• Effects of a calcium phosphate coating on the osseointegration of endosseous implants in a rabbit model.

  The aim of the present study was to evaluate a Ca-P coated implant surface in a rabbit model. The Ca-P surface (test) was compared to the titanium porous oxide surface (control) in terms of bone-to-implant contact (BIC) and removal torque value. Two hundred and sixteen dental implants were inserted in the tibia and in the femur of 36 rabbits. One hundred and eight were represented by Ca-P oxidized surface implant and other 108 were titanium porous oxide surface modified implants. Each rabbit received six implants. Animals were sacrificed after 2, 4 and 9 weeks of healing. Each group included 12 rabbits. The femoral implant and the proximal implant of the tibia of each animal were subjected to the histologic analysis and the distal implants of the tibia underwent removal torque test (RTQ). Histological analysis in terms of BIC and RTQ did not revealed any significant difference between the Ca-P oxidized surface and the oxidized surface at 2 and 4 weeks. At 9 weeks, the oxidized surface demonstrated better results in terms of RTQ in the tibia. In conclusion, findings from the present study suggested that the Ca-P coating had no beneficial effect in improving bonding strength at the bone-implant interface either at 2, 4 and 9 weeks.

• Orthodontic treatment of periodontally involved teeth after tissue regeneration.

  In this consecutive series, 14 patients with severe intrabony defects and pathologic tooth migration were treated with guided tissue regeneration (GTR) and subsequent orthodontic therapy in anIn this consecutive series, 14 patients with severe intrabony defects and pathologic tooth migration were treated with guided tissue regeneration (GTR) and subsequent orthodontic therapy in an attempt to evaluate the validity of this multidisciplinary approach. Probing pocket depths (PPD), clinical attachment levels (CAL), and gingival recessions were assessed at baseline, 1 year after GTR, and at the end of orthodontic therapy. Radiographs were obtained at all time points. Esthetic parameters were recorded with the papilla presence index (PPI). Statistical analyses were carried out to compare the data at each time point. From baseline to 1 year after GTR, the mean PPD reduction was 5.57 mm, with a residual mean PPD of 2.71 mm; mean CAL gain was 5.86 mm. Both differences were statistically significant. There were no statistically significant differences between 1 year after GTR and the end of orthodontic therapy (mean PPD reduction 0.07 mm; mean CAL gain 0.43 mm). The reduction in PPI reflected the enhancement of papilla height that was observed in 9 of the 14 patients. Within the limits of this research, this study affirms the possibility of a combined orthodontic-periodontal approach that prevents damaging the regenerated periodontal apparatus and produces esthetic improvements as a result of realignments and enhancement of papilla height.

• Fatigue in the masseter and temporalis muscles at constant load.

  Fatigue is usually defined as the point at which a particular level of force can be no longer maintained. In the present study, surface EMG of the masseter and temporalis anterior muscles wasFatigue is usually defined as the point at which a particular level of force can be no longer maintained. In the present study, surface EMG of the masseter and temporalis anterior muscles was measured in ten healthy young adults performing a unilateral molar (right side) clench. The subjects clenched on a bite force transducer at a fixed force level of 13 kg (127 N) as long as they could (endurance). The test ended when the subjects could no longer produce the required bite force. From the EMG recordings, the median power frequency was calculated at the beginning of the task (T0), after one minute of clenching (T1), and at the end of the task (T2, endurance time). For each subject and muscle, percentage decrements in the median power frequency were also computed at T1 and T2. Endurance time ranged between 79 and 470 s. Significant modifications in the median power frequency in both masseter muscles (right side, p=0.003; left side, p=0.02, analysis of variance) were found, with a significant difference for the median frequency at T2 (p<0.02 at post hoc test). The modifications in the temporalis muscles were not significant (p>0.05). Additionally, at T1, significant percentage decrements in the median power frequency were found for both right side muscles (p<0.05, paired Student's ). The left side muscles modifications (p>0.05) were not significant. A significant effect of side was found (p=0.007, analysis of variance), without effects of muscle and no muscle x side interaction. At T2, both masseter muscles and the right side temporalis had a significant modification in their median power frequency. Overall, the modifications were larger in the masseter than in the temporalis muscles (p=0.022, analysis of variance), without effects of side and no muscle x side interaction. In conclusion, a fixed submaximal muscular contraction provoked fatigue modifications in the EMG power spectra that were well comparable to those obtained in previous investigations using forces computed as percentages of individually assessed maximum bite forces The present protocol (endurance clenching at a fixed force level) could be used as both a research and a clinical tool that would allow an easier and less troublesome assessment of both healthy persons and patients.

