    ABSTRACT: Despite the many benefits of percutaneous thoracic sympathectomy, it also has serious complications such as pneumothorax. This study was conducted in order to determine the safe percutaneous entering window and angles for the needle during T2 and T3 thoracic sympathectomy avoiding pneumothorax.

    ABSTRACT: Background Crohn's Disease Activity Index (CDAI) is complex, time-consuming, and impractical. The aim of this study was to investigate whether a newly developed, simple, web-based self-reporting Crohn's Disease symptom diary (CDSD) was as effective as CDAI in assessing disease severity. Methods CDSD consisted of 5 clinical parameters based on the Harvey–Bradshaw Index (HBI), which could easily be recorded online, by using CDSD website (www.cdsd.or.kr). Images were added to help patients better understand complications. All patients were asked to visit the website and record their symptoms 7 days before their next hospital appointment. CDAI scores were calculated at the subsequent hospital visit. The collected data were analyzed to determine if the CDAI scores correlated with those obtained from CDSD, and to define a cut-off value of CDSD that would be representative of disease remission. Results Analysis of 171 visits showed a positive correlation between scores from CDSD and CDAI (Spearman correlation coefficient r = 0.720, p < 0.001). Receiver Operating Characteristic curves showed CDSD score ≤ 5 points as corresponding with CDAI score ≤ 150 points (clinical remission). Using a cut-off value of 5 points by CDSD, the positive and negative predictive values for clinical remission were 91.7% and 88.5%, respectively. Conclusion This study demonstrates that CDSD correlated well with CDAI. CDSD score of 5 is the cut-off value for clinical remission (CDAI score ≤ 150). Use of CDSD might permit a simple, patient-friendly assessment of CD activity, which can provide useful early-phase information on patients with CD as part of their long-term clinical assessment.

    ABSTRACT: Only a few studies have evaluated the population-adjusted prevalence of gallbladder polyps (GBP). This study aimed to evaluate the changes in GBP prevalence and risk factors at a single health screening center in Korea from 2002 to 2012. Of 48,591 adults who underwent health screening between 2002 and 2012, 14,250 age- and gender-matched subjects were randomly selected to evaluate prevalence. Risk factors were analyzed between the GBP-positive and GBP-negative groups during 2002-2004 (Period A) and 2010-2012 (Period B). The annual prevalence of GBP over the 11-yr period was 5.4%. Annual prevalence increased from 3.8% in Period A to 7.1% in Period B. Male gender and obesity were independent risk factors for GBP in both periods. Hepatitis B virus surface antigen (HBsAg) positivity was a risk factor for GBP in Period A but not in Period B. The risk factors for GBP changed from HBsAg positivity to lipid profile abnormalities. Other variables including age, hypertension, diabetes, impaired fasting glucose, chronic hepatitis C virus infection, and liver function tests did not correlate with GBP. In conclusion, GBP prevalence is increasing and risk factors for GBP have changed in Korea. More attention should be paid to this issue in the future.

    ABSTRACT: Malignant transformation or recurrence of intracranial mature teratoma is an extremely rare occurrence, compared to the usual ovarian counterpart. Previously, yolk sac tumor elements have been considered to be selective progenitors of enteric-type adenocarcinoma arising from intracranial germ cell tumors. However, the present case demonstrates the occurrence of enteric-type adenocarcinoma in recurrent intracranial mature cystic teratoma 12 years after gross total removal, a case of which has not previously been documented in the literature. The 11.5-cm long, dura mater-based tumor on the right fronto-temporal lobe displaced the brain; however, the patient had no neurologic symptoms or discomfort other than pus-like discharge on the scalp. Microscopic examinations revealed a small focus of adenocarcinoma and dysplastic colonic mucosa in the mature cystic teratoma. No immature elements were seen. The cystic wall was almost denuded and showed an exuberant xanthogranulomatous reaction with foreign-body type giant cells engulfing keratin materials and cholesterol clefts, suggesting that chronic inflammation due to repeated cyst wall rupture and the previous resection may contribute to malignant transformation. The adenocarcinoma showed strong immunohistochemical expression of CK20 and p53, but CK7 in patches. The molecular profile of the adenocarcinoma showed a mutation in KRAS and wild-type BRAF, which might be associated with malignant transformation of intracranial mature teratomas. In conclusion, the intracranial mature teratomas should require long-term follow-up, and clinicians, radiologists and pathologists should be aware of the potential for malignant progression of recurrent intracranial mature cystic teratoma despite gross total resection and no neurologic symptoms.

