    ABSTRACT: Today, many countries have succeeded in integrating telemedicine and advanced technologies into a broad-range of healthcare processes in-1 cluding diagnosis, treatment, disease prevention, and health education & research. Nevertheless, many developing countries are still unable to sustain meaningful telemedicine projects. Egypt has achieved significant progress in building the Information Society (IS), by providing an enabling legal and regulatory framework, and Information Technology Institute (ITI), Ministry of an adequate Information and Communications Technology (ICT) infra-Communications and structure. However, telemedicine projects in Egypt still face common Information Technology (MCIT), Giza, Egypt problems and challenges that hinder the wide-scale adoption of eHealth systems. This study provides a comprehensive Strengths, Weaknesses, Opportunities , and Threats (SWOT) analysis of the current telemedicine applications in Egypt. Based on that, four future trends in Telemedicine in Egypt 2020 have been identified from governmental, financial, technological , and medical perspectives. Consequently, these future trends were aligned to the global trends in telemedicine. The main output of this study is that telemedicine should be part of a National eHealth Initiative. Zusammenfassung Bis heute konnten in vielen Ländern erfolgreich Telemedizin und moder-ne Informationstechnik in vielen medizinischen Prozessen von der Diagnostik über Therapie und Vorsorge bis hin zur Medizinischen Aus-bildung und Forschung integriert werden. Aber gerade in Entwicklungs-ländern ist es oft nicht gelungen, nützliche Telemedizin-Lösungen dau-erhaft zu etablieren. Ägypten hat bereits erhebliche Fortschritte dabei gemacht, sich zu einer Informationsgesellschaft zu entwickeln. Es wurden rechtliche und regu-latorische Rahmenbedingungen geschaffen, in deren Rahmen eine leistungsfähige Infrastruktur für Informations-und Kommunikations-technik aufgebaut werden konnte. Dennoch sehen sich ägyptische Te-lemedizinprojekte immer noch mit erheblichen Problemen und Heraus-forderungen konfrontiert, wenn es darum geht, die Ergebnisse flächen-deckend und in Routine anzuwenden. Diese Arbeit berichtet über eine umfassende SWOT-Analyse (Strengths, Weaknesses, Opportunities, and Threats) der Telemedizin in Ägypten. Mit Hilfe der SWOT-Analyse konnten vier Entwicklungstrends identifiziert werden, welche für die Entwicklung der ägyptischen Telemedizin bis 2020 bestimmend sein werden. Diese Trends beschreiben mögliche und notwendige Entwicklungen in den Bereichen der Regierungsverant-wortung, der Finanzierung telemedizinischer Vorhaben, der Technik und der Medizin, die eng mit den entsprechenden globalen Trends zu-sammenhängen. Telemedizin wird in Ägypten aber nur dann erfolgreich sein, wenn sie Teil einer nationalen eHealth-Initiative ist.

    ABSTRACT: Telemedicine is the use of telecommunications technology to support health care at a distance. Dermatology relies on visual cues that are easily captured by imaging technologies, making it ideally suited for this care model. Advances in telecommunications technology have made it possible to deliver high-quality skin care when patient and provider are separated by both time and space. Most recently, mobile devices that connect users through cellular data networks have enabled teledermatologists to instantly communicate with primary care providers throughout the world. The availability of teledermoscopy provides an additional layer of visual information to enhance the quality of teleconsultations. Teledermatopathology has become increasingly feasible because of advances in digitization of entire microscopic slides and robot-assisted microscopy. Barriers to additional expansion of these services include underdeveloped infrastructure in remote regions, fragmented electronic medical records, and varying degrees of reimbursement. Teleconsultants also confront special legal and ethical challenges as they work toward building a global network of practicing physicians. Copyright © 2014 American Academy of Dermatology, Inc.     ABSTRACT: Despite the importance of pain management to each patient's overall experience with a total knee replacement, opportunities to improve pain care exist. The authors target an unnecessarily fragmented pain management trajectory as one cause of variability in pain outcomes. They propose that a technology-enhanced patient-centered pain management continuum running from the preoperative through the recovery phase offers effective and efficient pain management.
