Educating patients about warfarin therapy using information technology: A survey on healthcare professionals’ perspectives

ABSTRACT To explore healthcare professionals' views about the benefits and challenges of using information technology (IT) resources for educating patients about their warfarin therapy.
A cross-sectional survey of both community and hospital-based healthcare professionals (e.g., doctors, pharmacists and nurses) involved using a purpose-designed questionnaire. The questionnaires were distributed using a multi-modal approach to maximise response rates.
Of the total 300 questionnaires distributed, 109 completed surveys were received (43.3% response rate). Over half (53.2%) of the healthcare participants were aged between 40-59 years, the majority (59.5%) of whom were female. Fifty nine (54.1%) participants reported having had no access to warfarin-specific IT-based patient education resources, and a further 19 (38.0%) of the participants who had IT-access reported that they never used such resources. According to the healthcare participants, the main challenges associated with educating their patients about warfarin therapy included: patient-related factors, such as older age, language barriers, cognitive impairments and/or ethnic backgrounds or healthcare professional factors, such as time constraints. The healthcare professionals reported that there were several aspects about warfarin therapy which they found difficult to educate their patients about which is why they identified computers and interactive touch screen kiosks as preferred IT devices to deliver warfarin education resources in general practices, hospital-based clinics and community pharmacies. At the same time, the healthcare professionals also identified a number of facilitators (e.g., to reinforce warfarin education, to offer reliable and easily comprehensible information) and barriers (e.g., time and costs of using IT resources, difficulty in operating the resources) that could impact on the effective implementation of these devices in educating patients about their warfarin therapy.
The findings of the study suggest that there is a need for improving healthcare professionals' use of, and access to IT-based warfarin education resources for patients. The study findings also suggest addressing the concerns raised by the healthcare professionals when implementing such IT resources successfully to help educate patients about their warfarin therapy.

    ABSTRACT: European citizens are increasingly being offered Internet health services. This study investigated patterns of health-related Internet use, its consequences, and citizens' expectations about their doctors' provision of e-health services. Representative samples were obtained from the general populations in Norway, Denmark, Germany, Greece, Poland, Portugal and Latvia. The total sample consisted of 7934 respondents. Interviews were conducted by telephone. 44 % of the total sample, 71 % of the Internet users, had used the Internet for health purposes. Factors that positively affected the use of Internet for health purposes were youth, higher education, white-collar or no paid job, visits to the GP during the past year, long-term illness or disabilities, and a subjective assessment of one's own health as good. Women were the most active health users among those who were online. One in four of the respondents used the Internet to prepare for or follow up doctors' appointments. Feeling reassured after using the Internet for health purposes was twice as common as experiencing anxieties. When choosing a new doctor, more than a third of the sample rated the provision of e-health services as important. The users of Internet health services differ from the general population when it comes to health and demographic variables. The most common way to use the Internet in health matters is to read information, second comes using the net to decide whether to see a doctor and to prepare for and follow up on doctors' appointments. Hence, health-related use of the Internet does affect patients' use of other health services, but it would appear to supplement rather than to replace other health services.

    ABSTRACT: The authors analyzed 1,200 physician responses to a Deloitte Research/Fulcrum Analytics survey of office-based physician use of the Internet and other information technology (IT). Overall, the results suggest that 40 to 50 percent of all respondents are using, or are ready to use, IT for substantial clinical care. However, time and liability concerns about patient e-mail were pervasive across all IT user categories. The results also indicate that some public/private policies aimed at increasing physician IT use for clinical management should be tailored to specific segments of the physician IT user spectrum, rather than using a "one-size-fits-all" policy approach.

    ABSTRACT: Given the increased popularity of complementary and alternative medicine (CAM) and traditional Chinese medicine (TCM), this study aimed to evaluate the effects of interactive media on self-directed learning in patients' knowledge of diabetes and related CAM/TCM strategies, the ability to control blood sugar levels, and self-care in people with diabetes. An experimental study design was used. The experimental group received patient education through interactive multimedia about diabetes for 3 months, while the control group received a routine 3-month patient education. On completion, 60 subjects from Taiwan were evaluated-30 in an experimental group and 30 in a control group. Data were collected from both groups at baseline and at the completion of the patient education. The findings were then compared to evaluate the effects of the intervention on the subjects' knowledge of diabetes, blood sugar control, and self-care. The experimental group showed greater improvement in understanding diabetes than the control (t=3.29, p<.001). There was no significant difference in control of blood sugar levels (t=-1.72, p=.10) and self-care (F=1.03, p=.32). The use of an interactive multimedia device to intervene in diabetes self-care was effective only in raising the subjects' knowledge about the disease. Additionally, the subjects may need more time to implement more effective blood sugar control and self-care activities after receiving instruction.
