• [Our experience in the treatment advanced neoplasms of floor of the mouth].

  The aim of this study was to evaluate results of our surgical treatment of advanced neoplasms of the floor of the mouth, treated from 2005-2011 in Department of Otolaryngology and Department ofThe aim of this study was to evaluate results of our surgical treatment of advanced neoplasms of the floor of the mouth, treated from 2005-2011 in Department of Otolaryngology and Department of Plastic Surgery Medical University of Gdańsk We analyzed group of 12 patients (aged 36 to 70 years, mean 55) treated by primary surgical excision of neoplasms of the floor of the mouth (with tracheotomy and mandibulotomy) and selective or radical neck dissection (uni- or bilateral) in combination with chemoradiotherapy. All patients referees with advancement of locoregional disease: primary tumors (T3 or T4) and neck metastases (N1 or N2b) Most of the patients required to use free microvascular reconstruction with free forearm skin flaps. Histological examinations revealed squamous cell carcinoma (11/12) and adenocarcinoma (1/12). Two patients die in 1 and 4 months after surgery. Three die from 2 to 3 years after surgery. Rest live from 6 month to 6 year after therapy. Free forearm skin microvascular flap is good (anatomical and functional) for reconstruction for large defects after resection neoplasms of the floor of the mouth.

  The aim of this study was a review of 67 children treated for salivary gland tumors in the ENT Department Medical University of Gdańsk since 1974 to 2008. In that group we observed 52 children withThe aim of this study was a review of 67 children treated for salivary gland tumors in the ENT Department Medical University of Gdańsk since 1974 to 2008. In that group we observed 52 children with tumor of the parotid gland, 13 with tumor of submandibular gland and 2 with subligual gland tumors. Tumor-like lesions were stated the largest group of tumors and were observed in 33 (49%) of children, mainly of chronic sialadenitis (17 cases). Predominant number of tumors was observed in scholarship children. The most common symptoms were slow enlargement of tumor and swelling of the salivary gland. In the group of benign tumors most common was pleomorphic adenoma (19 cases) and in the group of malignant tumors most common was clear cell carcinoma (2 cases) and rhabdomyosarcoma (2 cases). In this report the authors describe basic epidemiological and clinical data (age, sex, duration of symptoms, histological structure of tumor, diagnostic methods, methods and results of treatment) and observations connected with differences of salivary gland tumors in that group of patients.

  The aim of his study was to assess the treatment results and prognostic factors in patients with parotid gland carcinoma. The material consisted of 109 patients treated surgically, with or withoutThe aim of his study was to assess the treatment results and prognostic factors in patients with parotid gland carcinoma. The material consisted of 109 patients treated surgically, with or without complementary radiotherapy, between 1978 and 2008 (follow-up at least 5-years). 5-year overall and disease-specific survival were observed in 57.0% of the patients and 5-year disease-free survival was achieved in 50.0%. Univariate analysis including ten clinical and pathological features to assess their prognostic value was done. Parapharyngeal space invasion, facial nerve palsy, and high grade of tumor malignancy were the factors with the highest influence on the treatment results, because their presence decreased the chance for recovery 9.8, 9.7, and 8.2 times, respectively. Histologically positive cervical lymph nodes and extraparenchymal extension were the other factors connected with poor prognosis (prognosis worse 6.7 and 5.4 times, respectively). Clinically positive cervical lymph nodes, positive/uncertain microscopic margin, involvement of the deep lobe, or the whole gland increased the risk of treatment failure 3.4, 3.1, and 2.8, respectively. The age ≥60 years and male gender were statistically significant factors, correlated with poor prognosis and decreased chance for recovery 2.4 and 2.6 times. T-status and clinical stage had important influence on 5-year disease-free survival rate because there were significant differences in the treatment results between individual stages. Multivariate analysis proved that the independent prognostic value, among anatomic structures involved by the neoplasm, had mandible, facial nerve, and skin infiltration. Among tumor-related factors, T-stage and grade had the statistically significant influence on treatment results, and stage and lymph nodes metastases among clinical and pathological features. These results confirm the value of actually used TNM classification (2002). Although the parapharyngeal space invasion is a factor, which seems to have a significant, poor prognostic value, it was not included in this classification.

Histopathologic examination of the tumor revealed numerous smooth muscle cells, mature adipose tissue and numerous thin and large irregular vessels. Both histological and immunohistochemical findings were typical for tumors of the mucocutaneous angiomyolipoma group.

