  • Article: [An experimental study of the cervical lymphatic imaging in interstitial magnetic resonance lymphography of tongue using Dextran-DTPA-Gd].

    ABSTRACT: To explore the application value of the cervical lymphatic imaging in interstitial magnetic resonance lymphography using submucosal injection of Dextran-DTPA-Gd. 0.2 mL Dextran-DTPA-Gd (3.96 mmol/L) was injected into the submucosa of the bilateral lingual margins in 12 New Zealand rabbits,and then massaged the injection site for 30 seconds. MR images were obtained before injection and 10, 15, 20, 25, 30, 35, 40, 50, 90 minutes after injection by 3D TOF CE-MRA sequence.The signal intensities of cervical lymph node were measured, the enhancing rates(E%) were calculated and the signal enhancing rates -time curve was drawn. The data was analysed using SPSS11.5 software package. The cervical lymph nodes,the first and second lymphatics were strengthened significantly after injecting Dextran-DTPA-Gd, but the blood vessels were not enhanced at the same time. The enhancing rates of cervical lymph node reached the peak(344%) at 30-min,and the best strengthening effect was achieved between 20-min and 50-min. As IMRLG contrast agent,the Dextran-DTPA-Gd could image lymphatic drainage lines of the neck and the cervical lymph nodes efficiently.

  • Article: [The effect of hyperbaric oxygen therapy on treatment of late healed wounds after pharyngeal and laryngeal surgery].

    ABSTRACT: To investigate effect of hyperbaric oxygen (HBO) therapy on treatment of late healed wounds after pharyngeal and laryngeal carcinomas surgery. Eighty-three patients with late healed wounds after pharyngolaryngectomy were divided into two groups, which were HBO treatment group (n = 48) and control group (n = 35) by random. 48 cases were treated with the general therapeutic measure along with HBO (the pressure is 2 x 10(5) Pa, 110 minutes per day for 5 to 10 days). The other 35 cases were treated with the general therapy without HBO as control. There were fresh granulation erupted from the wound of entry after 3 or 4 days of HBO) treatment, then the granulation and the new blood capillary erupted continually to fill the wound surface, and the wound of entry was healed over finally. In the HBO and the control groups, the average healing time was 27.50 and 45.00 days (P < 0.01) in the wound by necrosectomy of the myocutaneous flap or forearm flap, 8. 50 and 14.09 days (P < 0.01) in the pharyngeal fistula, 5.93 and 8.62 days (P < 0.01) in the infected or fluid-filled wound. HBO therapy is an effective and powerful treatment for late healed postoperative wounds in pharyngeal and laryngeal carcinomas surgery.

    ABSTRACT: To investigate the expression of collagen type I (Col I), collagen type IV (Col IV) and fibronectin (Fn) in laryngeal squamous cell carcinomas (LSCC) and their pathobiological relationship with invasion and metastasis of tumor. The expression of Col I, Col IV and Fn was detected by immunohistochemistry method in normal tissue of latero-carcinoma and tissue of carcinoma in 60 specimens of LSCC. Integral optical density (iOD) was detected by image analysis of computer and was analyzed by SPSS13.0. The expression of Col I was obvious and integrity. The expression of Col IV and Fn of basal membrane was like intact line-shape appearance and Fn of interstitial substance appeared like a complete network in the normal tissue of latero-carcinoma. Their expression decreased gradually and their integrity was broken and disappeared gradually from well to poorly differentiated LSCC. Their expressions also fell off with the tumor size, clinical stage and cervical lymphatic metastasis gradually and consistently in LSCC. Their variances were statistical significance separately (P<0.05 or P<0.01). The expression of Col I, Col IV and Fn was closely related to tumor invasion, the regional lymph node metastasis and other some pathobiological features in LSCC. A detection of Col I, Col IV and Fn was of definite significance on a better comprehension of the possibility of metastasis, choice of surgery and prognosis.

  • Article: [A comparison between CT and SPECT in detecting mandibular invasion by lower gingival squamous cell carcinoma].

    ABSTRACT: The purpose of this study is to compare the value of CT and SPECT in diagnosis of lower gingival carcinoma invading the mandible. From February 2002 to October 2006, twenty-one patients with lower gingival squamous cell carcinoma were enrolled.The data of CT and SPECT were studied,and compared with histopathological findings. Among the 21 patients,the sensitivity, accuracy, negative predictive value and Youden's index of SPECT were 100.00%, 95.24%, 100.00% and 1.00,respectively. While the sensitivity, accuracy, negative predictive value and Youden's index of CT were 80.00%, 80.95%, 20.00% and 0.80, respectively. There were four false negatives assessments of bone invasion(80.00%) by CT scan, while no false negatives by SPECT. SPECT is superior to CT, and can be used as a routine screening method to assess lower gingival carcinoma invading the mandible.

  • Article: [The value of single photon emission computed tomography-computed tomography in diagnosis of carcinoma invasion of mandible].

