Metastatic urothelial carcinoma with signet ring features: cytomorphologic findings in abdominal paracentesis.

ABSTRACT Signet ring cells, characterized by large clear cytoplasmic vacuoles eccentrically displacing and indenting the nuclei, seen in cytopathology effusion specimens are most commonly due to metastatic adenocarcinoma of gastric, esophageal, and breast origin. Urothelial carcinoma can display a wide range of differentiation and is an extremely rare cause of metastatic signet ring cells. Here, we report the case of a 75-year-old male with high-grade urothelial carcinoma with signet ring features, who was status-post chemoradiation and radical cystoprostatectomy. The patient subsequently developed an abdominal mass and ascites. The paracentesis cytology revealed a hypercellular specimen with malignant cells dispersed singly as well as in small fragments displaying pleomorphism, nuclear irregularity, hyperchromasia, and binucleation. Numerous malignant cells displayed well-formed signet ring morphology, identical to adenocarcinoma. This is the first report of metastatic high-grade urothelial carcinoma manifesting as signet ring cells in peritoneal fluid.

    ABSTRACT: Thyroid follicular neoplasms with signet ring cell morphology represent a challenging cytological and histopathologic diagnosis. The low frequency of these neoplasms and their broad differential diagnosis contribute to this difficult scenario. Here, we present an exceptionally rare case of thyroid follicular carcinoma with signet ring cell morphology in a 62-year-old female. We analyze the characteristics in fine-needle aspiration cytology, histopathology, and immunohistochemistry and compare our results in a discussion with previous literature reports.

    ABSTRACT: Background: Malignant serous cavity effusion caused by primary thyroid cancer is extremely rare in routine clinical practice. Therefore, it is often not included in the differential diagnostic workup of patients presenting with positive effusion cytology. Methods: The clinical features were reviewed for 6 patients seen at our institution over the last 26 years for malignant effusion resulting from metastatic thyroid cancer. The cytomorphology from 4 of these cases was also reviewed. Results: All of the patients found in this study presented with malignant pleural effusion - other serous cavity effusions resulting from metastatic thyroid carcinoma were not seen. These comprised 0.25% of all patients with a known history of thyroid carcinoma and 0.67% of all malignant pleural effusions. One patient had a history of bone metastases, but all the others had no pathological evidence of distant metastatic disease prior to the pleural effusion. Conclusions: Malignant pleural effusion rarely results from a thyroid carcinoma after some latency. The diagnosis requires immunohistochemical staining as well as the pertinent clinical context.

    ABSTRACT: OBJECTIVE: To study the cytomorphological features of squamous cell carcinoma (SCC) in serous effusions. METHODS: Conventional smears from 23 SCC were reviewed for the following features: cellularity, single cells, cell clusters, long cords, squamous pearls, cell-in-cell, 'windows', polygonal cells, anucleated cells, tadpole cells, fibre cells, third-type cells, refractile cytoplasmic ring, two-tone cytoplasm, cytoplasmic density, cytoplasmic vacuoles, signet ring cells, nuclear/cytoplasmic ratio, nuclear hyperchromasia, irregular nuclear membrane, nuclear centricity, double or multiple nuclei, nucleolus, anaplastic cells and mitosis. Subsequently, we also reviewed 15 reactive mesothelium, 18 mesotheliomas and 24 adenocarcinomas for comparison. RESULTS: Most SCC smears comprised mixed populations of single cells and cell clusters. Keratinized squamous cells and squamous pearls were diagnostic for well-differentiated SCC. Squamous eddies, appreciated on the cell block sections appearing as two-dimensional cell clusters with a swirling pattern, were characteristic for SCC. Poorly differentiated/third-type cells with atypical cytoplasmic keratinization, manifest as two-tone cytoplasm with a well-defined border and occasionally a refractile central ring, were an important clue for the squamous origin of the cells. Features including cell-in-cell, cytoplasmic vacuolation, double or multinucleated cells, anaplastic cells and mitotic figures were present in various degrees, and were not of diagnostic value. CONCLUSIONS: Certain cytological features are diagnostic for SCC, whereas other features of poorly differentiated SCC often overlap with those of reactive mesothelial cells, mesothelioma, metastatic adenocarcinoma or urothelial carcinoma in serous effusions. The combination of clinical history, appropriate immunohistochemical panel and cytomorphological features are important to ensure a correct diagnosis.
