Career fit and burnout among academic faculty.

ABSTRACT Extensive literature documents personal distress among physicians and a decrease in their satisfaction with the practice of medicine over recent years. We hypothesized that physicians who spent more of their time in the aspect of work that they found most meaningful would have a lower risk of burnout.
Faculty physicians in the Department of Internal Medicine at a large academic medical center were surveyed in the fall of 2007. The survey evaluated demographic variables, work characteristics, and career satisfaction. Burnout was measured using the Maslach Burnout Inventory. Additional questions evaluated which professional activity (eg, research, education, patient care, or administration) was most personally meaningful and the percentage of effort that was devoted to each activity.
Of 556 physicians sampled, 465 (84%) returned surveys. A majority (68%) reported that patient care was the aspect of work that they found most meaningful, with smaller percentages reporting research (19%), education (9%), or administration (3%) as being most meaningful. Overall, 34% of faculty members met the criteria for burnout. The amount of time spent working on the most meaningful activity was strongly related to the risk of burnout. Those spending less than 20% of their time (approximately 1 d/wk) on the activity that is most meaningful to them had higher rates of burnout (53.8% vs 29.9%; P<.001). Time spent on the most meaningful activity was the largest predictor of burnout on multivariate analysis (odds ratio, 2.75; P = .001).
The extent to which faculty physicians are able to focus on the aspect of work that is most meaningful to them has a strong inverse relationship to their risk of burnout. Efforts to optimize career fit may promote physician satisfaction and help to reduce attrition among academic faculty physicians.

    ABSTRACT: As the numbers of female physicians continue to grow, fewer medical marriages are comprised of the traditional dyad of male physician and stay-at-home wife. The "two-career family" is an increasingly frequent state for both male and female physicians' families, and dual-doctor marriages are on the rise. This qualitative study explored the contemporary medical marriage from the perspective of male spouses of female physicians. In 2010, we conducted semi-structured, in-depth interviews with nine spouses of internal medicine resident and faculty physicians. Interviewers queried work-home balance, career choices, and support networks. We used an interpretive, inductive, iterative approach to thematically analyze interview transcripts and develop broad, consensus-derived themes. A conceptual framework based on three major themes emerged: "A time for us? Really?", "Supporting and protecting her, sometimes at my expense,'" and "Hers is a career, mine is a job." This framework described the inflexibility of physicians' time and its impact on spousal time, career development, and choices. Having a set time for synchronizing schedules, frequent verbal support, and shared decision-making were seen as important by the husbands of female, full-time physicians. This exploratory study examined the contemporary medical marriage from the male spouse's perspective and highlights specific strategies for success.

    ABSTRACT: Mental health is a growing concern in the fi eld of occupational health globally due to its social, health, and economic implications on the individual and the community at large. As universities in Malaysia are advancing towards becoming world-class research universities, academicians are now faced with more pressures, making them more vulnerable to burnout. A cross sectional study was conducted among 194 randomly sampled academicians from a faculty of a public university. Data was collected using a self-administered questionnaire to assess the sociodemographic characteristics, occupational characteristics and burnout. Analysis was done using SPSS version 21. The response rate was 87.1%. The overall prevalence of burnout was 10.7% with signifi cant associations between gender (χ 2 = 4.61; p = 0.03), teaching experience (χ 2 = 0.83; p = 0.02), job satisfaction (χ 2 = 11.33; p = 0.001). Female academicians have a higher prevalence of burnout (14.3%). Academicians who have fewer years of teaching experience ((6 years) and those with low job satisfaction scores reported a higher prevalence of burnout (16.8% and 28.6%, respectively). As for prediction analysis, females were 4 times more likely to experience burnout compared to males (aOR = 4.53; 95%CI 0.95-21.72) and those with teaching experience of less than 6 years were also four times more likely to experience burnout (aOR = 4.14; 95%CI 1.21–14.19). The odd of experiencing burnout among those who were dissatisfi ed with their job was seven times more than among academicians who were satisfi ed with their jobs (aOR = 6.72; 95%CI 2.15–21.04). Burnout was found to be prevalent among academician. Being a female academician, with fewer years of teaching experience and low job satisfaction were among the factors that have signifi cant associations with burnout. Therefore, these groups should be targeted by the university administration to fi nd mechanisms to reduce the prevalence of burnout which will eventually contribute to the quality of teaching and high job commitment and pave way for the institution towards a world-class university.
