  • Article: [Using attachment measures in the context of providing expert witness through psychiatric assessment in family court proceedings].

    ABSTRACT: Prognostic evaluation of child development in the context of his/her actual family situation plays an important role in family law disputes. However, there is a lack of empirically validated instruments to assess socio-emotional development in very young children. Attachment research provides instruments which could be utilized in clinical practice. At this, the focus should be on assessing the quality of parent-child-relationship and the occurrence of risky parenting behaviour. The article illustrates the use of attachment measures in the context of a family court proceeding according to subsection 1666 German Civil Code. Risk assessment is carried out through direct observation of the quality of interaction between mother and ten months old infant as well as through evaluation of the attachment representations of both parent caregivers. Instruments used are the Strange Situation Test (to assess infant attachment), the CARE-Index (to assess parental sensitivity), the Adult Attachment Interview, and the Adult Attachment Projective (to assess parental attachment representations).

  • Article: [Evaluation of the E-learning training, early prevention and intervention in child protection"].

    ABSTRACT: Prevention, intervention, and child protection in early childhood essentially need well-established interdisciplinary systematic networking. Currently there is a lack of education, training and qualification programs which take the specific demands for interdisciplinary knowledge about the early childhood as well as practical experience about early prevention and intervention in child protection into account. The Internet-based E-Learning training "Early Intervention and Child Protection" was developed to fill this gap. A pilot-study with 45 test-users showed, that they learned significantly while using the platform and changed aspects of their everyday routine. Thereupon the platform was opened in June 2011 to all professionals. At the moment 2,300 people are registered and participate at the training. The aim of this study, namely to make a systematic training on early prevention and intervention in child protection wide-ranging available, was achieved

    ABSTRACT: Nicht zuletzt durch die Etablierung einer entwicklungspsychopathologischen Perspektive als eines eigenständigen Bereichs an der Schnittstelle zwischen Kinderpsychiatrie und Entwicklungspsychologie (Cicchetti 1984; Sroufe u. Rutter 1984) wurde die Bedeutung sicherer bzw. unsicherer Bindungserfahrungen für die Selbst- und Persönlichkeitsentwicklung von Kindern zunehmend klinisch diskutiert (Rutter u. Sroufe 2000). Für die Erlebens- und Verarbeitungsweisen eines Kindes mit Bindungsstörung wird unterstellt, dass seine Bedürfnisse nach und seine Gefühle von emotionaler Sicherheit so nachhaltig gestört sind, dass das Risiko dauerhaften Leids bzw. der Entwicklung einer psychopathologischen Störung besteht (Wakefield 1992). Es handelt sich um Kleinkinder, die keine exklusive und eindeutig bevorzugte Bindung mit einer vertrauten Bezugsperson entwickelt haben (Rutter et al. 2009; von Gontard 2010; AAPAC 2005).

  • Article: [Epidemiology of risks and child maltreatment in early childhood: a pilot study].

    ABSTRACT: There is a lack of prevalence data of child maltreatment and neglect in German speaking countries. The aim of this pilot study was to develop a questionnaire as a generally useable method for surveys of early child maltreatment and to test its feasibility and usefulness. The pilot study of this methodology would also give a first indication of the prevalence of risk factors and of child maltreatment itself. The study was carried out in 21 day-care centres in Vorarlberg, Austria. The questionnaire was judged as very user-friendly and useable. Prevalence was 5.3 % for neglect and 1.2 % for abuse in the study sample. The results demonstrate that systematic data collection in the field of youth welfare is practicable in German speaking countries. In addition, day-care centres allow easy access to data collection. Further psychometric testing of the questionnaire with normative data samples and across different professional groups is required.

  • Article: Assessing parent--child interaction in the preschool years: A pilot study on the psychometric properties of the toddler CARE-Index.

    ABSTRACT: The purpose of this study was to test the utility of the Toddler CARE-Index, an assessment tool for parent-child interaction among preschool-aged children, for screening of parental sensitivity. The CARE-Index was initially developed for infants and had been adapted for toddlers up to 3 years of age. This study tests its utility for children up to 5.8 years old. Sixty-four children (2.3 to 5.8 years) and their mothers took part in the study and were examined with both the Toddler CARE-Index and the Preschool Assessment of Attachment. The sample comprised two groups, a sample that had come to professional attention (n = 21) and a normative sample (n = 43). Analysis of coder agreement showed adequate correspondence among three coders. Test-retest reliability was less robust. Testing validity, there was a significant relation between sensitivity of the mother and attachment security of the child in the total sample as well as in both subsamples. These results are a first step for using the Toddler CARE-Index as an economical and promising instrument for the assessment of parental sensitivity with children beyond toddlerhood in both normative and clinical settings.

  • Article: [Cooperation and networking in early prevention and intervention and child protection: the importance of evidence-based methods].

    ABSTRACT: Prevention, intervention and child protection in early childhood essentially need well-established interdisciplinary systematic networking. Individual, heterogeneous and complex needs of families cannot be met by one profession alone. Successful cooperation of various institutions and professions is based on fixed arrangements and cooperation pathways. Networking has to be systematically established in everyday routine to be able to work in difficult emergency cases of child protection. Only well established cooperation is experienced as a support for the participants of the network and not as an additional complication. Prerequisite for such a development of favorable conditions is an evidence-based knowledge of the impact of different structures of cooperation, relations and conditions. This requires data collection with the aim of further empirically based development of local network structures. Three tools for such surveys have been developed and tested in the field.

