Riesgo nutricional en pacientes con tuberculosis pulmonar: ¿cuestión del paciente o de los servicios de salud?

ABSTRACT OBJETIVO: Identificar factores de riesgo nutricional en pacientes con tuberculosis pulmonar (TBP).MATERIAL Y MÉTODOS: Se seleccionaron al azar 185 pacientes con TBP atendidos en dos instituciones de salud de Monterrey, Nuevo León, México, durante 1997. Se identificaron variables antropométricas, socioeconómicas, utilización del servicio de nutrición, accesibilidad a los alimentos, efectos secundarios de drogas antifímicas, y atribución de la enfermedad a la alimentación. El plan de análisis incluyó estadística descriptiva, análisis bivariado y multivariado de regresión logística múltiple, además se estimó razón de prevalencia e intervalos de confianza de 95%. RESULTADOS: El promedio de edad fue de 42.4±19.9 años. La media de índice de masa corporal fue de 19.8±3.2 y se encontraban desnutridos 56.8% del total de los pacientes. El 26.4% de éstos fue enviado al servicio de nutrición y, únicamente, 24.3% lo utilizó. El análisis multivariado mostró como factores de riesgo para desnutrición a los efectos secundarios de las drogas antifímicas, independientemente de la edad, sexo, escolaridad, ocupación, tiempo de evolución, accesibilidad a los alimentos, atribución de la enfermedad al tipo de alimentación y utilización del servicio de nutrición (ji²=10.58; p=0.0515, r²=0.42). CONCLUSIONES: El riesgo nutricional al que se enfrenta el paciente es responsabilidad tanto de éste, por la escasa utilización que hace del servicio de nutrición, como de los servicios de salud, debido a la existencia de barreras de tipo organizacional que dificultan el acceso al servicio de nutrición. Además, la falta de accesibilidad a los alimentos y el impacto de los efectos secundarios de las drogas antifímicas justifican la necesidad de focalizar la atención en este grupo de riesgo.

    ABSTRACT: We assessed nutritional status in 200 adult Tanzanian patients with smear-positive pulmonary tuberculosis before, during, and after 6 months of tuberculosis treatment; 148 patients (74%) were successfully followed for 12 months. Marked nutritional impairment was present on admission: 77% of males and 58% of females had a body mass index (BMI) below 18.5; approximately one-fifth had BMI < 16.0. The length of hospital stay and gender, rather than microbiological response, were the major determinants of weight gain during treatment. Patients infected with human immunodeficiency virus (HIV) gained more weight than uninfected patients. Most patients lost weight after completing treatment and returning home. At 12 months, 32% of male and 19% of female patients considered cured of tuberculosis had BMI < 18.5. It is concluded that patients with tuberculosis from this area of Tanzania frequently have evidence of malnutrition both before and after treatment for tuberculosis. Weight gain during therapy appeared to be an unreliable indicator of overall treatment response. However, the results also demonstrated that nutritional rehabilitation can be successfully achieved even in HIV-positive tuberculosis patients and in patients with a suboptimal response to therapy.

    ABSTRACT: This paper reviews current knowledge about the role that socio-economic and cultural factors play in determining gender differentials in tuberculosis (TB) and tuberculosis control. The studies reviewed suggest that socio-economic and cultural factors may be important in two ways: first, they may play a role in determining overall gender differences in rates of infection and progression to disease, and second, they may lead to gender differentials in barriers to detection and successful treatment of TB. Both have implications for successful TB control programmes. The literature reviewed in this paper suggests the following: Gender differentials in social and economic roles and activities may lead to differential exposure to tuberculosis bacilli; The general health/nutritional status of TB-infected persons affects their rate of progression to disease. In areas where women's health is worse than men's (especially in terms of nutrition and human immunodeficiency virus status), women's risk of disease may be increased; A number of studies suggest that responses to illness differ in women and men, and that barriers to early detection and treatment of TB vary (and are probably greater) for women than for men. Gender differences also exist in rates of compliance with treatment; The fear and stigma associated with TB seems to have a greater impact on women than on men, often placing them in an economically or socially precarious position. Because the health and welfare of children is closely linked to that of their mothers, TB in women can have serious repercussions for families and households. The review points to the many gaps that exist in our knowledge and understanding of gender differentials in TB and TB control, and argues for increased efforts to identify and address gender differentials in the control of TB.

    ABSTRACT: Wasting in African AIDS patients is severe, and its aetiology is probably multifactorial: persistent diarrhoea, poverty and tuberculosis may all contribute. We report a cross-sectional study of body composition measured anthropometrically in 75 adult patients with HIV-related persistent diarrhoea in Lusaka, and its relationship to gastrointestinal infection and systemic immune activation assessed using serum neopterin and soluble tumour necrosis factor receptor (sTNF-R55) concentrations. Patients as a group were generally severely wasted (mean body mass index (BMI) 15.8 kg/m2, range 11-22), but the severity of wasting was related neither to oesophageal candidiasis nor to intestinal infection. In men but not women, all measures of nutritional status were negatively related to serum sTNF-R55 concentration (fat-free mass in men, r = -0.64; 95% CI: -0.80, -0.41; p < 0.0001). Some wasted patients had cutaneous features of malnutrition, again associated with higher sTNF55 concentrations, and two had peripheral oedema. The diarrhoea-wasting syndrome in this part of Africa seems to be associated with evidence of high cytokine activity in men, rather than oesophageal candidiasis or any particular intestinal opportunistic infection. This immune activation requires further investigation in the context of the sex difference we have observed.