    ABSTRACT: The risk of cancer varies with the subtype of colorectal "laterally spreading tumors" (LSTs). However, visual interpretations vary among endoscopists. The aim of this study was to evaluate inter-observer agreement and accuracy in the endoscopic classification of LST subtypes among experts and trainees. In total, 40 LST images were collected and reviewed independently by 14 gastroenterology experts and 10 trainees. All investigators recorded their findings as one of the following four categories: homogeneous, nodular mixed, flat-elevated, and pseudo-depressed. Agreement was assessed in terms of the kappa (κ) statistic and AC1 estimate. Accuracy is reported as percentage agreement with the gold standard, based on the gross morphology of the resected specimens. Of the possible 91 pair-wise κ estimates among experts, 41 (45.1 %) were >0.75, indicating excellent agreement, while only 2 (4.44 %) of the 45 pair-wise κ estimates among trainees were >0.75. Agreements for individual LST subtypes in the trainee group were significantly lower than those in the expert group. The κ and AC1 estimates showed similar values in individual subtypes of LSTs. The overall accuracy of LST was also significantly higher for the experts than the trainees (85.9 vs. 72.5 %, P < 0.001). Notably, the flat-elevated subtype showed the lowest agreement and accuracy and was frequently misclassified as the pseudo-depressed subtype by both groups. Inter-observer agreement and accuracy for LST subtype classification differ significantly between experts and trainees. Implementation of an adequate training system for beginners is necessary to better identify colorectal LSTs.

    ABSTRACT: To compare the esophagogastric junction (EGJ) areas observed in sedated and non-sedated patients during esophagogastroduodenoscopy (EGD). Data were collected prospectively from consecutive patients who underwent EGD for various reasons. The patients were divided into three groups according to the sedation used: propofol, midazolam, and control (no sedation). The EGJ was observed during both insertion and withdrawal of the endoscope. The extent of the EGJ territory observed was classified as excellent, good, fair, or poor. In addition, the time the EGJ was observed was estimated. The study included 103 patients (50 males; mean age 58.44 ± 10.3 years). An excellent observation was achieved less often in the propofol and midazolam groups than in the controls (27.3%, 28.6% and 91.4%, respectively, P < 0.001). There was a significant difference in the time at which EGJ was observed among the groups (propofol 20.7 ± 11.7 s vs midazolam 16.3 ± 7.3 s vs control 11.6 ± 5.8 s, P < 0.001). Multivariate analysis showed that sedation use was the only independent risk factor for impaired EGJ evaluation (propofol, OR = 24.4, P < 0.001; midazolam, OR = 25.3, P < 0.001). Hiccoughing was more frequent in the midazolam group (propofol 9% vs midazolam 25.7% vs control 0%, P = 0.002), while hypoxia (SaO2 < 90%) tended to occur more often in the propofol group (propofol 6.1% vs midazolam 0% vs control 0%, P = 0.101). Sedation during EGD has a negative effect on evaluation of the EGJ.

    ABSTRACT: Dielectric elastomers (DEs) have been suggested as generators to harvest electrical energy from natural mechanical energy sources, such as human movements and ocean waves. In this study, a donut-shaped DE generator (DEG) has been fabricated and its performance is characterized depending on the stretch deformation. A simple new stretchable electrode system using multi-walled carbon nanotubes has been suggested. Measurements on the resistance, capacitance, and electrical power generation are made depending on the area expansion. The capacitance and harvested energy are parabolically increased with increased area expansions. The theoretical prediction of energy harvesting is in good agreement with measured values of capacitance changes with stretching. FE analysis is also applied for calculation of strains for the DEG to figure out the distribution of strains. It is suggested that the DEG has promising applications in the field of designing an energy harvesting device depending on the type of energy available. © 2013 Wiley Periodicals, Inc. J. Appl. Polym.     ABSTRACT: In this article, the effect of deposition amount on self-assembled Au droplets fabricated on GaAs(111)B and (110) is presented. The investigation is systematically performed by the variation of the Au deposition amount from 2 to 20 nm while fixing the other growth parameters such as annealing temperature and duration to clearly observe the effect. Under identical growth conditions, the self-assembled Au droplets show significantly different size and density depending on the amount of Au deposition: i.e. the average height varies by 436% from 21.8 to 95.5 nm and the average diameter swings by 827% from 52 to 430 nm, showing that the size increase is dominated by the lateral expansion. Meanwhile the average density varies by over two orders of magnitude from 1.24 × 108 to 4.48 × 1010 cm−2 on GaAs(111)B. With relatively low Au deposition amounts, below 3 nm, round dome-shaped mini Au droplets with high packing density can be fabricated, while super large Au droplets result with higher deposition amounts, above 10 nm, with a density two orders of magnitude lower. It is also found that the surface index has a minor effect on the fabrication of self-assembled Au droplets with the variation of deposition amount. The results are systematically analyzed and discussed in terms of atomic force microscopy and scanning electron microscopy images, line profiles, power spectrums, r.m.s. surface roughness, and size and density plots.

