Differential usefulness of various markers in the flow cytometric detection of paroxysmal nocturnal hemoglobinuria in blood and bone marrow.

ABSTRACT In this study, we examined the usefulness of various markers on blood cell populations in the diagnosis of paroxysmal nocturnal hemoglobinuria (PNH). We also evaluated bone marrow specimens, which generally are considered less suitable than blood owing to variable expression of glycosyl phosphatidylinositol (GPI)-linked antigens during hematopoietic cell differentiation. All 15 patients in our cohort had subpopulations of CD16/CD55-deficient granulocytes and CD14/CD55-deficient monocytes ("PNH clones"). The PNH clone size of granulocytes and monocytes was greater than that of erythrocytes or lymphocytes in the majority of the cases. It is interesting that CD59 showed limited usefulness for detecting PNH+ monocytes. Normal monocytes exhibited significantly dimmer CD59 expression than normal granulocytes. PNH-deficient monocytes expressed only marginally lower CD59, making it a less robust marker for highlighting PNH+ monocytes compared with CD14 or CD55. Finally, our study demonstrated the definitive usefulness of a limited combination of markers (CD16/CD55/CD45/CD14) in detecting GPI-deficient monocytes and granulocytes in bone marrow specimens submitted for flow cytometric evaluation of cytopenias.

    ABSTRACT: Paroxysmal Nocturnal Hemoglobinuria (PNH) is a rare hematological disorder that is often suspected in a patient presenting with non-immune hemolytic anemia associated with pancytopenia or venous thrombosis. This disorder is a consequence of acquired somatic mutations in the Phosphatidylinositol Glycan class A (PIG-A) gene in the hematopoietic stem cells (HSC) of patients. The presence of these mutations leads to production of blood cells with decreased GPI-anchored cell surface proteins, making red blood cells (RBC) derived from the clone more sensitive to complement mediated hemolysis. The diagnosis of PNH may be difficult in some cases due a low proportion of PNH cells in the blood and occasionally due to difficulties in selecting the most appropriate diagnostic studies. The latest generation of tests allow for detection of very small populations of PNH cells, for following the natural course and response to therapy of the disease and for helping to decide when to initiate therapy with monoclonal antibody targeting the terminal complement protein C5 (Eculizumab), anticoagulation, and in some cases allogeneic HSC transplant. In this paper we review the different diagnostic tests available to clinicians for PNH diagnosis.

    ABSTRACT: Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired clonal hematopoietic disorder characterized by the existence of somatic mutations in the PIG-A (phosphatidylinositolglycan complementation class A) gene, which encodes for a protein involved in the biosynthesis of the glycosyl phosphatidylinositol (GPI) molecule that serves as an anchor for many cell surface proteins. This genetic alteration translates into a total or partial deficiency in the PNH clone of surface proteins attached to the cell by a GPI anchor. Evaluation of deficient expression of GPI-associated proteins is currently used for the diagnosis of paroxysmal nocturnal hemoglobinuria. Among other proteins, deficiency of CD55 and CD59 leads to an increased susceptibility of PNH cells to complement-mediated cell lysis. Variable degrees of cytopenia, an increased susceptibility to infections and recurrent thrombotic events are other symptoms of the disease. In this paper we review the recent advances in the knowledge about the pathogenic mechanisms of the disease and the current approaches for the diagnosis and monitoring of PNH patients.
