Diagnostically challenging spindle cell lipomas: a report of 34 "low-fat" and "fat-free" variants.

ABSTRACT Spindle cell lipomas (SCL) classically occur as subcutaneous masses in the upper trunk/neck of older men and are composed of mature fat, CD34-positive spindled cells, ropey collagen, myxoid matrix, and blood vessels. A number of variants have been reported, including SCL with pseudoangiomatous change, composite SCL hibernoma, and composite SCL/pleomorphic lipoma. A review of over 300 consultation cases diagnosed as SCL revealed 34 cases in which fat was noted to be present in <5% of the tumor (n = 30) or absent (n = 4). These cases posed diagnostic difficulties because of the dearth of fat; we propose the terms "low-fat" and "fat-free" SCL for these variants. The tumors presented in older men (mean, 56 years; ratio of males to females, 11:1) and presented as small (mean, 2.0 cm) circumscribed dermal or subcutaneous masses of the head/neck (n = 18), back (n = 7), shoulder (n = 5), leg (n = 2), arm (n = 1), or unknown location (n = 1). In the majority, referring pathologists considered benign diagnoses, usually benign nerve sheath tumors, but in four cases low-grade sarcoma was considered. In only three cases was SCL considered. The tumors were composed of aggregates of CD34-positive, bland spindled cells arranged in characteristic parallel arrays, admixed with ropey collagen and myxoid matrix. Isolated clusters or single adipocytes were present in 30 cases; four were devoid of fat. CD34 was diffusely positive (10/11). A high index of suspicion based on clinical context and identification of other typical features of SCL are key features to the diagnosis of low-fat and fat-free SCL.

    ABSTRACT: A spindle cell lipoma (SCL) is a relatively common tumor that can be challenging to the radiologist, pathologist, or surgeon to diagnose, particularly when internal fat content is scant or absent. Although these lesions may be found at various locations, the typical presentation for this lesion is a well-circumscribed and non-aggressive subcutaneous mass in the posterior neck presenting in a middle-aged to elderly man. In this article, the typical and atypical imaging characteristics of a spindle cell lipoma (SCL) will be reviewed. Knowledge of the common imaging and pathologic features of SCLs can help suggest the diagnosis and guide patient management.

    ABSTRACT: Lipomatous tumours are rare in the head and neck region. Their biological behaviour varies greatly, from absolutely benign to histologically benign but locally infiltrative and, finally, invasive with metastatic potential. Each lipomatous tumour has to be treated accordingly. It is of paramount importance before eventual surgery is planned to perform adequate imaging, diagnostic biopsy and careful assessment. Only in small (<5 cm), superficial soft tissue tumours or when magnetic resonance imaging has demonstrated specific features of lipoma, may diagnostic biopsy be omitted. In these cases, expectant management or simple excision is appropriate. Adequate preoperative diagnosis is important to assure adequate tumour control as well as optimal functional and cosmetic outcome. The major problem in the treatment of lipomatous tumours of the head and neck region is the presence of nearby delicate structures. Especially, wide surgical excision of liposarcomas may be hindered by anatomic constraints and may result in impaired functional and cosmetic outcome. Neoadjuvant radiotherapy and specific systemic chemotherapy may be helpful in the treatment of liposarcoma, especially when unresectable or when primary surgery is expected to result in poor oncological, functional or cosmetic outcome. Greater emphasis placed on the underlying biology of individual sarcoma subtypes, development and evaluation of novel therapies and greater specificity in the selection of chemotherapy agents based on activity in individual histological subtypes are expected to lead to improved efficacy of systemic treatment.

    ABSTRACT: Background Myxoid liposarcoma typically presents as a deep-seated mass in the lower extremity of adults. Presentation as a primary subcutaneous tumor is rare. Here we discuss clinicopathologic characteristics of three such cases and their differential diagnosis to alert dermatopathologists to this unusual clinical presentation of a potentially aggressive entity.Methods Cases of myxoid liposarcoma were retrieved from archives and consultation files. Inclusion required location above the subcutaneous fascia with no evidence of a metastatic origin. Clinicopathologic features were retrospectively reviewed. Fluorescent in situ hybridization for DDIT3 (CHOP) gene rearrangement was performed on all cases.ResultsThe tumors affected young adults (2M;1F, mean 36 yrs, range 32–40 yrs). No prior history of myxoid liposarcoma or deep soft tissue mass was identified. The tumors occurred in the foot, thigh, and hand. All demonstrated multilobular architecture with abundant myxoid stroma, prominent branching capillary vascular network, and lipoblastic differentiation. No dermal involvement was seen. Round cell features were identified in one case and represented < 5% of the tumor. All patients remain disease-free following local excision only at 6, 8 and 13 months.Conclusions Myxoid liposarcoma can rarely present as a primary subcutaneous mass and should be considered in the differential diagnosis of cutaneous myxoid tumors in adults.
