Fibroepithelial Polyp of the Lower Urinary Tract in Adults

Fibroepithelial polyps of the urinary tract are rare with most cases reported in children.
We report the clinicopathologic features of 12 fibroepithelial polyps of the lower urinary tract in adults.
There were 9 males, 2 females, and 1 patient where the gender was unknown (median age, 44 years; range, 17-70 years).
Chief clinical symptoms were hematuria, urinary urgency, and hesitancy. Five patients were asymptomatic, where the lesions were discovered incidentally. Most lesions were located near the verumontanum or the bladder neck. Ten patients were treated by transurethral resection. Of the 10 patients with follow-up information, none showed recurrence (mean, 20 months; median, 17 months). Histologically, all of the fibroepithelial polyps were lined by normal-appearing urothelium, with in one lesion the additional finding of a columnar epithelial lining. There were three overall architectural patterns seen within fibroepithelial polyps. The most common pattern (Pattern 1) seen in 5 cases consisted of a polypoid mass with club-like projections resembling a cloverleaf with florid cystitis cystica et glandularis of the nonintestinal type in the stalk. The second pattern (Pattern 2) seen in 4 cases consisted of a papillary tumor composed of numerous small, rounded fibrovascular cores containing dense fibrous tissue. The last morphologic pattern (Pattern 3) consisted of a polypoid lesion with secondary tall finger-like projections, which was seen in 3 cases. All lesions lacked prominent edema and inflammation seen in polypoid cystitis. Fibroepithelial polyps contained broader stalks with dense fibrous tissue, in contrast to the thin delicate loose fibroconnective tissue seen in the stalk of papillomas. One lesion contained atypical degenerative-appearing stromal cells.
Although fibroepithelial polyps have been considered to be congenital, we think that some of these polyps could develop after birth because all of our patients first showed clinical symptoms in adulthood. Because fibroepithelial polyps in adults are rare, some of these cases can be misdiagnosed as urothelial neoplasms or reactive conditions. Recognition of the precise histologic features of fibroepithelial polyp can facilitate its correct diagnosis.

    ABSTRACT: A high-resolution 200 MHz frequency GPR profile with 0.1 m station spacing was collected using a Pulse EKKO 100 system, adjacent to a trench site that had been previously excavated and interpreted. The profile intersected the fault scarp on one of the alluvial fans on the northern slope of the Piano di Pezza; this fault scarp was thought to be associated with repeated earthquake activity along the Ovindoli-Pezza fault. The GPR profile was conducted to evaluate the effectiveness of the technique in radar mapping and in extending the trenching information laterally and in depth in this environment. The work was carried out as part of a wider research programme of GPR profiling along the Ovindoli-Pezza fault system. The GPR data were processed using Win EKKO radar data processing software and time-depth conversion was facilitated using field-derived CMP velocity analysis. The results provided a detailed image of the GPR data, which shows a generally high spatial correlation with the mapped fault and 14C-dated lithologic/ pedologic units. Furthermore, the GPR data has provided a framework for estimating the extent of extensional and episodic catastrophic activity along the fault, and has elucidated the geometric relationship between the faulting in response to recent tectonic events.

    ABSTRACT: The spectrum of neoplasms involving the urinary bladder is diverse, and, at times, different entities with distinct prognostic and managerial implications may have significant morphologic overlap. The presence of a neoplasm with an unusual morphologic appearance, such as an undifferentiated spindled, or plasmacytoid pattern may necessitate immunohistochemistry to establish the diagnosis. In this review, we discuss a series of distinct diagnostic scenarios, including high-grade undifferentiated carcinoma versus prostatic adenocarcinoma, enteric-type adenocarcinoma versus secondary colorectal adenocarcinoma, spindle cell proliferations, neoplasms with plasmacytoid morphology, endophytic tumors with a nested growth pattern, and flat urothelial lesions with atypia. We also discuss markers supporting urothelial differentiation in the context of a metastatic carcinoma from an unknown primary. The importance of using a morphologically derived differential diagnosis to guide the selection and interpretation of immunohistochemical studies is emphasized, and the varying utility (specificity) of the individual immunohistochemical markers within each setting is addressed.
