  Acute retinal necrosis (ARN) is characterized by the triad of acute vitritis, peripheral necrotizing retinitis and vasculitis. We report a case of 54-year-old woman with bilateral acute retinal necrosis associated with neuroinfection. Her past medical history included renal transplantation, hypertension and aortic stenosis. Observational case report: Diagnostic investigations included biochemical tests, lumbar puncture, eye ultrasonography and MRI of the brain. Anti-HSV IgG antibody titers were elevated in the blood and cerebrospinal fluid. In MRI T2-mode, inflammatory changes were found in the white matter of the right hemisphere. The patient was treated with systemic acyclovir, itraconazole, metronidazole and ciprofloxacin for 3 weeks. Retinal detachment was observed in both eyes. Acute retinal necrosis can be the single manifestation of herpes virus reactivation in patients after organ transplantation.

• [Peripheral retina in AMD patients].

  To determine the frequency and possible correlation between peripheral retinal lesions and AMD. 92 patients (50-89 years old), with AMD were evaluated by indirect ophthalmoscopy and biomicroscopyTo determine the frequency and possible correlation between peripheral retinal lesions and AMD. 92 patients (50-89 years old), with AMD were evaluated by indirect ophthalmoscopy and biomicroscopy with triple mirror. The control group consisted of 92 patients without any lesions in macula and other retinal changes. At the AMD patients the peripheral benign lesions were found more frequent than in control group (p = 0.003, especially: drusen (p = 0.026), atrophy (p = 0.011), and chorioretinal degeneration (p = 0.001). The presence of peripheral retinal lesions may correlate with AMD evaluation.

  To evaluate changes in the macular thickness and volume using optical coherence tomography in patients after phacoemulsification and intracapsular implantation of a foldable intraocular lens. TheTo evaluate changes in the macular thickness and volume using optical coherence tomography in patients after phacoemulsification and intracapsular implantation of a foldable intraocular lens. The study included 82 patients (37 males and 45 females) after phacoemulsification and intracapsular implantaion of the same type of a foldable intraocular lens, without any other eye disease. Phacoemulsification was performed with an INFINITI machine. In all patients, macular thickness and volume were measured with an optical coherence tomograph (Stratus OCT) using the Fast Macular Thickness Map. The OCT evaluation was performed on days 1, 7, 30 and 90 postoperatively. In 58 patients (71%), it was additionally performed at 12 months after surgery and in 52 patients (63%) the macular parameters in the healthy and operated eyes were compared. A statistically significant increase in the minimal retinal thickness was observed on days 30 (p<0.0005) and 90 (p<0.005) postoperatively compared to post-operative day 1. A statistically significant increase in the foveal volume was seen on days 30 (p<0.00005) and 90 (p<0.0005). A statistically significant increase in the volume of the entire macula was found on days 7, 30 and 90 (p<0.00005). Uncomplicated cataract phacoemulsification is followed by increases in the central retinal thickness, foveal volume and volume of the entire macula on days 30 and 90 and at 12 months postoperatively. Further observation of patients is required to confirm whether the macular parameters will return to their values on day 1 postoperatively and if so, when this will occur.

• Angioid streaks - clinical course and photodynamic therapy. Case report.

  Angioid streaks develop as a result of damage to the Bruch's membrane. They may occur as idiopatic angioid streaks or in association with pseudoxanthoma elasticum, Paget's disease and theAngioid streaks develop as a result of damage to the Bruch's membrane. They may occur as idiopatic angioid streaks or in association with pseudoxanthoma elasticum, Paget's disease and the haemoglobinopathies. Other pathologies of the fundus of eye may coexist together with angioid streaks, such as drusen of an optic disc and granular-like retina in posterior pole known as "peau d'orange". Subretinal neovascularization as well as atrophy in macula are also observed in the cases of long-lasting pathologies. The diagnosis is based mainly on clinical manifestations. Photodynamic therapy in treatment patients with secondary choroidal neovasularisation appeared to be effective.

