Different strategies for diagnosing gestational diabetes to improve maternal and infant health

ABSTRACT Gestational diabetes mellitus (GDM) is carbohydrate intolerance resulting in hyperglycaemia with onset or first recognition during pregnancy. If untreated, perinatal morbidity and mortality may be increased. Accurate diagnosis allows appropriate treatment.
To evaluate and compare alternative tests for diagnosis of GDM, in terms of maternal and infant health and use of health service resources.
We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (31 May 2011).
We included randomised trials if they evaluated tests carried out to diagnose GDM. We excluded studies that used a quasi-random model.
Two review authors independently assessed trials for inclusion, trial quality and extracted data.
We identified five small trials, including 578 women. One trial compared three different methods of delivering glucose: a candy bar (39 women), a 50 g glucose polymer drink (40 women) and a 50 g glucose monomer drink (43 women). We have reported results for this trial as two separate comparisons.75 g oral glucose tolerance test (OGTT) versus a 100 g OGTT (one trial, 248 women): women given the 75 g OGTT had a higher relative risk of being diagnosed with GDM (risk ratio (RR) 2.55, 95% confidence interval (CI) 0.96 to 6.75).This difference was borderline for statistical significance. No other measures of maternal morbidity, or outcome for the baby were reported.Candy bar versus 50 g glucose monomer drink (one trial, 82 women): women receiving the candy bar, rather than glucose monomer, reported fewer side effects (RR 0.50, 95% CI 0.26 to 0.97) and preferred the taste (RR 0.62, 95% CI 0.44 to 0.87). No outcomes were reported for the baby.50 g glucose polymer versus 50 g glucose monomer (three trials, 259 women): mean difference (MD) in gestation at birth was -0.80 weeks (one trial, 100 women; 95% CI -1.69 to 0.09). Side effects were less common with the glucose polymer (one trial 82 women; RR 0.20 95% CI 0.07 to 0.54), with no clear difference in taste acceptability (one trial 83 women; RR 0.96; 95% CI 0.78 to 1.18). Significantly fewer women reported nausea following the 50 g glucose polymer drink compared to the 50 g glucose monomer drink (one trial 66 women; RR 0.29; 95% CI 0.11 to 0.78) and bloatedness (two trials 149 women; RR 0.22; 95% CI 0.08 to 0.60). No other measures of maternal morbidity or outcome for the baby were reported.50 g glucose in food versus 50 g glucose drink (one trial, 30 women): women receiving glucose in their food, rather than as a drink, reported fewer side effects (RR 0.08, 95% CI 0.01 to 0.56). There was no clear difference in number of women requiring further testing (RR 0.14, 95% CI 0.01 to 2.55). No other measures of maternal morbidity or outcome for the baby were reported.
There is insufficient evidence to assess which is the best strategy for diagnosing GDM.

    ABSTRACT: Early diagnosis of gestational diabetes mellitus (GDM) is important for both maternal and fetal health. The literature has varying recommendations about one-step and two-step tests for GDM screening and diagnosis. The present study aimed to investigate the difference in the cost and duration of hospital stay of a one-step procedure compared to a two-step procedure, which is routinely performed in our hospital. The two-step procedure was performed in 2,724 pregnant women, and the one-step procedure was performed in 185 pregnant women. The one-step and two-step screening procedures for gestational diabetes were compared with respect to the duration of hospital stay and cost. The test cost per woman was 0.75 TL less in the one-step procedure; however, the duration of the one-step test was 18.6 min longer, and the number of blood sampling procedures was 1.08 times higher. The one-step method may be preferred over the two-step (or glucose challenge) test due to its diagnostic value and lower cost.

    ABSTRACT: The aim of the present study was to determine the effect of various perinatal factors on cord blood TSH among newborns in Isfahan, Iran. This was a descriptive-analytic cross sectional study which performed in Isfahan Iran. During a period of four months, since February to May 2012 a total number of 440 newborns delivered in Alzahra and Shahid beheshti hospitals were enrolled in the study. For all newborns one mL blood sample from umbilical vein was obtained by one of the project investigators and sent to laboratory for further examinations. Cord blood TSH and birth body weight (BBW), gestational age, history of gestational diabetes mellitus (GDM), apgar at one minute, apgar at five minute, newborn gender and the mother's age were documented. Differences considered statistically significant if P < 0.01. 440 newborns enrolled in the study, 221 (50.2%) were male and 219 (49.8%) were female. Among study parameters, method of delivery had statistically significant relation with cord blood TSH (P < 0.001), and other factors such as BBW, gestational age, GDM, apgar at one minute, apgar at five minute, newborn gender and the mother's age didn't have statistically significant relationship with cord TSH level. In conclusion we deduce that the only factor that can affect cord blood TSH was method of delivery. Infant with vaginal delivery has higher TSH level in cord blood. Other factors that were evaluated in this study didn't have any statistically significant relationship.

    ABSTRACT: Background Gestational diabetes mellitus (GDM) is any degree of impaired glucose tolerance first recognised during pregnancy. Most women with GDM revert to normal glucose metabolism after delivery of their babies; however, they are at risk of developing type 2 diabetes later in life as are their offspring. Determining a country’s GDM prevalence can assist with policy guidelines regarding GDM screening and management, and can highlight areas requiring research. This systematic review assesses GDM prevalence in Africa. Methods and Findings Three electronic databases were searched without language restrictions; PubMed, Scopus and the Cochrane Library. Thirty-one search terms were searched. Eligible articles defined GDM, stated what GDM screening approaches were employed and reported GDM prevalence. The reporting quality and risk of bias within each study was assessed. The PRISMA guidelines for systematic reviews were followed. The literature search identified 466 unique records. Sixty full text articles were reviewed of which 14 were included in the systematic review. One abstract, for which the full text article could not be obtained, was also included. Information regarding GDM classification, screening methods and prevalence was obtained for six African countries; Ethiopia (n = 1), Morocco (n = 1), Mozambique (n = 1), Nigeria (n = 6), South Africa (n = 4) and Tanzania (n = 1). Prevalence figures ranged from 0% (Tanzania) to 13.9% (Nigeria) with some studies focussing on women with GDM risk factors. Most studies utilised the two hour 75 g oral glucose tolerance test and applied the World Health Organization’s diagnostic criteria. Conclusions Six countries, equating to 11% of the African continent, were represented in this systematic review. This indicates how little is known about GDM in Africa and highlights the need for further research. Considering the increasing public health burden of obesity and type 2 diabetes, it is essential that the extent of GDM is understood in Africa to allow for effective intervention programmes.