    ABSTRACT: Interobserver variation by experience was documented for the diagnosis of esophagitis using the Los Angeles classification. The aim of this study was to evaluate whether interobserver agreement can be improved by higher levels of endoscopic experience in the diagnosis of erosive esophagitis. Endoscopic images of 51 patients with gastroesophageal reflux disease (GERD) symptoms were obtained with conventional endoscopy and optimal band imaging (OBI). Endoscopists were divided into an expert group (16 gastroenterologic endoscopic specialists guaranteed by the Korean Society of Gastrointestinal Endoscopy) and a trainee group (individuals with fellowships, first year of specialty training in gastroenterology). All endoscopists had no or minimal experience with OBI. GERD was diagnosed using the Los Angeles classification with or without OBI. The mean weighted paired κ statistics for interobserver agreement in grading erosive esophagitis by conventional endoscopy in the expert group was better than that in the trainee group (0.51 vs 0.42, p<0.05). The mean weighted paired k statistics in the expert group and in the trainee group based on conventional endoscopy with OBI did not differ (0.42, 0.42). Interobserver agreement in the expert group using conventional endoscopy was better than that in the trainee group. Endoscopic experience can improve the interobserver agreement in the grading of esophagitis using the Los Angeles classification.

    ABSTRACT: In this paper, we propose multiple recording process of photopolymer for a full-color multi-view including multiple-view auto-stereoscopic 3D display system based on VHOE (Volume Holographic Optical Element). To overcome the problems such as low resolution, and limited viewing zone of conventional 3D-display without glasses, we designed multiple recording condition of VHOE for multi-view display. It is verified that VHOE may be optically made by angle-multiplexed recording of pre-designed multiple-viewing zone that uniformly is recorded through optimized exposuretime scheduling scheme. Here, VHOE-based backlight system for 4-view stereoscopic display is implemented, in which the output beams that playing a role reference beam from LGP(Light guide plate)t may be sequentially synchronized with the respective stereo images displayed on the LCD panel.

    ABSTRACT: Background/Aims: The eradication rates of Helicobacter pylori infection have been reported to have decreased over the years due to antibiotics resistance. The aim of this study is to investigate the trend of eradication rates of first-line triple therapy for H. pylori over the past 13 year period, and to evaluate factors affecting H. pylori eradication in Daegu and Gyeongsangbuk-do, Korea. Methods: A total of 2,982 patients with H. pylori infection who were treated with either 1 week or 2 weeks first-line therapy (proton pump inhibitor [PPI], amoxicillin, and clarithromycin) from January 1999 through December 2011 were included in this study. Data were collected by retrospectively reviewing the medical records. Results: The overall H. pylori eradication rate was 87.2%. The eradication rates from 1999 to 2011 fluctuated between 78.0% and 95.7%, but no definite evidence of a decreasing tendency was seen over the 13 year period (p=0.113). Furthermore, there was no significant difference in the eradication rate according to the duration of therapy (p=0.592). However, there was a significant difference in the eradication rate among various PPIs (p<0.01). Conclusions: There was no decreasing trend in the H. pylori eradication rate over the past 13 years in Daegu and Gyeongsangbuk-do, Korea. There also was no difference in the eradication rates depending on duration of therapy. However, a significant difference was noted among various PPIs. (Korean J Gastroenterol 2014;63:82-89).

