Nutritional rickets among children in the United States: review of cases reported between 1986 and 2003.

ABSTRACT Reports of hypovitaminosis D among adults in the United States have drawn attention to the vitamin D status of children. National data on hypovitaminosis D among children are not yet available. Reports from 2000 and 2001 of rickets among children living in North Carolina, Texas, Georgia, and the mid-Atlantic region, however, confirmed the presence of vitamin D deficiency among some US children and prompted new clinical guidelines to prevent its occurrence. We reviewed reports of nutritional rickets among US children <18 y of age that were published between 1986 and 2003. We identified 166 cases of rickets in 22 published studies. Patients were 4-54 mo of age, although in 17 studies the maximal age was <30 mo. Approximately 83% of children with rickets were described as African American or black, and 96% were breast-fed. Among children who were breast-fed, only 5% of records indicated vitamin D supplementation during breast-feeding. The American Academy of Pediatrics (AAP) recently recommended a minimal intake of 200 IU/d vitamin D for all infants, beginning in the first 2 mo of life. AAP recommends a vitamin D supplement for breast-fed infants who do not consume at least 500 mL of a vitamin D-fortified beverage. Given our finding of a disproportionate number of rickets cases among young, breast-fed, black children, we recommend that education regarding AAP guidelines emphasize the higher risk of rickets among these children. Education should also emphasize the importance of weaning children to a diet adequate in both vitamin D and calcium.

Na pri-­ meira metade do século XX observou-­se uma redução dramática da sua prevalência nos países desenvolvidos. No entanto, nos últimos anos, têm aumentado os relatos desta doença associados a vários factores incluindo dietas vegetarianas. Este artigo descreve o caso de uma criança de 17 meses que desenvolveu raquitismo carencial após um período prolongado de dieta vegetariana estrita. Os regimes vegetarianos podem proporcionar aportes nutricionais apropriados ao crescimento e desenvolvimento de crianças e adolescentes mas devem ser adequadamente planeados, orientados e monitorizados.     ABSTRACT: While some representatives of the animal kingdom were improving their biological mechanisms and properties for adapting to ever-changing life conditions, the genus Homo was developing backward: human individuals were losing their adaptation to life areas conquered earlier. Losing step-by-step their useful traits including the body hair cover, the primitive genus Homo retained his viability only under very favorable conditions of the equatorial Africa. Protection from UV radiation danger was provided only by pigmentation of skin, hair, and eyes. However, "impoverished" individuals of this genus gained the ability to walk upright. Their hands became free from participation in movement and became fine tools for producing useful instruments, from the stone knife to the computer. The major consequence of upright movement and hand development became the powerful development of the brain. A modern human, Homo sapiens, appeared capable of conquering very diverse new habitats. The human's expansion on the Earth occurred somewhat limited by his dependence on vitamin D. His expansion into new areas with lower Sun activity was partially associated with the loss of skin pigmentation. But there is an open question, whether under these new conditions he is satisfactorily provided with vitamin D. This paper discusses the following problems: how can we ensure a sufficient intake of vitamin D, how much does an individual require for his existence and optimal life, what will be consequences of vitamin D deficiency, and what are the prospects for better provision with vitamin D?

    ABSTRACT: Nutritional rickets is a disease that can be prevented through the inges- tion of vitamin D and calcium and by adequate sunlight exposure. In the ������������������������������������������������������������������������������������������������������������������������������������������������first half of the XX century a dramatic reduction in the prevalence of nutritional rickets occurred in developed countries. However, the past years witnessed an increase in the number of reported cases of this disease that were associated with several factors including vegetarian diets. This paper reports the case of a 17 months old child who develo- ped rickets after a long period of strict vegetarian diet. Vegetarian regimens can provide appropriate nutritional intake for the growth and development of children and adolescents but must be care- fully planned, guided and monitored by health professionals together with families. When these aspects are not taken into account, nutri- tional defects can appear.