  • Article: [Promoting sensitivity and parenting competencies in teenage mothers].

    ABSTRACT: Relevant concepts of parenting emphasize biologically oriented behavioral tendencies as intuitive parenting that unconsciously guides interaction with the infant. They can be violated by risk factors or adverse life conditions and are to be perceived in parental problems to cope with the infants' needs and even in neglecting and maltreating behavioral displays. In particular high risk groups as teenage mothers are affected. Early and preventive intervention efforts promoting parent-child relationship seem to be successful to prevent maltreatment and later behavioral problems in the child. An attachment-oriented and video-based program for intervention with teenage mothers is described. Chances and limits of promoting parent-child attachment in teenage mothers are discussed.

  • Article: [Risk assessment of neglect and maltreatment in infants and toddlers. Chances of early preventive intervention].

    ABSTRACT: The German child protection system lacks of interdisciplinary and standardized procedures for screening and diagnosing child maltreatment as well as for service delivery. Evidence based assessments of risk-screening in combination with consensus-based models for risk-judgement seem to predict the best possible results. A central part of determining whether an infant is at risk is to evaluate various domains of parental competencies and skills. In particular assessment procedures based on attachment research have proven to be practically and methodologically relevant. These include interactive and video-based observational methods as well as parental representations as sources of information for risk assessment. Attachment based intervention and treatment programs are effective, especially with regard to enhancing paternal sensitive behavior. However, these programs need to be adapted to delivery for specific risk groups. Overall an interdisciplinary approach with regard to the programs as well as to the training of the professionals has to be taken into account.

    ABSTRACT: Neglect and threatened welfare in childhood have a background of complex interactions of a multitude of risk factors. Their high level of vulnerability put infants and young children at particular risk in regard to their social and emotional, cognitive and physical development, and a life-threatening situation can quickly arise. The immediate threats to endangered children are rooted in the deficiencies of their parents with regard to upbringing, and their incompetence in the field of interpersonal relationships. As a rule, an imminent threat to the child's wellbeing is signaled by the negative quality of parent-child interaction, resulting in an early bonding problem. As a supplement to the routine diagnostic work-up, therefore, early interaction diagnosis is recommended. This enables (as yet) fragile signs of incipient neglect or threats to the child's well-being to be recognized in good time.

  • Article: [Managing self determination and sexual violence in homes for young adults with mental handicap--report from an ongoing German model project].

    ABSTRACT: This report is based upon a model project financially assisted by a grant from the German Federal Ministry for Families, Senior Citizens, Women and Youth, starting in 1999 and ending at the end of 2003. In cooperation with two disability care residential institutions, a conceptual approach and method to the questions of sexual self-determination and sexualized violence is being developed. Through qualitative methods of research e.g. focus groups, professional helpers of all hierarchies of the institution, including management, and the residents themselves, all had the chance to contribute their own opinions and experiences to the research topic. Specifically, to ensure their voice was heard, the people with mental retardation had an important impact on the questionnaire themselves. All aspects of sexuality were discussed--with notable difficulty arising in particular over the subject of sexual violence. It turned out that nonverbal communication and the interaction between the group members in the residents group were most indicative of their concerns. The staff at the nursing and living areas discussed the following topics: distance and closeness in the interaction between staff and residents, standards, reflections of their own professional attitudes, questions of legality as well as the tense topic of individual needs and tasks of the group. How those questioned described their solutions and ways of coping, and the impressions of the researchers forms the starting point for the compiled work. The initial ideas for the topics and the design of the concept are now laid out.

    Article: Risikoeinschätzung bei Vernachlässigung und Kindeswohlgefährdung im Säuglings- und Kleinkindalter - Chancen früher beziehungsorientierter Diagnostik

    ABSTRACT: In Deutschland fehlt eine interdisziplinäre Verständigung auf standardisierte und wissenschaftlich geprüfte Risikoscreening- und Diagnoseverfahren sowie auf Vorgehensweisen und Angebote bei Vernachlässigung und Kindeswohlgefährdung. Für die Risikoeinschätzung haben sich Vorgehensweisen bewährt, die auf empirisch geprüfte Verfahren zurückgreifen und auf Grundlage aller Einzelergebnisse eine zusammenfassende "klinische Synopse" vornehmen. Bei Säuglingen und Kleinkindern mit besonders hoher Gefährdung hat es sich bewährt, elterliche Erziehungs- und Beziehungskompetenzen als relevante Faktoren bei der Risikoeinschätzung, aber auch als Ansatzpunkt für Prävention und Intervention zu berücksichtigen. Hierzu gehören bindungsorientierte und videogestützte Interaktionsdiagnostik beziehungsweise der Einbezug elterlicher Bindungsrepräsentationen. Ebenso haben sich für Intervention und Versorgung bindungstheoretisch konzeptualisierte Angebote zur Förderung elterlicher Feinfühligkeit bzw. zur Förderung sicherer Bindung als wirksam erwiesen. Eine interdisziplinäre Ausrichtung sowohl der Programme als auch der Fachkräfte, die solche Angebote durchführen, wird für wesentlich erachtet. (ZPID)
