ABSTRACT: To summarize the surgical technique and the effectiveness of CK Parastomal patch in laparoscopic repair of parastomal hernia.
The clinical data were retrospectively analysed from 24 patients who received laparoscopic repair of parastomal hernia using CK Parastomal patch between June 2006 and March 2010. There were 15 males and 9 females with a median age of 55 years (range, 47-80 years). Among them, 19 patients were with colon parastomal hernia in the left lower quadrant and 5 patients with ileum parastomal hernia in the right lower quadrant. The parastomal hernia duration was 1 to 4 years (mean, 2.4 years). The maximal diameter of the hernia ring was 3 to 7 cm (mean, 5.2 cm). All patients did not receive hernia repair.
Laparoscopic repair of parastomal hernia was successfully performed in 18 cases, and open repair was used in 6 cases because of extensive adhesion. The average operating time was 121 minutes (range, 78 to 178 minutes). All wounds healed by first intention. Wound seroma occurred in 8 cases at 3 to 7 days after operation and disappeared by aspiration. Eleven patients complained of pain in the operative area within 1 month after operation and it disappeared without intervention. All patients were followed up 6 to 39 months (mean, 27 months). One recurrence occurred at 3 months after operation, and no recurrence occurred in the other patients.
Laparoscopic repair of parastomal hernia using CK Parastomal patch is a safe and feasible procedure with a satisfactory short-term effectiveness, but the long-term effectiveness should be further observed.

ABSTRACT: For the regenerative therapy of refractory diseases, stem cells have become an excellent source of seed cells due to their strong self-renewal and multi-differentiation abilities. Microcapsules can provide a three-dimensional growth environment with a good immunoisolation and biocompatibility for cells, and the microencapsulation of stem cells provides a new technical support for large-scale cell culture with high activities in vitro and long-term preservation, consequently opening up a new alternative for cell transplantation. In this review, we first outlined the development of microencapsulation, then introduced the present materials and methods for the microencapsulation of stem cells and its immunoisolation, and discussed the progress in microencapsulation technology, various types of stem cell used in recent years in details. Finally, we addressed perspectives of stem cell microencapsulation technology.

ABSTRACT: To summarize the clinical effect of allogenic acellular dermal matrix in repair of abdominal wall hernia and defect.
The clinical data were analyzed retrospectively from 31 patients with abdominal wall hernia and defect repaired by allogenic acellular dermal matrix between March 2007 and November 2009. There were 19 males and 12 females with an age range of 10-70 years (median, 42 years), including 6 abdominal wall defects caused by abdominal wall tumor resection, 4 patches infection after abdominal wall hernia repair using prosthetic mesh, 2 incisional hernia, 1 parastomal hernia, 1 recurrent parastomal hernia receiving mesh repair, 1 mesh infection caused by parastomal hernia repair using prosthetic patch, 3 mesh infection caused by tension free inguinal after hernia repair, and 13 inguinal hernia. There were 12 patients with contaminated or infectious wound. The disease duration was from 1 to 34 months (6 months on average). The defect size of abdominal wall ranged from 6 cm x 4 cm to 19 cm x 10 cm. Abdominal wall hernia or defect underwent repair using allogenic acellular dewall matrix.
Of the 31 patients, 29 patients recovered with primary wound healing. Chronic sinus tract occurred in 1 patient and the wound was cured by change dressing. Wound dehiscence and patch exposure occurred in 1 patient, and second healing was achieved after change dressing. All the 31 patients were followed up 6-36 months, no abdominal wall hernia or hernia recurrence occurred in other patients except 1 patient who had abdominal bulge. And no foreign body sensation or chronic pain in wound area occurred.
It is feasible and safe to use allergenic acellular dermal matrix patch for repair of abdominal wall hernia or soft tissue defect, especially in contaminated or infectious wound.

ABSTRACT: This study sought to evaluate the effect of steep pulsed electric fields (SPEFs) on the immune response of Wistar mice inoculated with Walker256 sarcoma. Thirty mice were randomly divided into three groups: control group (group A, inoculated with Walker256 sarcoma, not treated), treatment group (group B, inoculated with Walker256 sarcoma, treated by SPEFs), and normal control group (group C, inoculated with normal saline, not treated). Tumor size was measured before and every 3 days after treatment by vernier caliper. MTT methods were used to assess the lymphocytes proliferation and the natural killer (NK) cells activity. TNF-a activity was measured by ELISA. Statistical analysis was performed utilizing the SPSS10.0 software package. The experiment results revealed that tumor growth was significantly inhibited in group B as compared with group A (P < 0.01), and that lymphocytes proliferation, NK cells activity and TNF-a activity in group B were not significantly different from those in group C (P = 0.953, P = 0.130, P = 0.080, respectively) but markedly higher than those in group A (P < 0.05). The results also showed that SPEFs could not only kill tumor cells but also induce antitumor immune response and improve the immune function of the host efficiently.