  BACKGROUND: Pleomorphic adenoma (PA) is the most common neoplasm of the major salivary glands. There are three subtypes of malignant PA: carcinoma ex pleomorphic adenoma (CXPA); carcinosarcoma (trueBACKGROUND: Pleomorphic adenoma (PA) is the most common neoplasm of the major salivary glands. There are three subtypes of malignant PA: carcinoma ex pleomorphic adenoma (CXPA); carcinosarcoma (true malignant mixed tumor) and metastasizing pleomorphic adenoma. The most common subtype of malignant PA is CXPA which develops in primary or recurrent PA. For proper diagnosis of CXPA, a statement of coexistence of pleomorphic adenoma and carcinoma (or carcinoma after prior PA surgery) is needed. Own material is presented because of rarity and clinicopathologic specificity of this neoplasm. METHODS: Retrospective analysis of the medical data of 19 patients who were treated at our department because of CXPA from 1990 to 2002 was done. The following clinical factors were evaluated: age, sex, symptoms (time of lasting, evolution), tumor size, invasion of the adjacent structures and facial nerve, neck nodes, clinical stage, treatment outcome. One pathologist reviewed histological material of 18 out of 19 patients who had been operated and pathological factors such as sensitivity and accuracy of fine needle aspiration biopsy, grade, histology and proportion of malignant component in tumor, lymph nodes metastases were analyzed. RESULTS: There were 11 men and 8 women, the mean age was 57 years. In 15 cases, tumor was localized in the parotid and in 4 in the submandibular gland. Two patients had history of prior surgery of PA. Duration of symptoms of benign PA was from 2 to 40 years (mean 17.8 years). Symptoms of malignant transformation occurred in 15 patients, the most common were rapid enlargement of tumor, pain and facial nerve palsy. Tumor size varied from 2 to 20 cm with a mean of 6 cm. In 14 patients, the neck was evaluated clinically as No, in 5, examination identified enlarged lymph nodes. Adjacent structures were invaded by neoplasm in 5 cases. Sensitivity and accuracy of fine needle aspiration biopsy in detection of malignant character of PA were 60% and 46% respectively. 17 out of 18 tumors reviewed by the pathologist were high grade. Only in 6 patients, proportion of carcinoma in the mass was less than 50%. The most common malignant component in CXPA was adenocarcinoma (9 cases) and undifferentiated carcinoma (6 cases). Pathologic examination showed metastases to the lymph nodes in 7 out of 10 patients with prior neck dissection. 16 patients were treated surgically (12 of them had also radiation therapy) and 3 patients underwent only (chemo)radiotherapy. Determinate survival at 5 years was 73.6% and in the group treated surgically (alone or with postoperative irradiation) 87.5%. CONCLUSIONS: Malignant transformation of PA occurs in 5 to 25% untreated patients, usually after 15-20 years and warning symptoms are present in the most cases. Fine needle aspiration biopsy has insufficient sensibility and accuracy in detection of malignant character of PA. Clinical picture and histopathological examination determinate the diagnosis of CXPA. Proper histological classification of malignant component can be found difficult. Surgical treatment (alone or with postoperative irradiation), if possible, allows to achieve good locoregional control of CXPA. 5-years survival varied between 30 to 76%. The best method of prevention and treatment of CXPA is early and radical removal of all major salivary glands tumors.

  Otogenic skull base osteomyelitis (SBO) of fungal etiology is a very rare but life-threatening complication of inflammatory processes of the ear. The authors present a case of otogenic SBO caused by Aspergillus flavus in a 65-year-old man with a fatal course. Because of the encountered difficulties with the proper diagnosis and treatment, the authors reviewed the literature on the subject.

• [Renal cell carcinoma metastases to the head and neck]

The objective of this paper is to analyze the clinical presentation, diagnostic difficulties, management and outcome of metastatic renal cell carcinoma to the head and neck. MATERIAL AND METHODS: Retrospective review of four patients with renal cell carcinoma who have developed head and neck metastases and have been hospitalized in the ENT Department of the Medical University of Gdansk over the past 35 years. RESULTS: There were three women and one man in the examined material. The localizations of metastasizing renal cell carcinoma were: the larynx, the parotid gland, the cervical lymphnodes and the nasopharynx. In two patients metastatic tumor was the first clinical presentation of the disease and in the next two, time between treatment of the primary localization and occurring head and neck metastases was one and ten years. All patients, except one, were treated surgically. Two patients died, two are alive and the survival periods are one and three years. CONCLUSIONS: Renal cell carcinoma metastases to the head and neck region are infrequent but can cause difficulties in diagnosis and proper management. Despite of possibility of local excision, prognosis in metastasizing renal cell carcinoma is still unfavourable.

