Article: Percutaneous transluminal septal myocardial ablation in a child with hypertrophic obstructive cardiomyopathy and Noonan syndrome.

ABSTRACT: We report an 8-year-old child presenting with classical features of hypertrophic obstructive cardiomyopathy and Noonan syndrome with New York Heart Association (NYHA) class III symptoms. Due to progressively worsening symptoms, the child was taken up for percutaneous transluminal septal myocardial ablation, which was successfully performed. Postextrasystolic gradient fell down from 125 to 35 mm Hg. Article: Anesthesia for percutaneous transcatheter closure of perimembranous ventricular septal defect.

ABSTRACT: To review the anesthetic management for percutaneous transcatheter closure of perimembranous ventricular septal defect (VSD) with an Amplatzer asymmetric occluder device and to highlight the hemodynamic effects and potential complications associated with its delivery. Retrospective review of prospectively collected data. University-affiliated teaching hospital. Nine consecutive children undergoing elective percutaneous transcatheter closure of perimembranous VSD. General anesthesia with sevoflurane for cardiac catheterization and percutaneous transcatheter device placement. Ten anesthetics were delivered in 9 children ages 23 to 65 months with perimembranous VSD for attempted placement of an Amplatzer asymmetric device. The device was successfully placed in 7 patients. In 1 patient the device embolized to the right femoral artery, and was retrieved with a bioptome. Fluoroscopy time (59.8 +/- 17.24 min) was prolonged compared to that in other studies of placement of this device. All patients had episodes of arrhythmia and hemodynamic disturbance. Arrhythmias ranged from atrial or ventricular ectopic events to various degrees of atrioventricular block. Complete heart block occurred during the procedure in 1 patient and after the procedure in another patient. Hypotensive episodes occurred in 7 patients, and were attributed to arrhythmias in 5 patients and hypovolemia in 2 patients. Two patients were given blood transfusions after the procedure because they had signs of hypovolemia and a greater than 10% decrease in hemoglobin levels. ABSTRACT: We report a case of mycotic aneurysms due to Staphylococcus aureus infection in the left anterior descending coronary artery in a 56-year-old male after implantation of a sirolimus-eluting stent. Article: Life-saving PTMC for critical calcific mitral stenosis in cardiogenic shock with balloon impasse.

ABSTRACT: A 32-year-old male patient, a case of critical calcific mitral stenosis (following closed mitral valvotomy in 1989) was admitted for mitral valve replacement in September 2001. In hospital, he developed cardiogenic shock, pulmonary oedema and oliguria precluding surgery. Article: Percutaneous transluminal angioplasty in an embolic middle cerebral artery occlusion.

ABSTRACT: A young lady developed left middle cerebral artery embolism after percutaneous transseptal mitral commissurotomy. She was successfully treated with immediate percutaneous transluminal angioplasty. There was complete recovery of neurological deficit.Indian heart journal 56(6):658-60. -
Article: Congenital atresia of left main coronary artery.

ABSTRACT: We report a rare case of congenital atresia of left main coronary artery in an elderly male presenting with angina and positive stress thallium test. Coronary angiogram showed absence of left main coronary artery in the left aortic sinus. Collaterals from right coronary artery supplied left anterior descending artery and left circumflex artery territories. Despite collaterals, the left anterior descending and left circumflex arteries were rudimentary. Poor size of left-sided arteries precluded any surgical revascularization which is the treatment of choice in this rare entity.Indian heart journal 57(3):255-7. -
Article: Acute myocardial infarction secondary to myocardial bridge treated with drug-eluting stent.

ABSTRACT: We report a case of myocardial bridge in left anterior descending coronary artery associated with acute anterior wall myocardial infarction. The patient had many unusual features like coronary ectasia and atherosclerosis within the myocardial bridge segment. The patient was treated with drug-eluting stent under intravascular ultrasound guidance with good result over 6 months follow-up.Indian heart journal 57(6):734-7. -
Article: Angioplasty of congenital pulmonary vein stenosis.

ABSTRACT: We present a unique case of idiopathic pulmonary vein stenosis in an adult who presented with progressive dyspnea and severe pulmonary arterial hypertension. Magnetic resonance imaging confirmed the diagnosis. The patient was treated by balloon angioplasty and is well at 6 months post-treatment follow-up. The etiology of pulmonary vein stenosis in this case is difficult to ascertain, but is likely to be congenital.Indian heart journal 57(6):709-12. -
