Management of congenital nasolacrimal duct obstruction

To present the success rate in treating congenital nasolacrimal duct obstruction with hydrostatic pressure (Crigler method).
Clinical data from 186 children aged up to 32 months treated from 1990 until 2005 were reviewed retrospectively. The procedure was repeated up to 3 times at a one week interval. Using EPI INFO version 6.04 software for analysis, data were tabulated and statistical significance was evaluated by applying chi-square and Kruskal-Wallis tests.
Leading age group was 1 to 5 months old (56.4%) for males and females. Affection was bilateral in 36.3%, 34.1% on right side and 29.6% on the left side. All cases had epiphora and 56.5% presented mucopurulent discharge. Cure was achieved with one massage for 43.6%, 23.3% with two, 17.4% with three and, with 4 and 5+ sessions in respectively 5.8% and 9.0%. Those 14 cases who did not respond to massage were treated with probing.
The Crigler method was effective in managing congenital nasolacrimal duct obstruction cases.

    ABSTRACT: A cohort of 4,792 infants was observed in order to determine the incidence and natural history of epiphora during the first year of life. Evidence of defective lacrimal drainage was present in 964 (20%) at some time during the year. 95% became symptomatic during the first month of life. Spontaneous remission occurred throughout the year and 96% had resolved before the age of one. This study provides no evidence to support probing before the age of one year.

    ABSTRACT: PURPOSE: The aim of this study was to evaluate the outflow from the lacrimal excretory system (LES) at birth. METHOD: Two-hundred LES were evaluated by the Milder Test (DDT) and the modified Jones I Test (JIT) in 100 newborns (NBs) from 31 (T1) until 42 (T3) weeks of gestational age. The occurrence of obstruction was related to sex, weight at birth, presence of ocular discharge and the degree of maturity. RESULTS: Both tests were in agreement and showed LES obstruction in 61.5% (DDT) and 78.5% (JIT), respectively, while the LES outflow increased with increasing maturity. CONCLUSION: Evaluation of LES outflow at birth reveals obstruction rates that are much higher than those clinically observed in children with complaints of epiphora.

    ABSTRACT: One hundred thirteen consecutive children seen with congenital nasolacrimal duct obstruction were treated with local massage and topical antibiotic ointment. The obstruction was resolved in 107 patients within eight months of initiation of this form of management. Nearly all of the children were spared a surgical procedure that probably would have been performed if early probing of the nasolacrimal system had been advocated.
