Cardiorespiratory events detected by home memory monitoring and one-year neurodevelopmental outcome.

ABSTRACT To determine if infants with cardiorespiratory events detected by home memory monitoring during early infancy have decreased neurodevelopmental performance.
Infants (n = 256) enrolled in the Collaborative Home Infant Monitoring Evaluation also completed the Bayley Scales of Infant Development II at 92 weeks' postconceptional age. Infants were classified as having 0, 1 to 4, or 5+ cardiorespiratory events. Events were defined as apnea >or=20 seconds or heart rate <60 to 80 bpm or <50 to 60 bpm, for >or=5 to 15 seconds, depending on age.
For term infants, having 0, 1 to 4, and 5+ cardiorespiratory events was associated with unadjusted mean Mental Developmental Index (MDI) values (+/-SD) of 103.6 (10.6), 104.2 (10.7), and 97.7 (10.9), respectively, and mean Psychomotor Developmental Index (PDI) values of 109.5 (16.6), 105.8 (16.5), and 100.2 (17.4). For preterm infants, having 0, 1 to 4, and 5+ cardiorespiratory events was associated with unadjusted mean MDI values of 100.4 (10.3), 96.8 (11.5), and 95.8 (10.6), respectively, and mean PDI values of 91.7 (19.2), 93.8 (15.5), and 94.4 (17.7). The adjusted difference in mean MDI scores with 5+ events compared with 0 events was 5.6 points lower in term infants ( P = .03) and 4.9 points lower in preterm infants ( P = .04).
Having 5+ conventional events is associated with lower adjusted mean differences in MDI in term and preterm infants.

    ABSTRACT: Summary Question of the study There is still a demand for secure home-monitoring equipment for infants. The constructors of modern monitors should especially aim at reducing the rate of false alarms. The indication for home monitoring of a child must be as critically examined as the question of which type of device should be used. Patients and methods Two hundred and thirty-six randomly selected parents were offered the free use of a standard device to monitor their infants at home. Periods of application and general compliance were then assessed for 1 year by means of a longitudinal study. Furthermore, on 32 term-eutrophic infants, frequency of alarms were measured in their first 3 months of life (home monitoring study). In a second clinical trial under sleep laboratory conditions on 59 infants, the possibility of reducing the rate of false alarms by using logistic signal linking was examined with a newly developed monitor with integrated software (False Alarm Study). Results When monitoring babies using commercially available devices, there is a high rate of false alarms. This fact has a negative influence on the acceptance and therefore on the use of these monitors. A reduction of false alarms is obtained with the new type of monitoring equipment. Conclusion Further development of signal interconnection methods should improve the quality of monitoring devices. Significance can be improved by retrospectively analyzing the recorded alarms and by replacing event recording with continuous recording thanks to modern equipment with high memory capacity. German Zusammenfassung Fragestellung Nach wie vor existiert ein Bedarf nach einer sicheren Überwachungsmöglichkeit von Säuglingen im häuslichen Milieu. Bei neu zu entwickelnden Geräten müsste vor allem eine Reduktion der Fehlalarme erreicht werden. Unklarheiten herrschen bei der Indikationsstellung und in der Frage welcher Gerätetyp genutzt werden sollte. Patienten und Methoden Per Zufallsprinzip wurde 236 Eltern die kostenlose Nutzung eines handelsüblichen Event-Monitors zur häuslichen Überwachung ihres Säuglings angeboten. In einer Longitudinaluntersuchung wurden über die Dauer eines Jahres Anwendungszeiten und Compliancefragen, für 32 ehemals term-eutrophe Kinder während der ersten 3 Lebensmonate zusätzlich Alarmhäufigkeiten bewertet (Heimmonitorstudie). In einer zweiten Anwenderstudie wurde unter Schlaflaborbedingungen bei einem Monitor mit integrierter neu entwickelter Software an 59 Säuglingen überprüft, ob mittels logistischer Signalverknüpfung eine Reduktion von Fehlalarmen zu realisieren ist (Fehlalarmstudie). Ergebnisse Der Anteil falscher Alarme ist bei handelsüblichen Überwachungsgeräten im Säuglingsalter hoch. Die Akzeptanz und somit die Nutzung dieser Geräte wird dadurch negativ beeinflusst. Mit dem neuen Gerätetyp gelingt eine deutliche Reduktion der Fehlalarme. Schlussfolgerungen Die Weiterverfolgung der Signalverknüpfungsmethoden dürfte die Qualität von Überwachungsmonitoren verbessern. Bei der retrospektiven Analyse aufgezeichneter Alarme kann die Aussagekraft optimiert werden, wenn dank moderner Speicherkapazitäten der Ersatz einer Ereignisspeicherung (Eventrecordings) durch eine Daueraufzeichnung möglich wird.

    ABSTRACT: The infant car seat challenge (ICSC), or period of observation in a car safety seat before discharge to monitor for episodes of apnea, bradycardia and desaturation, is one of the most common tests performed on preterm neonates in the United States. However, the utility of the ICSC to identify infants at risk for adverse cardiopulmonary events in the car seat remains unclear. Minimal evidence exists to guide clinicians in performance of this test including appropriate inclusion criteria and failure criteria. In this article, the origins of the ICSC are discussed as well as potential etiologies of desaturations and bradycardia in the car seat position. Current literature on implementation, inclusion and failure criteria, incidence of failure and data on the meaning of a 'passed' vs 'failed' ICSC are discussed.     ABSTRACT: IMPORTANCE Preterm infants have immature respiratory control and resulting intermittent hypoxia (IH). The extent of IH after stopping routine caffeine treatment and the potential for reducing IH with extended caffeine treatment are unknown. OBJECTIVES To determine (1) the frequency of IH in premature infants after discontinuation of routine caffeine treatment and (2) whether extending caffeine treatment to 40 weeks' postmenstrual age (PMA) reduces IH. DESIGN, SETTING, AND PARTICIPANTS A prospective randomized clinical study was conducted at 16 neonatal intensive care units in the United States, with an 18-month enrollment period. Preterm infants (<32 weeks' gestation) previously treated with caffeine were randomized to extended caffeine treatment or usual care (controls) at a PMA of at least 34 weeks but less than 37 weeks. Continuous pulse oximeter recordings were obtained through 40 weeks' PMA. Oximeter data were analyzed by persons masked to patient group. INTERVENTION Continued treatment with caffeine. MAIN OUTCOMES AND MEASURES Number of IH events and seconds with less than 90% hemoglobin oxygen saturation (Sao2) per hour of recording. RESULTS Our analysis included 95 preterm infants. In control infants, the mean (SD) time at less than 90% Sao2 at 35 and 36 weeks' PMA was 106.3 (89.0) and 100.1 (114.6) s/h, respectively. The number of IH events decreased significantly from 35 to 39 weeks' PMA (P = .01). Extended caffeine treatment reduced the mean time at less than 90% Sao2 by 47% (95% CI, -65% to -20%) to 50.9 (48.1) s/h at 35 weeks and by 45% (95% CI, -74% to -17%) to 49.5 (52.1) s/h at 36 weeks. CONCLUSIONS AND RELEVANCE Substantial IH persists after discontinuation of routine caffeine treatment and progressively decreases with increasing PMA. Extended caffeine treatment decreases IH in premature infants. TRIAL REGISTRATION Identifier: NCT01875159.
