  • Article: [Two faces of an illness: rheumatoid arthritis followed by ankylosing spondylitis].

    ABSTRACT: Author presents the clinical history of a 35 years old female patient, who suffered from seronegative rheumatoid arthritis at the age of 29. Remission of the illness was followed by successful gravidity. Almost two years later spondylarthritis was diagnosed, showing only axial symptoms. The major histocompatibility complex analyzed by normal serological methods revealed the HLA B27 haplotype, typical for ankylosing spondylitis, and the HLA DR1 haplotype, typical for rheumatoid arthritis. During the examination of HLA DRB polymorphism, presence of DR B1 0101 allele was found, that plays role in the pathophysiology of rheumatoid arthritis. This case proves that a combination of HLA alleles typical for ankylosing spondylitis and rheumatoid arthritis can be found in the same patient.

  • Article: [Our experience with nephron-sparing surgery in the past nine years].

    ABSTRACT: Surgical therapy is the only curative therapeutic approach for the treatment of renal cell carcinoma. Nephron-sparing surgery for renal cell carcinoma, when performed by proper indication, provides recurrence-free and long-term survival rates similar to those observed after a radical surgical procedure. In this retrospective study we present our experiences on open nephron-sparing surgery. We describe the rates of different indications of nephron-sparing surgery, the rates of different hystopathological findings, the complications and the long-term cancer specific survival rates. We reviewed retrospectively the data of 33 patients who had nephron-sparing surgery with T1 tumors between 1999 and 2008 in our department. The last patients' status evaluation was carried out in March 2009. The mean follow up was 4.64 years. 82% of the patients had an elective indication, 3% had relative and 15% had absolute indication for nephron-sparing surgery procedure. The mean tumor greatest dimension was 2.86cm. The hystopathologic review revealed 94% clear cell, 6% papillary renal cell carcinomas in the class of malignant renal tumors. The 1- and 5-year cancer-specific survival chance were 93.8%. We had to do nephrectomy because of postoperative bleeding and urinoma in 3 cases. Nephron-sparing surgery is an acceptable and safe treatment that provides excellent long-term cancer specific survival rates for T1a renal cell carcinoma patients.

  • Article: [Genetic investigation of human leukocyte antigens in rheumatoid arthritis patients with antiphospholipid syndrome].

    ABSTRACT: Two rheumatoid arthritis patients with antiphospholipid antibody syndrome were observed. The diagnosis was established by the clinical signs and the measurement of anticardiolipin antibodies. As a result of the genetic investigations, typical HLA markers of the rheumatoid arthritis and those of the antiphospholipid antibody syndrome were found in both patients. In connection with rheumatoid arthritis they were the HLA-DRB1*0101 allele in the female and the HLA-DRB1*0401 in the male patient. With regards to antiphospholipid antibody syndrome, the HLA alleles were the HLA DQB1*0602 in both patients and the HLA DQB1*0302 in the male patient.
