Self-reported sleep duration is associated with the metabolic syndrome in midlife adults.

ABSTRACT Short and long sleep duration have been linked to various risk factors for cardiovascular disease. In the present study, we evaluated the relationship between sleep duration and presence of the metabolic syndrome, which is a cluster of physiologically interrelated risk factors for cardiometabolic disease.
Cross-sectional community-based cohort study.
One thousand two hundred fourteen participants from the Adult Health and Behavior Project registry (aged 30 to 54 years).
Participants were divided into 4 groups based upon their reported sleep duration. The metabolic syndrome was defined according to the American Heart Association/National Heart Lung and Blood Institute's criteria. Logistic regression was used to test the hypothesis that sleep duration is a significant correlate of the metabolic syndrome and its components.
The observed metabolic syndrome rate (22%) was similar to that of published health statistics for American adults. After covariate adjustment, the odds for having the metabolic syndrome increased by more than 45% in both short and long sleepers, compared with those sleeping 7 to 8 hours per night. Sleep duration was also associated individually with abdominal obesity, elevated fasting glucose, and hypertriglyceridemia. After further adjustment for use of antihypertensive medication, prevalence of the metabolic syndrome and its components remained elevated in short sleepers only.
These data suggest that sleep duration is a significant correlate of the metabolic syndrome. Additional studies are needed to evaluate temporal relationships among these measures, the behavioral and physiologic mechanisms that link the two, and their impact on subsequent cardiometabolic disease.

    ABSTRACT: Objective To examine the association between sleep duration and cardiometabolic risk factors among individuals with recently diagnosed type 2 diabetes (n=391). Methods Sleep duration was derived using a combination of questionnaire and objective heart rate and movement sensing in the UK ADDITION-Plus study (2002-2007). Adjusted means were estimated for individual cardiometabolic risk factors and clustered cardiometabolic risk (CCMR) by five categories of sleep duration. Results We observed a J-shaped association between sleep duration and CCMR – individuals sleeping 7-<8 hours had a significantly better CCMR profile than those sleeping ≥9 hours. Independent of physical activity and sedentary time, individuals sleeping 7-<8 hours had lower triacylglycerol (0.62 mmol/l [0.29, 1.06]) and higher HDL-cholesterol levels (0.23 mmol/l [0.16, 0.30]) compared with those sleeping ≥9 hours, and a lower waist circumference (7.87 cm [6.06, 9.68]) and BMI (3.47 kg/m2 [2.69, 4.25]) than those sleeping <6 hours. Although sleeping 7-<8 hours was associated with lower levels of systolic- and diastolic- blood pressure, HbA1c, total cholesterol and LDL-cholesterol, these associations were not statistically significant. Conclusions Sleep duration has a J-shaped association with CCMR in individuals with diabetes, independent of potential confounding. Health promotion interventions might highlight the importance of adequate sleep in this high risk population.

    ABSTRACT: The interpersonal environment is strongly linked to sleep. However, little is known about interpersonal distress and its association with sleep. We examined the associations among interpersonal distress, objective and subjective sleep in people with and without insomnia. Participants in this cross-sectional observational study included men and women with insomnia (n=28) and good sleeper controls (n=38). Interpersonal distress was measured with the Inventory of Interpersonal Problems. Sleep parameters included insomnia severity, self-reported presleep arousal, and sleep quality; and polysomnographically-assessed sleep latency (SL), total sleep time (TST), wake after sleep onset (WASO), percent delta (stage 3+4 NREM), percent REM, and EEG beta power. Hierarchical linear regression was used to assess the relationship between distress from interpersonal problems and sleep and the extent to which relationships differed among insomnia patients and controls. More interpersonal distress was associated with more self-reported arousal and higher percentage of REM. More interpersonal distress was associated with greater insomnia severity and more cognitive presleep arousal for individuals with insomnia, but not for controls. Contrary to expectations, interpersonal distress was associated with shorter sleep latency in the insomnia group. Results were attenuated, but still significant, after adjusting for depression symptoms. Distress from interpersonal problems is associated with greater self-reported arousal and higher percent REM. Individuals with insomnia who report more distress from interpersonal problems have greater insomnia severity and cognitive presleep arousal, perhaps due to rumination. These findings extend our knowledge of the association between interpersonal stressors and sleep. Assessment and consideration of interpersonal distress could provide a novel target for insomnia treatment.
