Article: Cushing's legacy to transsphenoidal surgery.

ABSTRACT: Cushing's experience with the surgical treatment of pituitary adenomas and other lesions producing the chiasmal syndrome is reviewed. The conclusion is that his ultimate rejection of the transsphenoidal route was not due to his dissatisfaction with this procedure, but rather came about because of the transfrontal route provided him with access to the pituitary and, at the same time, enabled him to verify all suprasellar tumors if the preoperative diagnosis was uncertain. Until he gave it up in 1929, Cushing used the transsphenoidal route preferentially when the sella was large. Some of his assistants in the clinic, notably Norman Dott, came away with a high and lasting regard for the operation, and Dott's subsequent influence on Gérard Guiot is a crucial link in the return of transsphenoidal surgery to its current preeminent position.Journal of Neurosurgery 05/1981; 54(4):448-54. · 2.96 Impact Factor -
Article: Sir Victor Horsley (1857-1916): pioneer of neurological surgery.

ABSTRACT: Immortalized in surgical history for the introduction of "antiseptic wax," Sir Victor Horsley played a pivotal role in shaping the face of standard neurosurgical practice. His contributions include the first laminectomy for spinal neoplasm, the first carotid ligation for cerebral aneurysm, the curved skin flap, the transcranial approach to the pituitary gland, intradural division of the trigeminal nerve root for trigeminal neuralgia, and surface marking of the cerebral cortex. A tireless scientist, he was a significant player in discovering the cure for myxedema, the eradication of rabies from England, and the invention of the Horsley-Clarke stereotactic frame. As a pathologist, Horsley performed research on bacteria and edema and founded the Journal of Pathology. Horsley's kindness, humility, and generous spirit endeared him to patients, colleagues, and students. Born to privilege, he was nonetheless dedicated to improving the lot of the common man and directed his efforts toward the suffrage of women, medical reform, and free health care for the working class. Knighted in 1902 for his many contributions to medicine, Sir Victor met an untimely death during World War I from heat stroke at the age of 59. An iconoclast of keen intellect, unlimited energy, and consummate skill, his life and work justify his epitaph as a "pioneer of neurological surgery."Neurosurgery 04/2002; 50(3):607-11; discussion 611-2. · 2.79 Impact Factor -
Article: Key personalities in the development and popularization of the transsphenoidal approach to pituitary tumors: an historical overview.

ABSTRACT: Although the transsphenoidal procedure, as we know it today, was devised at the beginning of the last century, its acceptance and eventual widespread affirmation have been a slow but relentless process. Like many technologic and scientific advances, several individuals contributed to the perfection of an idea whose time had come. Yet, it is unequivocally accepted that a few key figures played a critical role in the development, preservation, and eventual widespread popularization of transsphenoidal surgery. These pioneers were original thinkers and bold (albeit careful) surgeons.
