Dietary strategies of immunomodulation in infants at risk for celiac disease

Celiac disease is an inflammatory disorder of the small intestine, triggered by the ingestion of gluten proteins contained in wheat, barley or rye, in genetically susceptible individuals. This disorder is considered to be mainly mediated by cellular immunity and restricted to the human leucocyte antigen-DQ presentation of gluten-derived toxic peptides to T-cells. Moreover, the involvement of innate immunity has been recently demonstrated to be necessary also for the development of intestinal tissue damage. Genetic susceptibility accounts for an uncertain proportion of the disease risk and gluten introduction works as the precipitating factor. However, currently, the research interest is also focused on environmental factors and gene-environment interactions, especially during the first months of life, which might help explain the onset of the disease. Infectious and dietary factors that could modulate the immune response orientating it either towards tolerance or intolerance/autoimmunity are the focus of primary attention. A significant number of studies have looked into the protective effect of breast-feeding against the disease. It is generally accepted that breast-feeding during the introduction of dietary gluten and increasing the duration of breast-feeding are associated with reduced risk of developing celiac disease. However, it is still not fully established whether breast-feeding truly protects with permanent tolerance acquisition or only reduces the symptoms and delays the diagnosis. Moreover, the timing and dose of gluten introduction also seem to be relevant and long-term prospective cohort studies are being carried out in order to elucidate its role in celiac disease development.

    ABSTRACT: Many recent studies overshadow the effects of gluten-free diet. Gluten-free diet positive effects were observed in celiac disease patients: increase in body mass index, higher energy intakes, reducing adiposity gain, moderates the risk of the associated complications. However, adhering to a gluten-free diet is difficult for many people. A new solution is needed for quality of life of celiac disease patients, not for celiac disease treatment. Health education on gluten-free diet at home and in society seems to be the solution. The aim of our study is to evaluate the recent research on gluten-free diet as a nutritional therapy for patients with celiac disease. To achieve this purpose we have analyzed the published studies from 2008 to the present on nutrition in celiac disease.

    ABSTRACT: Interaction energies between dipalmitoylphosphatidylcholine (DPPC) bilayers and corundum (α-Al(2)O(3)) or quartz (α-SiO(2)) are calculated according to extended-Derjaguin-Landau-Verwey-Overbeek (DLVO) theory in order to examine oxide-dependent multiple bilayer adsorption. Energies are investigated at two ionic strengths (I=17 or 217 mM) and compared to corresponding DPPC adsorption experiments on corundum and quartz particles. The repulsive hydration interaction diminishes sharply with separation distance, and beyond ∼3 nm, the interaction energy is determined by the electrostatic and van der Waals contributions only. We revise the electrostatic interaction to account for solution exclusion by the bilayers, extending the distance over which this interaction occurs. Calculated oxide-bilayer interaction energies explain our experimentally-observed adsorption of three bilayers on corundum compared to two on quartz at low I, and only two bilayers on both oxides at high I. The results are consistent with our hypothesis that electric double-layer extension at low I allows oxide-bilayer electrostatic interactions to contribute to deposition of DPPC bilayers at large (∼14 nm) separations. Theoretically-predicted pseudo-infinite supported bilayer stacks formed from vesicles are not observed experimentally due to kinetic considerations. Potential relevance to biomedical applications and to the role of mineral surfaces in proto-cell membrane self-assembly is discussed.

    ABSTRACT: Many human genes have adapted to the constant threat of exposure to changing environmental conditions. Balancing selection is the result of an initial stage of positive selection that favors the spread in a population of a new allele until selection opposes its fixation and balanced situation is established. It is suggested that the disadvantageous celiac patients survived the last two millennias echo-catastrophes by adapting to the extreme temperature changes and their consequences in Europe. Genetic selective diversity induced by changing environment, enabled the celiac population to survive. Such a genetic positive selection is represented by the HEF C282Y mutation of hemochromatosis, SH2B3 loci and the HLA celiac disease-associated repertoire, enabling the celiac to overcome iron deficiency anemia and micro pathogen richness, respectively. The increased incidence of those evolutionary events in the celiac patients is a recent phenomenon that occurred in the latest era of the modern human history. The present hypothesis can shed light on additional selective genetic adaptations, echo catastrophe-driven that are at the basis of autoimmune disease-affected population survival and current expansion.
