Associations between Polycyclic Aromatic Hydrocarbon–Related Exposures and p53 Mutations in Breast Tumors

Previous studies have suggested that polycyclic aromatic hydrocarbons (PAHs) may be associated with breast cancer. However, the carcinogenicity of PAHs on the human breast remains unclear. Certain carcinogens may be associated with specific mutation patterns in the p53 tumor suppressor gene, thereby contributing information about disease etiology.Objectives: We hypothesized that associations of PAH-related exposures with breast cancer would differ according to tumor p53 mutation status, effect, type, and number.
We examined this possibility in a population-based case-control study using polytomous logistic regression. As previously reported, 151 p53 mutations among 859 tumors were identified using Surveyor nuclease and confirmed by sequencing.
We found that participants with p53 mutations were less likely to be exposed to PAHs (assessed by smoking status in 859 cases and 1,556 controls, grilled/smoked meat intake in 822 cases and 1,475 controls, and PAH-DNA adducts in peripheral mononuclear cells in 487 cases and 941 controls) than participants without p53 mutations. For example, active and passive smoking was associated with p53 mutation-negative [odds ratio (OR) = 1.55; 95% confidence interval (CI), 1.11-2.15] but not p53 mutation-positive (OR = 0.77; 95% CI, 0.43-1.38) cancer (ratio of the ORs = 0.50, p < 0.05). However, frameshift mutations, mutation number, G:C-->A:T transitions at CpG sites, and insertions/deletions were consistently elevated among exposed subjects.
These findings suggest that PAHs may be associated with specific breast tumor p53 mutation subgroups rather than with overall p53 mutations and may also be related to breast cancer through mechanisms other than p53 mutation.

    ABSTRACT: Vehicle engine exhaust includes ultrafine particles with a large surface area and containing absorbed polycyclic aromatic hydrocarbons, transition metals and other substances. Ultrafine particles and soluble chemicals can be transported from the airways to other organs, such as the liver, kidneys, and brain. Our aim was to investigate whether air pollution from traffic is associated with risk for other cancers than lung cancer. We followed up 54,304 participants in the Danish Diet Cancer and Health cohort for 20 selected cancers in the Danish Cancer Registry, from enrolment in 1993-1997 until 2006, and traced their residential addresses from 1971 onwards in the Central Population Registry. We used modeled concentration of nitrogen oxides (NO(x)) and amount of traffic at the residence as indicators of traffic-related air pollution and used Cox models to estimate incidence rate ratios (IRRs) after adjustment for potential confounders. NO(x) at the residence was significantly associated with risks for cervical cancer (IRR, 2.45; 95% confidence interval [CI], 1.01;5.93, per 100 μg/m(3) NO(x)) and brain cancer (IRR, 2.28; 95% CI, 1.25;4.19, per 100 μg/m(3) NO(x)). This hypothesis-generating study indicates that traffic-related air pollution might increase the risks for cervical and brain cancer, which should be tested in future studies.

    ABSTRACT: Although melancholy has been the focus of a large number of studies, this has not been the case for mania and even less for the passage from melancholy to mania, more particularly the passage from a depressive state to a self-triumphant state, the latter expression being used by Freud to characterize states of self-induced excitement or elation. On the basis of the specific pathological model of the passage from melancholy to mania, the present study has therefore attempted to determine the underlying metapsychological processes for what are commonly termed cyclic disorders or changes of mood, the dynamics of which is connected with the types of interaction between the various elements within the psychic system. The theoretical approach that has been adopted is essentially psychoanalytical, and in parallel to Freud’s analysis of this state, we are already able to delineate the context in which cyclic disorders occur: it consists of an “interior theatre”, and on its stage the self in its relation to the ideal self which continues to play the same role as it had previously assumed in its relation to the exterior object, i.e. before it had incorporated the exterior object and allowed it to take its place. The self thus finds itself in the position of the object confronted with its ideal self, and the omnipotence of the latter henceforth determines the course of their relations.Melancholy and mania are thus related to the same “complex” and their metapsychological difference lies in the fate of the introjected object: if in the case of melancholy the object has taken over the role assumed by the self, what has become of the self in the manic state? In the present study, the hypothesis proposed is that of committing an intrapsychic act, an indefinitely perpetrated crime, i.e. the act of murdering the object constructed on the basis of a working model of mourning which requires that the mourner “strikes a fatal blow” for a second time in order to actively confirm the subject’s own satisfaction at remaining alive. However, the state of manic exhaltation, just like the state of melancholic depression, is linked to the conflict between the self and its ideal self and to the law of an alter ego that is always as cruel toward a mythical lost object which remains a cause of irrepressible nostalgia.

    ABSTRACT: Exposures to carcinogenic polycyclic aromatic hydrocarbons (PAHs) have been linked to human lung cancer. The purpose of this study was to assess lung cancer risk caused by inhalation exposure to nano/ultrafine particle-bound PAHs at the population level in Taiwan appraised with recent published data. A human respiratory tract model was linked with a physiologically based pharmacokinetic model to estimate deposition fraction and internal organic-specific PAHs doses. A probabilistic risk assessment framework was developed to estimate potential lung cancer risk. We reanalyzed particle size distribution, total-PAHs, particle-bound benzo(a)pyrene (B[a]P) and PM concentrations. A dose-response profile describing the relationships between external B[a]P concentration and lung cancer risk response was constructed based on population attributable fraction (PAF). We found that 90% probability lung cancer risks ranged from 10(-5) to 10(-4) for traffic-related nano and ultrafine particle-bound PAHs, indicating a potential lung cancer risk. The particle size-specific PAF-based excess annual lung cancer incidence rate due to PAHs exposure was estimated to be less than 1 per 100,000 population, indicating a mild risk factor for lung cancer. We concluded that probabilistic risk assessment linked PAF for limiting cumulative PAHs emissions to reduce lung cancer risk plays a prominent role in future government risk assessment program.