  Central areolar choroidal dystrophy is localized in macular region and is characterized by atrophy of pigment epithelium, photoreceptors and choriocapillaris. This paper presents the history of two sibilings at the age of 23 and 30, with central aleolar choroidal dystrophy coexisting with alopecia. The results of erg, eog and fluorescein angiography are presented. Conclusion: The results of therapy for glaucoma associated with the Sturge-Weber syndrome are often disappointing.

  PURPOSE: The aim of this study was a clinical analysis of patients with symptoms typical of pemphigoid. MATERIAL AND METHODS: 21 patients were enrolled on the study within the period 1988-2004. ThePURPOSE: The aim of this study was a clinical analysis of patients with symptoms typical of pemphigoid. MATERIAL AND METHODS: 21 patients were enrolled on the study within the period 1988-2004. The frequency and type of ophthalmological and extraocular symptoms and accompanying diseases were assessed. The analysis also contains immunological findings as well as administered treatment. RESULTS: Ophthalmological changes were detected in 100% of examined patients with pemphigoid. The most frequent were dry eye syndrome and conjunctivitis. Immunohistochemical tests revealed the presence of antibodies in 68.2% of the examined cases. All patients required specific treatment--local, general and surgical. Despite the administered therapy the development of cicatricial changes was observed. CONCLUSIONS: Chronic conjunctivitis and keratitis require diagnostics into pemphigoid. Intensive therapy is necessary in patients with ocular cicatricial pemphigoid (OCP) because of high risk of blindness. Delayed diagnostics disable immunohistochemical confirmation of disease.

  PURPOSE: To describe a case of a malignant melanoma of the limbus. MATERIAL AND METHODS: A 49-year-old female patient with a suspected malignant limbic melanoma in the left eye. The first signs appeared in 1975, manifested as reddening of the left eye on the temporal side with markedly dilated conjunctival vessels. In 1993, the lesion became pigmented and nine years later its progression was noted. In May 2003, lamellar keratoplasty of the left eye was performed under general anaesthesia. RESULTS: Histologic assessment--spindle cell malignant melanoma. The entire tumour was resected and the cosmetic result was very good. The results of accessory diagnostic studies were within normal limits. Over a 28-month follow-up period no local recurrence or metastases to other sites were observed. CONCLUSION: Patients with discoloration of the conjunctivae should be regularly evaluated by an ophthalmologist and an oncologist.

  The dry eye syndrome is nowadays known as one of the most frequent reasons of long lasting discomfort and irritation of the eyes. From the clinical point of view, there are many etiologic causes,The dry eye syndrome is nowadays known as one of the most frequent reasons of long lasting discomfort and irritation of the eyes. From the clinical point of view, there are many etiologic causes, several combinations of anatomy pathologic manifestation and different grades of severity of dry eye diseases in the dysfunctional tear film syndrome. The ophthalmologist must recognize this three parameters, quantify them, and establish the most appropriate treatment. So, for this reason, in the 14th Congress of the Societas Ophthalmologica Europaea (Madrid, 2003), a multicenter study developed a practical classification of patients with ocular surface dryness based on etiology, histopathology and clinical severity of symptoms.

  Pseudoexfoliation syndrome is an ocular manifestation of a systemic elastosis. Exact etiology of this condition remains unknown. The basic pathogenetic concept of PEX is a pathological process of thePseudoexfoliation syndrome is an ocular manifestation of a systemic elastosis. Exact etiology of this condition remains unknown. The basic pathogenetic concept of PEX is a pathological process of the extracellular matrix, characterized by the excessive production of an abnormal extracellular material which aggregates and accumulates and is not degraded in vivo. This material is produced primarily by the non pigmented epithelium of the ciliary body, the posterior iris pigment epithelium, and the preequatorial lens epithelium, while the corneal endothelium, trabecular cells, and vascular endothelia and smooth muscle cells of the iris have also been implicated. PEX material has a complex glycoprotein/proteoglycan, composition containing glycosaminoglycans (heparan sulfate, chondroitin sulfate, dermatan sulfate, hyaluronic acid). The prevailing presence of elastic fiber epitopes, mainly elastic microfibrillar components (elastin, vitronectin, amyloid P, fibrillin-1, LTBP-1), has led to the current theory explaining PEX as a type of elastosis, affecting especially elastic microfibrils. Ocular deposition of pseudoexfoliation material can lead to many complications in intraocular surgery like increased risk of zonular dehiscence, capsular rupture, vitreous loss during cataract extraction. Special attention is required before, during and after surgery.

