ABSTRACT 3 Modificações dietéticas, reposição hídrica, suplementos alimentares e drogas: comprovação de ação ergogênica e potenciais riscos para a saúde Dietary changes, water replacement, food supplements and drugs: evidence of ergogenic action and potential health risks Artigo originAl INTRODUÇÃO A Sociedade Brasileira de Medicina do Exercício e do Espor-te (SBME) mostrou preocupação com o crescente consumo de suplementos alimentares e drogas quando editou sua primeira publicação sobre o tema em 2003. Naquela ocasião, contou com colaboradores renomados em suas áreas de atuação que produzi-ram um texto que passou a ser referência na elaboração de traba-lhos e matérias sobre o assunto. Este grupo de trabalho, acrescido de nomes não menos importantes, atualizou essas informações, conferiu dogmas e verificou novidades na área sempre tendo a saúde e o bem-estar daqueles que praticam atividade física regular como seu principal objetivo. Nesta revisão, que mantém a estrutura da primeira publicação, constatamos que mudanças ocorreram, embora o consumo des-sas substâncias ditas ergogênicas continue e seja crescente, muitas vezes são oferecidas como sendo a única resposta ao melhor rendimento nas diversas modalidades e práticas esportivas. Devemos saudar o maior envolvimento das autoridades res-ponsáveis pela regulamentação e fiscalização do comércio legal e ilegal destas substâncias em nosso país, ainda tímido, mas vindo ao encontro das necessidades e anseios daqueles que trabalham e pesquisam nas ciências dos esportes. Infelizmente, pudemos também testemunhar o aparecimen-to de novas drogas e métodos, lícitos e ilícitos, que interferem no rendimento e são utilizados principalmente por atletas de alto rendimento, não obstante seu consumo ocorra também em academias, clubes e escolas de esportes, nas mais variadas mo-dalidades, além da crescente utilização de substâncias proibidas, no uso e fabricação, mesmo sem interferir diretamente no de-sempenho esportivo. Não restam dúvidas quanto às mudanças favoráveis da com-posição corporal e a influencia positiva sobre o desempenho esportivo de atletas após o manejo dietético, através do uso da suplementação alimentar para casos específicos. A alimentação saudável e adequada à quantidade de trabalho deve ser entendida e compreendida pelos atletas de alto rendi-mento como sendo o ponto de partida para obter o desempenho máximo e as manipulações nutricionais caracterizam uma estra-tégia complementar. As orientações que constam nesta seção destinam-se a atletas saudáveis, adultos e adolescentes em fase de maturação sexual final. Para os indivíduos que praticam exercícios físicos sem maio-res preocupações com o desempenho, uma dieta balanceada, que atenda às recomendações dadas à população em geral, é suficiente para a manutenção da saúde e possibilitar bom de-sempenho físico.

    ABSTRACT: The purpose of this investigation was to determine the effects of postexercise eucaloric carbohydrate-protein feedings on muscle glycogen restoration after an exhaustive cycle ergometer exercise bout. Seven male collegiate cyclists [age = 25.6 +/- 1.3 yr, height = 180.9 +/- 3.2 cm, wt = 75.4 +/- 4.0 kg, peak oxygen uptake (VO(2 peak)) = 4.20 +/- 0.2 l/min] performed three trials, each separated by 1 wk: 1) 100% alpha-D-glucose [carbohydrate (CHO)], 2) 70% carbohydrate-20% protein (PRO)-10% fat, and 3) 86% carbohydrate-14% amino acid (AA). All feedings were eucaloric, based on 1.0 g. kg body wt(-1). h(-1) of CHO, and administered every 30 min during a 4-h muscle glycogen restoration period in an 18% wt/vol solution. Muscle biopsies were obtained immediately and 4 h after exercise. Blood samples were drawn immediately after the exercise bout and every 0.5 h for 4 h during the restoration period. Increases in muscle glycogen concentrations for the three feedings (CHO, CHO-PRO, CHO-AA) were 118 mmol/kg dry wt; however, no differences among the feedings were apparent. The serum glucose and insulin responses did not differ throughout the restoration period among the three feedings. These results suggest that muscle glycogen restoration does not appear to be enhanced with the addition of proteins or amino acids to an eucaloric CHO feeding after exhaustive cycle exercise.

    ABSTRACT: When talented, motivated and highly trained athletes meet for competition the margin between victory and defeat is usually small. When everything else is equal, nutrition can make the difference between winning and losing. Although the primary concern of many athletes is to supplement the diet with protein, vitamins and minerals, and a range of more exotic compounds, key dietary issues are often neglected. Athletes must establish their nutritional goals, and must also be able to translate them into dietary strategies that will meet these goals. Athletes are often concerned with dietary manipulations in the period around competition, but the main role of nutrition may be to support consistent intensive training which will lead to improved performance. Meeting energy demand and maintaining body mass and body fat at appropriate levels are key goals. An adequate intake of carbohydrate is crucial for maintaining muscle glycogen stores during hard training, but the types of food and the timing of intake are also important. Protein ingestion may stimulate muscle protein synthesis in the post-exercise period, promoting the process of adaptation in the muscles. Restoration of fluid and electrolyte balance after exercise is essential. If energy intake is high and a varied diet is consumed, supplementation of the diet with vitamins and minerals is not warranted, unless a specific deficiency is identified. Specific strategies before competition may be necessary, but this requirement depends on the demands of the sport. Generally, it is important to ensure high pre-competition glycogen stores and to maintain fluid balance. There is limited evidence to support the use of dietary supplements, but some, including perhaps creatine and caffeine, may be beneficial.

    ABSTRACT: The consumption of oral creatine monohydrate has become increasingly common among professional and amateur athletes. Despite numerous publications on the ergogenic effects of this naturally occurring substance, there is little information on the possible adverse effects of this supplement. The objectives of this review are to identify the scientific facts and contrast them with reports in the news media, which have repeatedly emphasised the health risks of creatine supplementation and do not hesitate to draw broad conclusions from individual case reports. Exogenous creatine supplements are often consumed by athletes in amounts of up to 20 g/day for a few days, followed by 1 to 10 g/day for weeks, months and even years. Usually, consumers do not report any adverse effects, but body mass increases. There are few reports that creatine supplementation has protective effects in heart, muscle and neurological diseases. Gastrointestinal disturbances and muscle cramps have been reported occasionally in healthy individuals, but the effects are anecdotal. Liver and kidney dysfunction have also been suggested on the basis of small changes in markers of organ function and of occasional case reports, but well controlled studies on the adverse effects of exogenous creatine supplementation are almost nonexistent. We have investigated liver changes during medium term (4 weeks) creatine supplementation in young athletes. None showed any evidence of dysfunction on the basis of serum enzymes and urea production. Short term (5 days), medium term (9 weeks) and long term (up to 5 years) oral creatine supplementation has been studied in small cohorts of athletes whose kidney function was monitored by clearance methods and urine protein excretion rate. We did not find any adverse effects on renal function. The present review is not intended to reach conclusions on the effect of creatine supplementation on sport performance, but we believe that there is no evidence for deleterious effects in healthy individuals. Nevertheless, idiosyncratic effects may occur when large amounts of an exogenous substance containing an amino group are consumed, with the consequent increased load on the liver and kidneys. Regular monitoring is compulsory to avoid any abnormal reactions during oral creatine supplementation.
