Oxygen uptake efficiency slope and chronotropic incompetence in chronic heart failure and chronic obstructive pulmonary disease.

ABSTRACT Oxygen uptake efficiency slope (OUES) and chronotropic index (CRI) are measures of cardiorespiratory reserve and chronotropic incompetence, but no direct comparison of OUES and CRI in patients with chronic obstructive pulmonary disease (COPD) and chronic heart failure (CHF) has been done.
To compare OUES and CRI in CHF and COPD patients.
Fifty-one subjects divided in three groups--CHF group (n=17) (NYHA II and III functional class), COPD group (n=17) (FEV1% = 49.7 +/- 17.3) and healthy controls (C) (n=17), matched by age and BMI. COPD and CHF patients presented comparable decrease in functional capacity.
Subjects undertook maximal exercise test on a treadmill by means of Bruce protocol.
CHF and COPD patients showed a significant and comparable decrease in exercise capacity (VO2/kg = 18.5 +/- 4.0 vs. 19.5 +/- 4.9 vs. 28.3 +/- 6.2 mL x min(-1) x kg(-1), p < 0.001) and ventilatory efficiency in comparison with controls. COPD patients presented with a typical pattern of exercise limitation due to ventilatory restraints (%breathing reserve = 15.6 +/- 18.0%). OUES was lower than controls in both CHF and COPD groups, being lowest in CHF (1686 +/- 483 vs. 2101 +/- 478 vs. 2481 +/- 398 mL.min(-1) x logL(-1), p < 0.001). Chronotropic index was lower in CHF patients (0.60 +/- 0.15 in CHF vs. 0.72 +/- 0.16 in COPD vs. 0.81 +/- 0.18 in controls, p < 0.001) in comparison with controls.
OUES is decreased in CHF and COPD patients, but the decline is significantly more pronounced in the CHF group. CRI is lower in CHF patients.

    ABSTRACT: BAKGRUNN: Staking blir en stadig mer benyttet skiteknikk i takt med økt gjennomsnittshastighet i langrennskonkurranser og bedre overkroppstrente utøvere. VO2max korrelerer sterk med prestasjonen i utholdenhetsidretter (Costill et al. 1973, Støa et al. 2010). Ved maksimalt aerob arbeid i staking er det en dårlig utnyttelse av det maksimale oksygenopptaket. VO2peak i staking ligger kun på 85 % av VO2max målt i løping (Hoff et al. 2002, Nilsson et al. 2004). Dette studiet så derfor på om høyintensiv aerob intervalltrening utført som staking på rulleskimølle vil øke VO2peak i staking og flytte VO2peak i staking nærmere VO2max i løping, slik at en kan øke utnyttelsen av maksimalt oksygenopptak ved staking. Det ble også sett på endringer av stakeøkonomi og om en endring av disse prestasjonsbestemmende faktorene ville bedre prestasjonen på 3km staking. METODE: 16 forsøkspersoner med alder på 25 ± 9 år og VO2max på 69,3 ± 9,0 mL∙kg-1∙min-1gjennomførte studien, n = 9 i intervensjonsgruppa (2 jenter) og n = 7 i kontrollgruppa (kun menn). Intervensjonsgruppa gjennomførte 4x4min stakeintervalltrening, 3 ganger i uka i 6 uker på rulleskimølle. Intensiteten var på ≥ 90 % av Hfpeak målt i staking. Kontrollgruppa gjennomførte sin vanlige trening. RESULTATER: VO2peak i staking økte signifikant med 5,7 % fra 51,6 til 54,5 mL∙kg-1∙min- 1 og med 6,2 % fra 214,6 til 228,0 mL∙kg-0,67∙min-1. Arbeidsøkonomien forandret seg ikke signifikant, med det var en god tendens til forbedring. Ingen signifikante endringer ble funnet i VO2max (løping), mens VO2peak endret seg signifikant fra 81,6 til 88,4 % av VO2max. Prestasjonen på 3km staking bedret seg med 164 sek (19,7 %) i intervensjonsgruppa, bedringen var 139 sek (16,7 %) bedre enn i kontrollgruppa. KONKLUSJON: Høyintensiv aerob intervalltrening i staking øker VO2peak, utnyttelsen av maksimalt oksygenopptak ved staking og prestasjonen på 3km staking.

    ABSTRACT: A. Study Purpose and Rationale Chronotropic incompetence (CI) is the failure to adequately augment heart rate during physical exertion. Down-regulation and/or cardiac beta-receptor desensitization in the background of generalized increased sympathetic activation is currently thought to be the underlying mechanism of CI (1,2) . CI is independently associated with increased mortality, irrespective of other risk factors and whether or not individuals are receiving beta-blocker therapy (3, 4, 5) . Although CI can occur in the general population, it has specifically been observed in two populations; patients with heart failure and COPD. Over 30% of patients with heart failure have been found to have CI, which may impact physical exertion in this population. Patients with chronic heart failure are often limited in their ability to perform maximal physical activity as a result of a decrease in maximal oxygen uptake (VO2max). VO2 max is dependent on cardiac output, which is reduced in heart failure patients due to decreased stroke volume as a consequence of systolic dysfunction. To maintain cardiac output, heart failure patients are more reliant on their ability to increase heart rate, which is also dampened in patients with CI (1,2) . It has been observed that CI also occurs in the COPD population, but there is little published data relating to CI in these patients (6,7) . In the COPD population, we know that VO2max is typically decreased due to abnormalities in ventilation associated with the relatively shorter time for exhalation, gas exchange abnormalities, and skeletal muscle changes relating to the disease and deconditioning (8) . However, the specific role of CI in this group of patients has not been elucidated.

    ABSTRACT: Lung volume reduction surgery, originally used in the 1950s, has reemerged as a technique that may help some individuals with end-stage emphysema breathe more easily. The goal of the surgery is to enhance elastic recoil of the lung and to restore diaphragmatic function. The surgery is still in the experimental stages, but early outcome data are encouraging. The purpose of this study was to discuss the pathology of emphysema and the rationale for lung volume reduction. The patient selection criteria for the operation are discussed, and information is provided regarding the preoperative and postoperative management and rehabilitation of these patients by physicians, nurses, respiratory therapists, and physical therapists.