    ABSTRACT: Three-dimensional (3D) reformatted images provide a more inclusive representation of abnormalities than transverse images in cranial computed tomography (CT). The purpose of this study was to assess the value of 3D reformations for radiology residents in the interpretation of emergency cranial CTs. In total, 218 consecutive patients who underwent emergency cranial CT scans with 3D reformation were included in this retrospective study. Four blinded readers (three radiology residents and a neuroradiologist) interpreted the transverse and 3D images in two separate sessions. Each reader assessed 1) abnormal finding(s) and the confidence score(s) (5-point scale) for transverse and 3D images, 2) added value score of 3D images (5-point scale), and 3) interpretation time for both transverse and 3D images. We analyzed discordance between each radiology resident and the neuroradiologist on a lesion-by-lesion basis. In total, 509 lesions were detected in 218 patients. Discordance rates between the three residents and the neuroradiologist were 11.4%-20.2% (mean, 15.0%) and 8.8%-16.9% (mean, 12.1%) in the interpretation of transverse and 3D images, respectively. Confidence scores were higher for 3D images than for transverse images for all readers. The added value scores for the 3D images were relatively higher for the inexperienced residents. Interpretation times for 3D images were significantly higher than for transverse images for all readers. The 3D reformations assist radiology residents in the interpretation of emergency cranial CT examinations.

    ABSTRACT: Endoscopic submucosal dissection (ESD) is now recognized as the preferred treatment modality for gastrointestinal epithelial lesions. A 21-month-old female was admitted with a giant hyperplastic polyp causing a gastric outlet obstruction. Successful ESD was performed with caution. The post-procedural course was uneventful without a bleeding episode. Although further study of the feasibility of ESD in early children is necessary, ESD could be applied to avoid laparotomy even in young children.

    ABSTRACT: Background As a result of increased resistance to antibiotics, Helicobacter pylori eradication rates using standard triple therapy have been declining. Aim To validate the efficacy and tolerability of a concomitant regimen as a first-line treatment for H. pylori infection. Methods A total of 348 naïve H. pylori-infected patients from six hospitals in Korea were randomly assigned to concomitant therapy and standard triple therapy groups. The concomitant regimen consisted of 30 mg of lansoprazole, 1 g of amoxicillin, 500 mg of clarithromycin, and 500 mg of metronidazole, twice daily for 10 days. The standard triple regimen consisted of 30 mg of lansoprazole, 1 g of amoxicillin, and 500 mg of clarithromycin, twice daily for 10 days. Results Concomitant and standard eradication rates were 78.7% (137/174) vs. 70.7% (123/174) by intention-to-treat (p = 0.084) and 88.7% (133/150) vs. 78.4% (120/153) by per-protocol (p = 0.016), respectively. The two groups were similar with regard to the incidence of adverse events. Conclusions Although 10-day concomitant therapy was validated as a suboptimal treatment option for the treatment of H. pylori infection, this regimen is expected to be a promising starting point in the development of an optimal treatment regimen for H. pylori infection.

    ABSTRACT: Video-based human activity recognition (HAR) means the analysis of motions and behaviors of human from the low level sensors. Over the last decade, automatic HAR is an exigent research area and is considered a significant concern in the field of computer vision and pattern recognition. In this paper, we have presented a robust and an accurate activity recognition system called WS-HAR that consists of wavelet transform coupled with stepwise linear discriminant analysis (SWLDA) followed by hidden Markov model (HMM). Symlet wavelet has been employed in order to extract the features from the activity frames. The most prominent features were selected by proposing a robust technique called stepwise linear discriminant analysis (SWLDA) that focuses on selecting the localized features from the activity frames and discriminating their class based on regression values (i.e., partial F-test values). Finally, we applied a well-known sequential classifier called hidden Markov model (HMM) to give the appropriate labels to the activities. In order to validate the performance of the WS-HAR, we utilized two publicly available standard datasets under two different experimental settings, n??fold cross validation scheme based on subjects; and a set of experiments was performed in order to show the effectiveness of each approach. The weighted average recognition rate for the WS-HAR was 97% across the two different datasets that is a significant improvement in classication accuracy compared to the existing well-known statistical and state-of-the-art methods.