    ABSTRACT: To investigate the value of single photon emission computed tomography(SPECT)-computed tomography(CT) in diagnosis of oral carcinoma invasion to mandible. Thirty-four patients with oral carcinoma invasion to mandible were divided into two groups, group A (surrounding invasion) and group B (central invasion). The edge of the invasion was evaluated by SPECT-CT, CT and pathological examination. The results of CT and SPECT-CT were analyzed by quantitative methods. In group A, the cancer-invaded area of the mandible exhibited on SPECT-CT was 1.0 cm lager than that on pathological examination, 2.4 cm lager than that on CT. The difference of invaded area shown on CT was 1.4 cm smaller than that of pathological examination. There were significant difference among the three methods. In group B, the affected area on SPECT-CT was 1.2 cm lager than that of pathological examination, 4.2 cm lager than that of CT. The invision area on CT was 3.0 cm smaller than that of pathological examination. There were significant difference among the three methods. SPECT-CT could find the jaw central tumor earlier than CT and the range of lesion showed by SPECT-CT was the adequate range of bone incision during operation. The range of lesion showed by CT was influenced by the type of tumor and the range of bone incision was determined according to the pathological type. If the false negative result was eliminated, only SPECT-CT or CT was needed to estimate the invasion range of mandible.

  • Article: [Early laser intervention of hemangioma in facial and neck regions of infant].

    ABSTRACT: The study was to evaluate the method of early laser intervention of hemangioma in facial and neck regions of infant. Between January 1999 and December 2006, twelve patients, aged 6 days to 3 months, with cutaneous hemangioma in facial and neck regions, were treated with laser, eight cases with Nd:YAG laser therapy and four cases with Venus laser therapy. Four cases with hemangioma in facial and neck regions of infant treated with oral corticosteroid were as control. The outcome was recorded with 1 to 6 years of follow-up. Total resolution was obtained in twelve patients with laser intervention. Atrophic scars occurred in eight patients with Nd:YAG laser therapy, without other complications, such as ulceration, life-threatening hemorrhage and et al. No scar occurred in four patients with Venus laser therapy. Recurrence was not seen in twelve cases with laser therapy with follow-up. Hemangiomas enlarged continuously in four cases with oral corticosteroid therapy. Early laser intervention is an excellent management of cutaneous hemangioma in facial and neck regions of infant.

  • Article: [Early treatment of middle ear disease in cleft palate infants].

    ABSTRACT: To explore the effect of myringotomy with insertion of tube and tympanocentesis on alleviating secretory otitis media (SOM) and hearing loss in cleft palate infants. Nineteen cleft lip and palate infants with SOM and hearing loss (38 ears) were treated with myringotomy with insertion of ventilation tube at the same time of repair of the cleft lip, who were performed averagely at 6.9 months of age. Fifteen cleft lip and palate infants with SOM (30 ears) were treated with tympanocentesis at the same time of repair of the cleft lip averagely at 6.6 months of age. All cases were followed up from 1 week to 12 months and estimated by auditory brainstem response (ABR) and acoustic immitance audiometry. The average wave V reacting thresholds of ABR were separately 55.41 dBnHL and 28.48 dBnHL, and 79.17% tympanogram B changed to tympanogram A in cleft palate infants with insertion of tube before and after operation. The average wave V reacting thresholds of ABR were separately 40.63 dBnHL and 26.50 dBnHL, and 40.91% tympanogram B changed to tympanogram A in cleft palate infants with tympanocentesis preoperatively and in 1 week postoperatively, in whom the average hearing thresholds and tympanograms had no significant difference preoperatively and in 1 or 3 months postoperatively (P>0.05). The early myringotomy with insertion of tube is effective to restore the function of the middle ear in cleft palate infants with SOM, so to suggest to be performed at the same time of the repair of cleft lip within the first 1-year of life. The tympanocentesis should not be used as a regular management in the cleft palate infants with SOM.

  • Article: [Cytotoxicity study of composite oyster shell paste as a root canal filling material].

    ABSTRACT: To evaluate the cytotoxicity of new root canal filling material-composite oyster shell paste. According to the evaluated criteria of medical materials, biology and animal trials recommended in ISO 10993,the extracts of powder part and the mixture of the new material were prepared separately and selected as the experimental. The RPMI1640 subject was used as the negative control and the V79 cell as the experimental cell. The cells were cultured in extract of different thickness compound for 2,4,7 days respectively and the changes of the cells were observed under microscope.The proliferation of the cells were examined and the optical density(OD)was obtained using MTT assay. SPSS12.0 software was used for statistical analyses. During all culture periods, the V79 cells showed normal morphology and proliferation increased with culture time. The OD of the experimental groups were not significantly different from that of the negative groups(P>0.05)and the OD increased as time goes on. The new root canal filling material shows no detected cytotoxic effect from this study.

  • Article: [The characterization of scaffold made of N,O-carboxymethyl chitosan and tricalcium phosphate].