• Soft tissue facial areas and volumes in subjects with Down syndrome.

  The objective of this study was to supply quantitative information about the facial soft-tissues of a group of subjects with Down syndrome. The three-dimensional coordinates of 28 soft-tissue facialThe objective of this study was to supply quantitative information about the facial soft-tissues of a group of subjects with Down syndrome. The three-dimensional coordinates of 28 soft-tissue facial landmarks were obtained by an electromechanic digitizer in 17 male and 11 female subjects with Down syndrome aged 12-45 years, and in 429 healthy individuals of the same age, ethnicity, and sex. From the landmarks, facial areas (eyes, ears, nose, and lips) and volumes (nose and lips) were calculated according to a geometrical model of face. Data were compared to those collected in the normal subjects by computing z-scores. Male and female z-scores were not significantly different. Most of the facial volumes were significantly (Student's t, P < 0.05) smaller in subjects with Down syndrome than in their normal controls. Ear areas were significantly reduced (mean z-scores in males -2.07 right, -1.9 left; in females -2.11 right, -2.21 left), as well as nasal surface area (mean z-score -1.53 in males, -2.45 in females). In women, age and some z-scores were significantly correlated (P < 0.05): upper lip volume (r = 0.714), left and right eye area (r = 0.635, right; 0.604, left), nasal area (r = 0.603): with increasing age, the negative values of the z-scores approached the 0 value of the reference population. In the pooled sample, age and the z-score of the total lip area were significantly correlated (r = 0.423): increasing age corresponded to a larger z-score value. The method allowed a simple, low cost, fast, and noninvasive examination of the subjects, and provided a quantitative assessment of the deviation from the norm.

• Maximal bite forces in healthy young adults as predicted by surface electromyography.

The protocol could be used in a clinical context to obtain indicative values for the occlusal loads to be resisted by the prosthetic reconstructions. METHODS: Ten young healthy subjects performed; (1) a maximum voluntary clench (MVC) directly on their occlusal surfaces; (2) four simultaneous recording of submaximal bite forces (98, 196, 304 and 392 N on two transducers positioned on the left and right first mandibular molars) and surface EMG potentials of the masseter and temporalis anterior muscles. The actual force peak value was recorded. For each subject, a linear regression was run between the simultaneous bite force and EMG submaximal values recorded in the four tests. Using the EMG potentials obtained during the MVC tests, the best fitting line was used to estimate a maximum bite force. Two independent recordings were made by each subject (2 week interval) and analyzed by correlation analysis, paired Student's t-test, and Dahlberg statistic. RESULTS: Significant linear relationships were found between bite force and EMG potentials (p<0.01). The two series of indirect estimates of maximal bite force were correlated (r=0.626) without systematic differences (Student's t, p>0.01). Dahlberg statistic was 115.37 N (approximately 36% of the total variance of the group). CONCLUSIONS: Simultaneously recorded submaximal bite forces and surface EMG potentials of mandibular elevator muscles had a linear relationship. The estimates of maximum bite force were repeatable on a short-term basis (2 weeks). The method limited the disadvantages of bite force recordings, and it could be used to obtain indicative values for the occlusal loads to be resisted by the prosthetic reconstructions.

• Soft tissue facial anthropometry in Down syndrome subjects.

  The three-dimensional coordinates of 50 selected soft tissue facial landmarks were digitized on 28 white Italian subjects with Down syndrome (17 male and 11 female subjects aged 12 to 45 years) andThe three-dimensional coordinates of 50 selected soft tissue facial landmarks were digitized on 28 white Italian subjects with Down syndrome (17 male and 11 female subjects aged 12 to 45 years) and 429 healthy controls of comparable ages by an electromechanical instrument. From the landmarks, 16 facial dimensions were calculated. Data were compared with those collected in healthy individuals by computing z-scores. Overall, most variables were smaller in subjects with Down syndrome than in their normal controls selected for sex, age, and ethnicity (negative z-scores), even if not all of them reached statistical significance. Independently of sex, subjects with Down syndrome had faces that were significantly (P <0.05, paired Student t test) narrower (skull base and mandible), less deep (upper, middle, and lower face), and shorter (face and nose height) than the faces of normal subjects. Additionally, ear width and length were significantly reduced on both sides of the face. Only facial height was significantly different between sexes (P = 0.023, unpaired Student t test), with a female z-score that was more than two times the relevant male value. The present investigation represents the first detailed quantitative analysis of the facial soft tissue characteristics of Italian white subjects with Down syndrome.