The authors present technique and the range of measurements; topography of both anterior and posterior corneal surface from limbus to limbus, densitometry, anterior chamber angle, volume and depth, lens densitometry and 3D-virtual model of the anterior eye segment are taken. All those structures are shown thanks to rotating Scheimpflug camera, which generates three-dimension images. Any eye movements are detected by a second camera and corrected for, in the process. All results are written down on PC and are available for the prospective studies.

  Cerebral aneurysms are the most common reason of subarachnoid haemorrhage at the age of 50-60. Previous symptoms i. e. headache, ophthalmic disturbances, temporary neurological symptoms are often passed over. The authors present the case of a young woman with cerebral aneurysms, in which the visual acuity impairment was the only symptom of the disease.

  Stargardt's disease is one of the central retinal dystrophies with its onset in first two decades of life presenting gradual decrease in visual acuity and typical features in fluorescein angiography.Stargardt's disease is one of the central retinal dystrophies with its onset in first two decades of life presenting gradual decrease in visual acuity and typical features in fluorescein angiography. The aim of the study was to evaluate the macular area in patients with diagnosed Stargardt's disease with the use of optical coherence tomography. 11 patients (22 eyes) were enrolled to the study. OCT examination revealed characteristic similar features of macular images in all patients. They included decreased thickness of the retina, most prominent in foveola, and changes in external retinal layers: lack of photoreceptors and nuclear external layer and changes in retinal pigment epithelium. Because of typical features in macular appearance, optical coherence tomography may become a useful tool for confirming diagnosis of Stargardt's disease.

  Ocular cicatricial pemphigoid (OCP) is an autoimmune disease characterize by mucous membrane fibrosis and skin changes resulting with scarring. The pathogenic mechanisms of ocular cicatricialOcular cicatricial pemphigoid (OCP) is an autoimmune disease characterize by mucous membrane fibrosis and skin changes resulting with scarring. The pathogenic mechanisms of ocular cicatricial pemphigoid are incompletely understood. Antibasement membrane antibodies which lead to subepithelial blistering, granulation tissue and inflammatory infiltrate formation in the substantia propria are thought to be the main pathophysiological mechanisms in cicatricial pemphigoid. It has been found eosinophils and increased collagen type I and III. Human leukocyte antigen HLA-DR2, HLA-DR4 and DQw7 genotypes have been identified as conferring increased susceptibility to the development of this disease. Ocular cicatrical pemphigoid (OCP) is one of the forms of bullous pemphigoid. Initial symptoms of ocular pemfigoid are not characteristic. Conjunctival fibrosis may cause severe entropion, trichiasis, symblepharon, dry eye syndrome, corneal epithelial erosions or ulceration. Secondary glaucoma is one of the most frequent complications. Ocular cicatricial pemphigoid may be chronic, acute, or subacute disease with periodic exacerbation of conjunctival inflammation. The treatment in this disease are topical drops or ointment (lubricants, corticosteroids, antibiotics, antiglaucomatous). Oral dapsone and corticosteroids may control the activity of the disease. In other progressive cases immunosuppressive drugs must be used (azathioprine, cyclophosphamide, methotrexate, mycophenolan mofetil, daclizumab, intravenous immunoglobulin therapy). To make an early diagnosis of ocular cicatricial pemphigoid, biopsy and immunohistochemical analysis of conjunctiva should be performed in every case of persistent conjunctival inflammation.

• [Ocular manifestations in bone marrow transplantation--case report]

  We present two cases of patients who developed keratitis sicca (dry eyes) and pemphigoid-like symptoms after allogeneic bone marrow transplantation. Skin biopsy revealed changes typical of GVHD. In both cases immunosuppressive treatment, systemic and local, produced improvement of the clinical condition.
