Do professional interpreters improve clinical care for patients with limited English proficiency? A systematic review of the literature.

ABSTRACT To determine if professional medical interpreters have a positive impact on clinical care for limited English proficiency (LEP) patients.
A systematic literature search, limited to the English language, in PubMed and PsycINFO for publications between 1966 and September 2005, and a search of the Cochrane Library.
Any peer-reviewed article which compared at least two language groups, and contained data about professional medical interpreters and addressed communication (errors and comprehension), utilization, clinical outcomes, or satisfaction were included. Of 3,698 references, 28 were found by multiple reviewers to meet inclusion criteria and, of these, 21 assessed professional interpreters separately from ad hoc interpreters. Data were abstracted from each article by two reviewers. Data were collected on the study design, size, comparison groups, analytic technique, interpreter training, and method of determining the participants' need for an interpreter. Each study was evaluated for the effect of interpreter use on four clinical topics that were most likely to either impact or reflect disparities in health and health care.
In all four areas examined, use of professional interpreters is associated with improved clinical care more than is use of ad hoc interpreters, and professional interpreters appear to raise the quality of clinical care for LEP patients to approach or equal that for patients without language barriers.
Published studies report positive benefits of professional interpreters on communication (errors and comprehension), utilization, clinical outcomes and satisfaction with care.

    ABSTRACT: Data from an electronic medical record (EMR) system can provide valuable insight regarding health consequences in the aftermath of a disaster. In January of 2010, the U.S. Department of Health and Human Services (HHS) deployed medical personnel to Haiti in response to a crippling earthquake. An EMR system was used to record patient encounters in real-time and to provide data for decision support during response activities. During the Haiti response, HHS monitored the EMR system by recoding diagnoses into seven broad categories. At the conclusion of the response, it was evident that a new diagnosis categorization process was needed to provide a better description of the patient encounters that were seen in the field. After examining the EMRs, researchers determined nearly half of the medical records were missing diagnosis data. The objective of this study was to develop and test a new method of categorization for patient encounters to provide more detailed data for decision making. A single researcher verified or assigned a new diagnosis for 8,787 EMRs created during the Haiti response. This created a new variable, the Operational Code, which was based on available diagnosis data and chief complaint. Retrospectively, diagnoses recorded in the field and Operational Codes were categorized into eighteen categories based on the ICD-9-CM diagnostic system. Creating an Operational Code variable led to a more robust data set and a clearer depiction emerged of the clinical presentations seen at six HHS clinics set up in the aftermath of Haiti's earthquake. The number of records with an associated ICD-9 code increased 106% from 4,261 to 8,787. The most frequent Operational Code categories during the response were: General Symptoms, Signs, and Ill-Defined Conditions (34.2%), Injury and Poisoning (18.9%), Other (14.7%), Respiratory (4.8%), and Musculoskeletal and Connective Tissue (4.8%). The Operational Code methodology provided more detailed data about patient encounters. This methodology could be used in future deployments to improve situational awareness and decision-making capabilities during emergency response operations.

    ABSTRACT: The ability to work with interpreters is a core skill for UK medical graduates. At the University of Sheffield Medical School, this teaching was identified as a gap in the curriculum. Teaching was developed to use professional interpreters in role-play, based on evidence that professional interpreters improve health outcomes for patients with limited English proficiency. Other principles guiding the development of the teaching were an experiential learning format, integration to the core consultation skills curriculum, and sustainable delivery. The session was aligned with existing consultation skills teaching to retain the small-group experiential format and general practitioner (GP) tutor. Core curricular time was found through conversion of an existing consultation skills session. Language pairs of professional interpreters worked with each small group, with one playing patient and the other playing interpreter. These professional interpreters attended training in the scenarios so that they could learn to act as patient and family interpreter. GP tutors attended training sessions to help them facilitate the session. This enhanced the sustainability of the session by providing a cohort of tutors able to pass on their expertise to new staff through the existing shadowing process. Tutors felt that the involvement of professional interpreters improved student engagement. Student evaluation of the teaching suggests that the learning objectives were achieved. Faculty evaluation by GP tutors suggests that they perceived the teaching to be worthwhile and that the training they received had helped improve their own clinical practice in consulting through interpreters. We offer the following recommendations to others who may be interested in developing teaching on interpreted consultations within their core curriculum: 1) consider recruiting professional interpreters as a teaching resource; 2) align the teaching to existing consultation skills sessions to aid integration; and 3) invest in faculty development for successful and sustainable delivery.
