Suicide rates in relation to health care access in the United States: an ecological study.

ABSTRACT We tested the hypothesis that suicide rates in the United States are associated with indicators of access to health care services.
With an ecological study design, we compared age-adjusted suicide rates for men and women with demographic, socioeconomic, and other indices of access to health care, by state (N = 51, including the District of Columbia). The most recently available information from the National Statistics Reports at the U.S. Census Bureau, the U.S. Centers for Disease Control and Prevention National Center for Health Statistics, and the American Board of Medical Specialties was used. Data on suicide are from 2001; other measures were matched for the closest available year, except that state-based data on psychiatrists and physicians are from 2004.
Positive bivariate associations with state suicide rates (all p < or = .005) are ranked as follows: male sex, Native American ethnicity, and higher proportion of uninsured residents. Negative bivariate associations (all p < or = .002) are ranked as follows: higher population density, higher annual per capita income, higher population density of psychiatrists, higher population density of physicians, higher federal aid for mental health, and higher proportion of African Americans. All factors were associated with state suicide rates in expected directions. In multivariate models of associations between suicide rates and indices of access to health care, the state rate of federal aid for mental health was the strongest indicator, followed by the rate of uninsured persons and population density of psychiatrists and physicians and by population density.
Such aggregate analyses cannot specify risk indices for individual persons. Nevertheless, the methods employed detected several factors with well-established associations with suicide. They also yielded strong correlations of state-based suicide rates with proposed indicators of access to health care. The findings support the view that clinical intervention is a crucial element in the prevention of suicide.

    ABSTRACT: Objective: The risk factors associated with suicidal behaviour in bipolar patients still are not clear. We aimed to evaluate the frequency of suicide attempt, type of suicide attempt and significant clinical characteristics associated with suicide attempts. Methods: One hundred twenty two consecutive patients, from Bipolar Disorder Unit of Çukurova University, Faculty of Medicine, Department of Psychiatry, are included in this study. “Affective Disorders Patient Registry Form” was developed by Bipolar Disorder Unit and SCID-I were used in this study to collect the data. These forms are filled in through interviews with patients and their relatives, and evaluation of inpatient files. Results: The prevalence of suicide attempt was 19.7% in the outpatient group. Lifetime history of suicidal behavior was significantly associated with following characteristics: being a woman, depression as a first episode and indicators of severity of bipolar disorder including duration of illness, duration of untreated illness (latency), number of hospitalization, number of total mood episodes, number of depressive episodes, number of mixed episodes, positive familial psychiatric disorder history. Conclusion: Our study showed that patients with bipolar disorder have a high rate of suicide attempt. Reduction of the duration of untreated illness, prevention of mood episodes, assessment of suicide risk during depressive and mixed episodes should be the main targets of the treatment. Detection of warning signs about suicide among patients with bipolar disorder may help to distinguish the risky group and prevent suicide.

    ABSTRACT: In the last century Hungary had astonishingly high suicide rates characterized by marked regional within-country inequalities, a spatial pattern which has been quite stable over time. To explain the above phenomenon at the level of micro-regions (n=175) in the period between 2005 and 2011. Our dependent variable was the age and gender standardized mortality ratio (SMR) for suicide while explanatory variables were factors which are supposed to influence suicide risk, such as measures of religious and political integration, travel time accessibility of psychiatric services, alcohol consumption, unemployment and disability pensionery. When applying the ordinary least squared regression model, the residuals were found to be spatially autocorrelated, which indicates the violation of the assumption on the independence of error terms and - accordingly - the necessity of application of a spatial autoregressive (SAR) model to handle this problem. According to our calculations the SARlag model was a better way (versus the SARerr model) of addressing the problem of spatial autocorrelation, furthermore its substantive meaning is more convenient. SMR was significantly associated with the "political integration" variable in a negative and with "lack of religious integration" and "disability pensionery" variables in a positive manner. Associations were not significant for the remaining explanatory variables. Several important psychiatric variables were not available at the level of micro-regions. We conducted our analysis on aggregate data. Our results may draw attention to the relevance and abiding validity of the classic Durkheimian suicide risk factors - such as lack of social integration - apropos of the spatial pattern of Hungarian suicides.

    ABSTRACT: Objectives: Suicide is a major global health problem imposing a considerable burden on populations in terms of disability-adjusted life years. There has been an increasing trend in fatal and attempted suicide in Iran over the past few decades. The aim of the current study was to assess overall, gender and social inequalities across Iran’s provinces during 2006–2010. Design: Ecological study. Setting: The data on distribution of population at the provinces were obtained from the Statistical Centre of Iran. The data on the annual number of deaths caused by suicide in each province were gathered from the Iranian Forensic Medicine Organization. Methods: Suicide mortality rate per 100 000 population was calculated. Human Development Index was used as the provinces’ social rank. Gini coefficient, rate ratio and Kunst and Mackenbach relative index of inequality were used to assess overall, gender and social inequalities, respectively. Annual percentage change was calculated using Joinpoint regression. Results: Suicide mortality has slightly increased in Iran during 2006–2010. There was a substantial and constant overall inequality across the country over the study period. Male-to-female rate ratio was 2.34 (95% CI 1.45 to 3.79) over the same period. There were social inequalities in suicide mortality in favour of people in better-off provinces. In addition, there was an increasing trend in these social disparities over time, although it was not statistically significant. Conclusions: We found substantial overall, gender and social disparities in the distribution of suicide mortality across the provinces in Iran. The findings showed that men in the provinces with low socioeconomic status are at higher risk of suicide mortality. Further analyses are needed to explain these disparities.
