Effects of a weight maintenance program with or without exercise on the metabolic syndrome: a randomized trial in obese men.

ABSTRACT Weight maintenance (WM) after weight reduction (WR) is difficult, but increased physical activity may help. We studied whether adding exercise to diet counseling decreases the occurrence of the metabolic syndrome (MBO).
Ninety voluntary middle-aged men with a BMI range of 30-40 and a waist girth >100 cm were recruited to the research institute's clinic in 1997. After a very-low-energy diet for 2 months (WR), the men were randomized into a walking, resistance training or control group for 6 months (WM). All groups received similar dietary advice. After WM, there was a 23-month follow-up. Diagnosis of MBO was based on >or= 3 components.
After WR, the mean weight loss was 14.2 kg. At the end, the weight decrease was 4.8 kg (n = 68) with no statistically significant difference between the groups. All groups had improved some components (insulin, HDL cholesterol, body composition) of MBO. When the groups were combined, the odds ratio for the occurrence of MBO (vs. baseline) was 0.10 after WR, 0.08 after WM and 0.29 at the end.
Adding structured exercise to diet counseling did not alleviate MBO better than diet only. However, the occurrence of MBO was reduced in all groups at the end.

    ABSTRACT: Objectifs: Évaluer si un programme d’entraînement en résistance d’une durée d’un an prévient le regain de poids et majore l’amélioration du profil inflammatoire. Le second objectif de cette étude était de déterminer si la variation du tissu adipeux viscéral est associée aux changements dans les concentrations sanguines des marqueurs inflammatoires. Méthodes: Soixante-dix femmes post-ménopausées en surpoids ou obèses ont été randomisées dans un des deux groupes suivants : (1) Contrôle ou (2) Entraînement en résistance. La composition corporelle (absorptiométrie double à rayons X et tomographie axiale) et les marqueurs inflammatoires (protéine C-réactive, orosomucoïde, haptoglobine) ont été évalués avant et après la période de suivi d’une durée d’un an. Résultats: Suite à la période de suivi, un regain significatif de poids corporel et de masse grasse était observé dans le groupe contrôle et le groupe entraînement en résistance (p < 0,05). Une réduction des concentrations sériques de l’orosomucoïde et une hausse des niveaux sériques de l’haptoglobine étaient également notées dans les deux groupes (p < 0,05). La variation du tissu adipeux viscéral était seulement associée aux changements dans les concentrations sériques de la protéine C-réactive (r = 0,373, p < 0,05). Conclusion: Nos résultats suggèrent que l’entraînement en résistance ne prévient pas le regain de poids corporel et ne majore pas l’amélioration du profil inflammatoire chez des femmes post-ménopausées en surpoids ou obèses. De plus, nos résultats indiquent que la variation du tissu adipeux viscéral ne semble pas être un facteur clé impliqué dans les changements des concentrations sanguines des marqueurs inflammatoires. Objectives: To evaluate if a 1-year resistance training program improved weight loss maintenance and the inflammatory profile in overweight and obese postmenopausal women. The second objective of this study was to determine if the variation in visceral adipose tissue was associated with the changes in inflammatory markers concentrations. Methods: Seventy overweight and obese postmenopausal women were randomized to a control group or a resistance training group. Body composition (dual energy X-ray absorptiometry and computed tomography) and inflammatory markers (C-reactive protein, orosomucoid, haptoglobin) were measured before and after the 1-year weight loss maintenance intervention. Results: Following the weight loss maintenance intervention, we observed a significant body weight and fat mass regain in the control group and the resistance training group (p < 0.05). Significant reductions in orosomucoid and increases in haptoglobin concentrations were noted in both groups (p < 0.05). The variation in visceral adipose tissue was only associated with the changes in C-reactive protein concentrations (r = 0,373, p < 0.05). Conclusion: Our results suggest that resistance training do not improve weight loss maintenance and the inflammatory profile in overweight and obese postmenopausal women. Moreover, our results indicate that the variation in visceral adipose tissue may not be a key factor implicated in the changes in inflammatory markers concentrations.

    ABSTRACT: Introduction: Bariatric surgery patients with pre existing co morbid conditions such as diabetes and hypertension are at increased cardiovascular risk. The relationship of exercise in promoting a more healthy body composition with bariatric surgery is not clearly defined. Subjects: Bariatric surgery patients (n=223) at Tufts Medical Center in Boston, MA. were followed for changes in absolute and relative body weight over two years. Patients were stratified by surgical procedure, prevalence of baseline diabetes or hypertension, and participation in an exercise program or no exercise program at three semi-annual visits. Results: Surgical groups were significantly different in absolute and relative body weight at baseline and over the two year follow up. There were no significant differences in body weight at baseline between the co morbidity groups or over the two year follow up. The exercise group was significantly different from non exercise group at the last follow up (18-24 months) for absolute body weight. Only seven participants qualified for fitness assessments of which these patients increased in estimated MET capacity without any significant relationship to change in body weight over the follow up period. Conclusions: While surgical groups displayed significant differences in body weight across the two years there were no significant differences between the baseline co morbidity groups and differences only at 18-24 months between the exercise and non exercise groups. Participation in exercise programming possibly increased lean body mass for a corresponding increase in body weight possibly responsible for the increase in estimated MET capacity.

