysaccharides/LPS/, the major component of the outer membrane of gram-negative bacteria, can induce inflammation in the eyes of susceptible animals.The LPS-induceBACKGROUND: Injection of lipopolysaccharides/LPS/, the major component of the outer membrane of gram-negative bacteria, can induce inflammation in the eyes of susceptible animals.The LPS-induce ocular inflammation is termed endotoxin-induced uveitis/EIU/and is characterized by iris hyperemia, miosis, a rise in aqueous humor protein, and inflammatory cell infiltration into the anterior uvea and aqueous humor. Biological activities of endotoxin depend also on its molecular weight. Objectives: The aim of our study was to find the effect of different aggregation forms on the clinical and histopathological characteristics of the EIU. MATERIALS/METHODS: Lipopolysaccharides were electrodialyzed and neutralized by adding: triethylomine, sodium hydroxide, or calcium hydroxide. EIU was produced in Lewis rats by footpad injection of different enterobacterial LPS. Their eyes were examined for clinical signs of inflammation in slit lamp, protein and cells were measured in the aqueous hum RESULTS/CONCLUSIONS:The correlation between the physical parameters and biological activity is discussed. Our results have shown recently that monomeric form of endotoxin is more active than an aggregated form in induction of experimental uveitis.

• [Risk factors evaluation in age- related macular degeneration]

  Age-related macular degeneration (AMD) is a leading cause of irreversible loss of central vision in developing countries. Study of risk factors seems to be more significant because of the lack ofAge-related macular degeneration (AMD) is a leading cause of irreversible loss of central vision in developing countries. Study of risk factors seems to be more significant because of the lack of effective treatment. Exact recognition especially of modifiable risk factors of AMD development could increase prevention and decrease development and progression. The aim of our study was to evaluate selected modifiable and non-modifiable risk factors of AMD. The examination included 44 AMD patients and 30 healthy subjects in control group. In both groups age, sex, eye color, smoking, UV exposition, blood pressure, cholesterol level, type of diet, family history of AMD and cataract surgeries in anamnesis, were defined. The most significant risk factors of AMD development, in presented study were female gender and UV exposition. They increased a risk accordingly 1.55 and 3 times. Iris color and low vitamin and high fat intake in diet were also essential. There was no dependence on approved risk factor such smoking and AMD appearance.