ABSTRACT: It has been testified by running practice to that the internal and the external overvoltage of distribution networks are the main causes of electrical accidents. To capture and analyze the overvoltage in power system, authors have developed a set of overvoltage on-line monitoring system, and it has been running in a 10 kV distribution network for more than 36 months. Thousands of detailed data files have been collected and saved. Based on the brief introduction to the structure and the operational principle of the monitoring system, this paper presents an intelligently identification method of the internal overvoltage and the external overvoltage based on Fisher discriminate method. Firstly, the statistical parameters should be extracted from the original data recorded by the on-line monitoring system. According to the performance of many parameters during the period of the overvoltage occurrence, 8 of them such as time to crest, time to half value of voltage, total duration of overvoltage and etc. are selected as the characteristic parameters of identifying overvoltage types. Secondly, according to the operational data tracked by other monitoring instruments in the field and the daily report, the corresponding reference sample for either overvoltage are brought forward. Then, the identifying function can be constructed by the Fisher discrimination method. Any 8-dimensions data will be projected to 1-dimension. Lastly, the unknown overvoltage data samples are substituted to the identifying function. The distinguished results to 266 samples indicate that the outcomes are accordant, the identifying method along with the identifying function are correct and effective.

ABSTRACT: This paper discussed the lethal effects of steep pulsed electric field (SPEF) to target lymphatic capillaries in VX<sub>2</sub> implanted breast cancer of rabbits using methylene blue dye injection staining, 5'-AMP-ALPase enzyme histochemical double staining, electron microscopy and VEGF-C observations. The result of methylene blue dye perfusion showed that the edge of tumor treated with SPEF was indistinct, no circularity or liner structure. Enzyme histochemical staining examination displayed that no positive staining was found in SPEF-target region of breast tumor, and lymphatic capillaries presented piecemeal structure in the edge of target region. Transmission electron microscopic examination showed that the continuity and integrality of lymphatic capillaries in tumor were destroyed after the treatment of SPEF. The expression of VEGF-C decreased markedly. The experiment indicated that SPEF induced the severe destruction of tumor cell and lymphatic capillaries in target region, and inhibited proliferation of lymphatic. ABSTRACT: Pulsed electric fields (PEFs) with fixed frequency, width and gradually increased peak value of voltage was applied to 30 healthy rabbit liver tissues. The specific aims were to explore the feasibility of establishing a model of in vivo PEFs distribution in healthy rabbit liver tissues and to provide important references for clinical electrochemotherapy and for electrotransfer. Repeated experiment and self-comparison statistics design were implemented. The rabbit underwent the experiment under intravenous anesthesia and their liver tissues, after exposure to PEFs, were sent for HE staining. Necrotic borderline was visible 3 days after PEFs application, the necrotic shape of concentric circle was evident around the electrodes under optical microscope at lower voltage, as voltage increasing, two necroses in the shape of concentric circle gradually enlarged; nuclei with chromatin condensation, fragmentation and lysis alterations were seen in the middle region between the needles; concentric circles changed into ellipse fusiform and finally overlaped each other forming irregular necrosis contours. Cell cavitation and tissues ischemia were also observed within electric field. The shape of tissue necrosis from the experiment was noted to correlate with theoretic simulation of electric field distribution. Therefore, rabbit liver tissues can be a good carrier for in vivo modeling of electric field distribution when the lethal effects of PEFs in tissues are investigated. PEFs also show safety for the surrounding normal tissue while causing damage or injury to the target area therapeutically.

ABSTRACT: The design and realization of an on-line monitoring system for over-voltage of distribution grids was introduced. This device can monitor waveforms of varied over-voltage on 10 kV bus bar, monitor internal and atmospheric over-voltage, has high frequency response, short response time, and flexible adjustment of parameters. The system has become a long term and stable system in the Chongqing power grid. It can reflect the real over-voltages in power systems objectively and has provided strong scientific evidence for experts when analyzing insulation failures.

ABSTRACT: Electroporation (i.e., reversible electrical breakdown) has been used for biomedicine such as electroporation therapy, transdermal drug delivery, cellular drug carrier systems, and gene transfer therapy. However, little research on the biological effect of irreversible electrical breakdown has been published. In this paper, the killing and inhibiting effects of pulsed electric field, whose rising edge is 100 ns level (we call it steep pulsed electric field, SPEF), to target cancer cell and solid tumor were successfully observed. The SPEF consists of two parts: the rising process containing ample high-frequency components, and the falling process containing lots of low-frequency components. Cell exposed to the pulse presented multiple changes: destructed membrane, swollen nucleus, disintegrated karyotheca, and cavitated, swollen mitochondria in histological and ultrastructure observations. SPEF blocked cell transition from G0/G1 phase to S+G2/M phase in flow cytometry analysis, and tumor growth retardation was observed in tumor-bearing mice. These experimental results show SPEF have a certain function to generate irreversible electrical breakdown in tumor cells, and might provide a new promising therapeutic approach for tumor treatment.

ABSTRACT: The vascular effect and immunity effect of SPEF on Wistar mice were studied, and the possible treatment mechanisms and clinical trial methods of SPEF were discussed. We tested lymphocyte proliferation ratio and activity of NK cell, and the contents of IFN-alpha and IL-4 in the serum lymphocyte were inspected with ELISA method. Moreover, macrophage were collected, the content of TNF-alpha excreted by macrophage were inspected with ELISA method, too. Statistical analysis was performed using the SPSS10.0 software package and t-test. VEGF level in test group was significantly decreased, and VEGF expression was weakened evidently. SPEF strengthened activity of NK cell, enlarged quantities of TNF-alpha from test group, accelerated exudation of TH1 cell factor and slowed exudation of TH2 cell factor. SPEF showed effective anti-tumor effect through restraining the formation of fresh vessels and baffling the birth and transfer of tumor cell. It could strengthen organism immunity function, and had visible effect on Walker-256 tumor.
