Usefulness of Percutaneous Intervention with Transarterial Approach in the Salvage of Nonmaturing Native Fistulas Status-Post Transvenous Approach Failure
ABSTRACT The purpose of this study was to assess the usefulness of percutaneous intervention with transarterial approach in the salvage of nonmaturing native fistulas status post transvenous approach failure. Eight patients (M:F=2:6; mean age, 56.8 years) underwent percutaneous intervention with antegrade transarterial approach when the retrograde transvenous approach failed. Mean time from fistula creation to fistulography was 80.5 days. Five patients had brachiocephalic fistulas and three had radiocephalic fistulas. Brachial or radial arterial access was performed using real-time ultrasound guidance, and balloon angioplasty was performed for hemodynamically significant (>50%) stenosis. Technical and clinical success and complications were evaluated. Patency following percutaneous intervention was estimated with the Kaplan-Meier method. Technical success was achieved in 87.5% (seven of eight patients). Clinical success of normal hemodialysis (fistula maturation) occurred in 87.5% (seven of eight patients). One minor complication (regional hematoma) occurred in one patient. Mean primary patency at 6, 12, and 18 months was 75%, 56.2%, and 37.5%, respectively. Additional angioplasty (n=3) resulted in mean secondary patency at 12 and 18 months of 87.5% and 87.5% of the patients, respectively. In conclusion, percutaneous intervention with the transarterial approach can be a useful method for salvage of nonmaturing native fistulas when the transvenous approach fails.

ABSTRACT: Site of access vascular complications is infrequent after transradial interventions. We report the case of a 66-year-old man referred to our hospital because of right forearm swelling, oedema and pain with functional forearm disability, 1 year after a transradial primary percutaneous coronary intervention (PCI). The diagnostic and procedural issues are discussed. ABSTRACT: Purpose: To evaluate the usefulness of an approach through the radial artery distal to the arteriovenous anastomosis for salvaging nonmaturing radiocephalic arteriovenous fistulas. Methods: Procedures that fulfilled the following criteria were retrospectively reviewed: (1) autogenous radiocephalic fistulas, (2) fistulas less than 3 months old, (3) distal radial artery approach for salvage. From 2005 to 2011, a total of 51 patients fulfilling the above criteria were enrolled. Outcome variables were obtained from angiographic, clinical and hemodialysis records, including the success, complication, and primary and secondary patency rates. Results: The overall anatomical and clinical success rates for the distal radial artery approach were 96 and 94%, respectively. The average procedure time was 36 ± 19 min. Six patients (12%) experienced minor complications as a result of extravasations. No arterial complication or puncture site complication was noted. The postinterventional 6-month primary patency rate was 51%, and the 6-month secondary patency rate was 90%. When the patients were divided into a stenosed group (20 patients) and an occluded group (31 patients), there were no differences in the success rate, complication rate, or primary and secondary patency rates. Conclusion: An approach through the radial artery distal to the arteriovenous anastomosis is an effective and safe alternative for the salvage of nonmaturing radiocephalic arteriovenous fistulas, even for occluded fistulas.CardioVascular and Interventional Radiology 12/2012; 36(4). ABSTRACT: To report the technical success and long-term clinical outcomes of immature arteriovenous fistula (AVF) after percutaneous angioplasty (PTA). Patients with 58 immature AVF underwent PTA treatment in our institute during the past 9 years. Based on pretreatment ultrasound findings, the immature AVFs were categorized as stenosed, thrombosed, and obliterated type. An optimal entry site was punctured under ultrasound guidance followed by conventional fluoroscopy-guided PTA technique. Technical and clinical success as well as complications were recorded. Patency after angioplasty was estimated using Kaplan-Meier analysis. Predictors of patency were estimated using log-rank test. In our series, 50 % (29 of 58), 20.7 % (12 of 58), and 29.3 % (17 of 58) of immature AVF were stenosed, thrombosed, and obliterated, respectively. Technical and clinical success was achieved in 96.6 % (56 of 58) of cases with 100 % success in both the stenosed and thrombosed lesions and 88.2 % (15 of 17) success in the obliterated lesions. Vascular ruptures occurred in 12.1 % (7 of 58) of lesions. The overall primary and secondary patency rates of the 58 lesions were 45.4 and 84.2 % at 12 months and 36.5 and 80.1 % at 36 months. There were no significant differences of patency between the three types of lesions (p = 0.075 and 0.093) and the two groups of patients with or without residual side branches after intervention (p = 0.527 and 0.644). There was a significant difference of primary patency in patients with vascular rupture (p = 0.012) with a hazard ratio of 3.236. PTA could effectively prolong the lifetime of immature AVFs with high technical success. Vessel rupture was predictive of shorter primary patency, and long-term secondary patency was acceptable.CardioVascular and Interventional Radiology 02/2014; 37(3).