    ABSTRACT: Tavoitteena oli tutkia 40-vuotiaiden miesten terveyskäyttäytymistä, terveysuskomuksia ja miesten saamaa terveysneuvontaa Helsingissä. 273 miestä vastasi kyselyyn ja osallistui terveystutkimuksiin. Terveydentilan perusteella miehet arvioitiin matalan (n=145) ja korkean (n=128) riskin ryhmiin. Khin neliö-testillä tutkittiin elämäntapa- ja riskitekijöitä koetun terveyden (hyvä, keskinkertainen/huono) luokissa ja verrattiin matalan ja korkean riskin ryhmiä em. tekijöiden osalta. Askeltavalla logistisella regressiomallilla analysoitiin tulosmuuttujia taustatekijöiden, terveyskäyttäytymisen, terveysuskomusten ja kliinisten riskitekijöiden avulla sekä arvioitiin oireiden ja vaivojen suhdetta koettuun terveydentilaan. Korkeassa riskissä olevien terveyttä seurattiin vuosina 2001–2004 analysoimalla mini-intervention vaikutusta terveysriskeihin ja elintapoihin varianssianalyysin avulla (ANOVA) (n=46). Matalasta vastausprosentista johtuen (39.6%), ei-vastanneiden aineistoa kerättiin käyttämällä syvähaastattelua (n=28) sekä puhelinkyselyä (n=40). Lopullinen aineisto koostui 341 miehestä. Tulokset osoittivat, että miehillä oli sydän- ja verisuonitautiriskejä. Kaksi kolmesta osallistuneista oli ylipainoisia tai lihavia, yli kolmanneksella vyötärönympärys oli ≥100 cm, ja yli 40%:llä oli diastolinen verenpaine ≥90 mmHg. Yli puolet tupakoi päivittäin ja 40% käytti alkoholia runsaasti. Ristiriitaisuutta ilmensi se, että huolimatta riskitekijöistä noin puolet miehistä koki terveydentilansa hyväksi. Sairauden tai vamman puute, hyvä suun terveydentila ja normaali vyötärönympärys olivat yhteydessä hyväksi koettuun terveydentilaan. Suora yhteys voitiin havaita omaisten tarjoaman neuvonnan ja vähäisen alkoholin käytön välillä. Masennus ja unettomuus olivat voimakkaasti yhteydessä loppuun palamiseen. Miehillä oli erilaisia fyysisiä ja psyykkisiä oireita, jotka korreloivat voimakkaasti masennuksen kanssa. Pieni määrä miehistä koki saaneensa terveysneuvontaa hoitohenkilökunnalta verrattuna perheenjäseniltä saatuun ohjaukseen. Korkeariskisten miesten (n=46) arvot parantuivat merkitsevästi lyhyellä aikavälillä. Kolesteroliarvoja lukuunottamatta ne palautuivat kolmen vuoden kuluttua alkumittausarvoja kohti. Laadullinen tutkimus osoitti, että “ei-vastanneet“ eivät osallistuneet projektiin, sillä he olivat oireettomia tai kiireisiä. Heillä todettiin samoja terveysriskejä kuin projektiin osallistuneilla. Syvähaastattelussa miehet toivat esille kokemuksiaan huolista, vihan tunteista, peloista ja yksinäisyydestä. Hoidonantajien on tärkeää ymmärtää ristiriidat miesten subjektiivisen ja objektiivisen terveydentilan välillä, mikä auttaa havaitsemaan esteitä terveyskäyttäytymiselle. Yhä enemmän tarvitaan yhteistyötä yksityisen ja julkisen terveydenhuollon välillä varmistamaan terveystottumusten jatkuminen miesten keskuudessa. The aim of the study was to examine health behaviour, health beliefs and need for health counselling amongst 40-year-old men in Helsinki. 273 males answered the questionnaire and participated in health examinations. Based on the results of health situation, males were assessed to be at low (n=145) or high risk (n=128). With Chi-square tests were assembled to identify differences between health indicators and the groups of self-rated health (good, average/poor) and between the groups of low- and high-risk men. A step-wise logistic regression model was used to estimate the adjusted odds of each outcome by socio-demographic-, health behaviour-, health belief- and clinical factors. Males at high risks were followed during 2001-2004 to analyze the impact of brief-intervention to health risks and life styles by variance analyze (ANOVA) (n=46). Due to a low response rate (39.6%), more data from non-respondents were collected by in-depth interview (n=28) and telephone survey (n=40). The final data set comprised 341 males. The findings of the study indicated that males had risk factors for cardiovascular diseases. Two-thirds were overweight or obese, over one-third had a waist circumference ≥100 cm, and over 40% had diastolic blood pressure ≥90 mm Hg. Over one-half smoked daily and 40 % of the males used alcohol excessively. Despite risk factors males rated their health as good, which showed a great contradiction. Lack of illness or injury, good dental health, and normal waist circumference had the strongest associations with perceived good health. A positive association was found between alcohol advice by family members and lower use of alcohol. Depression and insomnia were significant predictors for burnout. Only a small percentage of males had received counselling from professionals, they were advised by family members. In the follow-up study (n=46) the risk factors improved significantly from baseline during a short time period. However, the final measurements in 2004 showed that the values, except cholesterol readings, tended to revert back to baseline. Qualitative research showed that “non-respondents“did not participate to the metabolic syndrome project, because they were ‘symptomless’ or busy. However they had same risk factors as participated males had. In-depth interview revealed sensitive information that included major causes of concern, anxiety, fears and loneliness. The results underscore the importance of understanding by health professionals that there exists a contradiction between objective and subjective health, which might help to perceive the attitudes towards health behaviour. Greater collaboration is needed between private health care agencies and official primary health care to ensure continuity in the improvement of health habits among males.