MATERIAL AND METHODS: Bone scanning was performed in three male patients with otogenic skull base osteomyelitis, aged 65-84 years utilizing Tc99m-MDP and dual-head gamma camera. RESULTS: In one case, bone scanning played a crucial role in establishing the diagnosis. In two cases, it provided confirmed MRI results. CT scanning was negative in two cases. CONCLUSIONS: Bone scans may give valuable information for establishing the diagnosis and assessing the severity of this disease, and add complementary physiological information to radiological imaging.

  Venous thromboembolic disease (deep vein thrombosis and pulmonary embolism) is a frequent and dangerous complication of surgical procedures. Guidelines considering prophylaxis in patients who have aVenous thromboembolic disease (deep vein thrombosis and pulmonary embolism) is a frequent and dangerous complication of surgical procedures. Guidelines considering prophylaxis in patients who have a risk of this complication exist in most surgical specialties. In otolaryngology it is a rare illness but a lack of Polish literature and quite few world publications on this subject made the authors check through the literature referring to venous thromboembolic disease in otolaryngology and maxillofacial surgery and indexed in the EMBASE and MEDLINE data bases up till 2003. Incidence of the disease, risk factors with regard to the kind of surgery and prophylaxis methods were presented, all with the aim of creating a kind of a guide useful in clinical practice in ENT.

The role of imaging procedures and bacteriological examination was found especially important in establishing the diagnosis and introducing the right treatment. Efficacy of external (transcutaneus) approach to the para- and retropharyngeal space and phlegmon drainage of these spaces preceded with tracheotomy and tonsillectomy was underlined. Surgical treatment was supplemented with multi-component antibiotic therapy and gammaglobulin administration.

• [Congenital and acquired cholesteatoma middle ear in children]

  Cholesteatoma of the middle ear in children may cause hypoacusis. Early diagnosis and optimal treatment is neccessary for good functional effect. We present 57 children (58 ears) with cholesteatomaCholesteatoma of the middle ear in children may cause hypoacusis. Early diagnosis and optimal treatment is neccessary for good functional effect. We present 57 children (58 ears) with cholesteatoma treated in ENT Department of Medical University in Gdańsk in 1991-2002. The age of patients ranged between 3 and 16 years, the most common 11-15 years. In 52 (89.6%) cases acquired cholesteatoma and in 6 (10.4%) congenital cholesteatoma was diagnosed. Epitympanal cholesteatoma was found in 32 children (55.1%) whereas in posterior part of tympanic cavity--in 20 children (34.5%). In 6 cases (10.4%) intact tympanic membrane was found. Mean air-bone gap in acquired cholesteatoma before treatment was 18.7 dB, after treatment 15.7 dB. In congenital cholesteatoma mean air-bone gap before treatment was 13.6 dB, after treatment 14 dB. The most frequent symptom was hearing loss (98.3%) and purulent otorrhea (85.4%). Positive bacteriological culture was obtained in 43.1% of the cases. X-ray revealed sclero-pneumatic mastoid in 26 (34.5%) cases, sclerotic in 25 (43.1%) and pneumatic mastoid in 7 (12.1%) cases. Intracranial complications were found in 2 cases, intratemporal in 2 cases and extracranial complication in 1 case. Radical surgery was performed in 23 cases (39.7%) and in each case, which required reoperation, modified radical mastoidectomy in 20 cases (34.4%), in another 15 children (25.9%) tympanoplasty was done. The most frequent failure was purulent otorrhea in 21 (36.2%) cases. Reoperation in cholesteatoma recurrence was performed in 17 children (29.3%). Improvement or the same as preoperatively hearing level was obtained in 35 (60.3%) ears, hearing loss was revealed in 23 (39.7%) ears. Treatment of temporal bone cholesteatoma in children is difficult due to silent beginning, aggressive growth and frequent recurrence. The best treatment results in children cholesteatoma are obtained in early clinical stage and with open tympanoplasty procedure.
