Phialemonium curvatum Prosthetic Valve Endocarditis with an Unusual Echocardiographic Presentation

Phialemonium species, an opportunistic fungal pathogen rarely causes invasive disease, have been described as opportunistic infection agents in humans, mainly as a result of immunosuppression and very rarely involves the heart. We present a case of a patient with Phialemonium curvatum prosthetic aortic valve endocarditis with an unusual initial transesophageal echocardiography (TEE) presentation, illustrating the important role of repeat TEE for the proper diagnosis and management of infective endocarditis.

ABSTRACT: Establishing the diagnosis of fungal endocarditis caused by zygomyces during life occurs rarely. We report on a child in whom zygomyces mitral valve endocarditis has developed. Floating thrombus attached to the mitral papillary muscles protruded in systole from the left ventricle into the aorta. Despite adequate medical and surgical treatment, this lesion recurred postoperatively with development of severe mitral obstruction and subsequent death.Infectious Disease in Clinical Practice 07/2009; 17(4):268-269. ABSTRACT: Melanized or dematiaceous fungi are associated with a wide variety of infectious syndromes. Many are soil organisms and are generally distributed worldwide, though certain species appear to have restricted geographic ranges. Though they are uncommon causes of disease, melanized fungi have been increasingly recognized as important pathogens, with most reports occurring in the past 20 years. The spectrum of diseases with which they are associated has also broadened and includes allergic disease, superficial and deep local infections, pneumonia, brain abscess, and disseminated infection. For some infections in immunocompetent individuals, such as allergic fungal sinusitis and brain abscess, they are among the most common etiologic fungi. Melanin is a likely virulence factor for these fungi. Diagnosis relies on careful microscopic and pathological examination, as well as clinical assessment of the patient, as these fungi are often considered contaminants. Therapy varies depending upon the clinical syndrome. Local infection may be cured with excision alone, while systemic disease is often refractory to therapy. Triazoles such as voriconazole, posaconazole, and itraconazole have the most consistent in vitro activity. Further studies are needed to better understand the pathogenesis and optimal treatment of these uncommon infections.Clinical microbiology reviews 10/2010; 23(4):884-928. ABSTRACT: Phialemoniopsis species are ubiquitous dematiaceous molds associated with a wide variety of superficial and systemic infections in human. In this study, we isolated a mold from the forearm nodule biopsy specimen from a patient with underlying liver cirrhosis, ankylosing spondylosis, and tuberculosis. He was treated with itraconazole, but unfortunately, he succumbed as a result of disseminated tuberculosis with multiorgan failure. The histology results of the skin biopsy showed necrotizing granulomas in which numerous fungal elements were found. On Sabouraud dextrose agar, the fungal isolate grew as white-to-cream and smooth-to-velvety colonies. Microscopically, oval-to-cylindrical conidia were observed from abundant adelophialides, which possessed barely visible parallel collarettes but no basal septa. The azole drugs voriconazole, itraconazole, and posaconazole, as well as amphotericin B, showed high activities against this fungus. Internal transcribed spacer, 28S nuclear ribosomal DNA (nrDNA), and β-actin and β-tubulin gene sequencing showed that this fungus is most closely related to but distinct from Phialemonium curvata. Matrix-assisted laser desorption ionization–time of flight mass spectrometry (MALDI-TOF MS) and hierarchical cluster analysis showed that the MALDI-TOF MS spectrum of this fungus is most closely related to that of Phialemonium pluriloculosa.