    ABSTRACT: To study the mechanical and chemical characterization of scaffold made of N,O-carboxymethyl chitosan (N,O-CMCS) and tricalcium phosphate(TCP) with various weight ratios. Three composites of N,O-CMCS/TCP with weight ratios of 2:1,1:1 and 1:2 were mixed using the same volume of distilled water. The characteristic bands, morphologic structure, compressive strength and Young modulus of the composites were analyzed by infrared spectroscopy (IR),scanning electron microscopy(SEM) and universal testing machine respectively. The shift of IR characteristic bands of -COO(-)and -NH(2) to lower wave number was observed in three composites gradually, which resulted from the complexation of -COO(-) and -NH(2) with calcium ions. The pores of the three-dimensional porous structure and compressive strength of the composite increased when there were more N,O-CMCS in the composite. When there were more TCP in the composite, its porosity became less and Young modulus increased, so that the composite demonstrated more rigidity. N,O-CMCS had the capability of complexing with calcium ions. The scaffold made of N,O-CMCS and TCP had the properties of three-dimensional porous structure, flexible remolding and mechanical strength, so it will be a promising biomaterial for bone substitute.

  • Article: [Incidence and natural history of middle ear disease in newborns and infants with cleft palate].

    ABSTRACT: The aim of this study is to investigate the incidence and natural history of secretory otitis media(SOM) and hearing loss in newborns and infants with cleft palate, consequently, define its audiological criteria and to predict SOM early. Seventy-three newborns and infants with a cleft palate (146 ears) were monthly estimated by tympanogram, static compliance, acoustic stapedius reflex and auditory brainstem response (ABR) under natural sleep within one year of age. Au the infants with cleft palate had the suspected SOM in the first 6 months of life. Among children with cleft palate, the suspected SOM were most prevalent in the 3-month-age. 78. 8% infants with cleft palate had the confirmed SOM in the first 12 months of life. SOM were most prevalent in the 6-month-age. The SOM prodromal period was averagely 3. 8 months from suspected SOM to confirmed SOM. 56. 2% infants with cleft palate had a conductive hearing loss in the first 12 months of life. The conduction hearing thresholds of ABR (2-4 Hz) were averagely 48. 6 dBnHL. The highest incidence of SOM and hearing loss in children with cleft palate appear in infants in the first 1 year of life. The process of SOM and hearing loss onset is progressive process. The infants with cleft palate should be estimated by ABR and acoustic immittance audiometry in each period of 2 or 3 months after birth.

  • Article: [Resection of carotid body tumor under interrupted block of common carotid artery].

    ABSTRACT: 7 patients with carotid body tumor (CBT) were evaluated by DSA preoperatively. Carotid artery (CA) blocking test was carried out under local anesthesia before operation. With CA blocked temporarily, tumors were resected under general anesthesia after the test was normal. Meanwhile some cases underwent restoration with CA repair, end-to-end anastomosis or artificial vascular grafting. No case underwent internal bypass. CA was blocked 2 to 4 times in operation. The average blocking time of CA was 22 minutes and the longest blocking time of that reached 28 minutes. The average blood loss of cases without embolization preoperatively was 1100 ml, cases with embolization was 480 ml. 3 cases had nerve complications. DSA is of important value in the diagnosis and treatments of CBT. Temporary blocking artery method can help to judge when to use internal bypass, make operation easy, decrease blood loss and nerve complications.

  • Article: [Using fenestration and decompression technique to treat enormous odontogenic keratocyst].

    ABSTRACT: 3 cases of enormous odontogenic keratocyst were treated with the technique of fenestration and decompression other than removal of jaws. After a follow-up period of two years, radiographic assessment showed that the image of keratocyst disappeared basically, neogenetic bone had been found and the defect of jaws had been restored. The teeth had no dysfunction, the lower lip had no numbness and the keratocyst epithelium was modulated histologically to mucosa after decompression. Fenestration and decompression are satisfied conservative approaches to treat enormous keratocyst which usually is treated by removal of jaws and extraction of teeth.

    ABSTRACT: The purpose of this study was to study the masticatory muscles affected by distraction osteogenesis of the mandible. The distraction osteogenesis (DO) was applied to distract the left mandible of 6 mongrel dogs that were divided into three experimental groups. After different distraction phase and consolidation phase, the masseter and the digastric muscle were taken out. The specimens were stained using hematoxylin/eosin and enzyme histochemistry. Afterwards, the specimens were observed with a light microscope to study the morphologic changes of the muscles. The contents of enzyme in the different groups were measured by VIDAS. The masseter showed consequently atrophy, but the digastric muscle showed a progress of histomorphologic reconstruction, including atrophy and hypertrophy. The changes of the contents of enzyme and histomorphology were identical in the masticatory muscles. The digastric muscle parallel to the vector of mandibular distraction adapts the distraction by the way of atrophy, regeneration and hypertrophy. And the contents of enzyme appear to decrease at the beginning, increase afterwards, and return to the normal level finally. But the masseter perpendicular to the vector of mandibular distraction shows consequent atrophy, and the contents of enzyme consequently decrease, which means the metabolism decrease.
