Nonverbal behavior of human addicts: multimetric analysis.

ABSTRACT Ethological approach followed by multimetric statistical analysis was applied to characterize and discriminate alcohol, heroin and dual, alcohol and heroin, dependent subjects.
Heroin, alcohol, and dual dependent patients (n=51) after one month of stabilization of remission and control volunteers (n=34) without a history of significant drug or alcohol use were interviewed and videotaped during the interview by approbation. Nonverbal behavioral cues monitored during the interview were analyzed by means of general linear procedure followed by correlation, factor and discriminant function analyses.
By using this approach the attempt to discriminate addicted groups between each other failed. Therefore we found acceptable to combine subjects in one group and to suggest the similarity between alcohol and heroin dependence. It was found that principal markers of behavioral structure in addicted subjects were higher responsivity to communicate distance, less expression of affiliation behavioral pattern, low level of correlations between different behavioral patterns, and unclear factor structure. Behavioral pattern "affiliation" was identified as discriminate behavior between control and addicted subjects.
Nonverbal cues of human behavior identified clear differences between healthy control and addictive subjects. Therefore, ethological approach described in this paper could be recommended for future use in clinical practice.

    ABSTRACT: A prospective study was undertaken over a 2 1/2 year period (July 1, 1985-December 31, 1987) to determine the value of a prospective anatomical severity score ["Estimated" Injury Severity Score (EISS)], in combination with age, for early risk assessment. Five hundred ninety-two blunt trauma patients were used to develop a predictive model of mortality through a logistic regression. The model was validated using 216 patients; a high degree of agreement (Kappa 0.73) was found. When isolated head injuries were removed from the model and validation groups, the Kappa increased to 0.80. Prospective anatomical scoring in combination with age may help to identify high-risk trauma patients early during their care.

    ABSTRACT: To identify which improvements in cognitive function are associated with symptom resolution in schizophrenic patients treated with atypical antipsychotics. a prospective open trial with atypical neuroleptics (risperidone, clozapine, quetiapine). Inpatient and outpatient units, Institute of Psychiatry. Thirty-nine patients with schizophrenia according to DSM-IV criteria were included. Clinical and cognitive assessment were done at baseline (T0) and again after six months of treatment (T2). Twenty-five patients completed the trial. New-generation antipsychotics during six months. Patients were considered as responders if their PANSS score decreased at least 20% (n = 15) and non-responders if it did not (n = 10). Outcome measures: a computerized cognitive assessment comprised tests of short-term-memory (digit span), explicit long-term memory (word pair learning), divided attention, selective attention and verbal fluency (orthographic and semantic). Clinical assessment included PANSS and ESRS. A discriminant function analysis was performed to determine which changes in cognitive performance predicted symptomatic response status. Semantic fluency and orthographic fluency were significant predictors. Together they correctly predicted responder status in 88% of cases. Memory was not a significant predictor of symptomatic response. Verbal fluency discriminated the responder from the non-responder group during a pharmacological treatment.

    ABSTRACT: Ethology is relevant to clinical psychiatry for two different reasons. First, ethology may contribute significantly to the development of more accurate and valid methods for measuring the behavior of persons with mental disorders. Second, ethology, as the evolutionary study of behavior, may provide psychiatry with a theoretical framework for integrating a functional perspective into the definition and clinical assessment of mental disorders. This article describes an ethological method for studying the nonverbal behavior of persons with mental disorders during clinical interviews and reviews the results derived from the application of this method in studies of patients who had a diagnosis of schizophrenia or depression. These findings and others that are emerging from current ethological research in psychiatry indicate that the ethological approach is not limited simply to a mere translation into quantitative and objective data of what clinicians already know on the basis of their judgment or the use of rating scales. Rather, it produces new insights on controversial aspects of psychiatric disorders. Although the impact of ethology on clinical psychiatry is still limited, recent developments in the fields of ethological and Darwinian psychiatry can revitalize the interest of clinical psychiatrists for ethology.
