Evaluación de las concentraciones de lípidos y apoproteínas A-I y B-100 en un grupo de escolares de cinco departamentos del centro-oriente de Colombia.

ABSTRACT Introducción. Existe evidencia sobre variaciones en las concentraciones de lípidos y
apoproteínas en los niños asociados a cambios fisiológicos o al medio ambiente, este último
puede inducir alteraciones futuras en los lípidos.
Objetivo. Evaluar las concentraciones de colesterol total, colesterol LDL, colesterol HDL y
apoproteínas A-1 y B-100 en escolares de cinco departamentos del centro-oriente colombiano,
y comparar por edad, sexo, índice de masa corporal, práctica de actividad física y preferencias
hacia algunos alimentos.
Materiales y métodos Se recolectaron 741 muestras de sangre que se analizaron por métodos
enzimáticos, colorimétricos e inmunoturbidimétricos. Todos los valores se presentaron como
media y desviación estándar y para la comparación entre grupos se utilizó prueba t, ANOVA y
Resultados. Se observaron variaciones en los lípidos y valores constantes en las
apolipoproteínas, las concentraciones de los lípidos se relacionaron con los cambios de edad
y sexo. Se observaron cambios que pueden asociarse al grado de maduración sexual, dieta y
nivel de actividad física. Los valores de colesterol total, colesterol LDL y colesterol HDL fueron
más bajos a los reportados en otros grupos de población mientras que en los triglicéridos hay
mayor variación con tendencia a ser superiores.
Conclusión. Existen otras variables diferentes a la edad y el género que influyen en las
variaciones de los lípidos; se requieren otros estudios para determinar la contribución de cada
factor. La promoción de estilos de vida saludable puede ayudar a disminuir el riesgo futuro de
enfermedad cardiovascular asociada a bajas concentraciones de colesterol HDL y mayores
de triglicéridos en esta población.

    ABSTRACT: Major new public health problems occur in developing countries as they become more affluent and change their traditional dietary patterns. To study this phenomenon in microcosm, we substituted an "affluent" diet for the traditional diet of a group of Tarahumara Indians, a Mexican people known to consume a low-fat, high-fiber diet and to have a very low incidence of risk factors for coronary heart disease. Thirteen Tarahumara Indians (five women and eight men [including one adolescent]) consumed their traditional diet (2700 kcal per day) for one week, and were then fed a diet typical of affluent societies, which contained excessive calories (4100 kcal per day), total fat, saturated fat, and cholesterol, for five weeks. After five weeks of consuming the affluent diet, the subjects' mean (+/- SE) plasma cholesterol level increased by 31 percent, from 121 +/- 5 to 159 +/- 6 mg per deciliter (3.13 +/- 0.13 to 4.11 +/- 0.16 mmol per liter, P less than 0.001). The increase in the plasma cholesterol level was primarily in the low-density lipoprotein (LDL) fraction, which rose 39 percent, from 72 +/- 3 to 100 +/- 4 mg per deciliter (1.86 +/- 0.08 to 2.59 +/- 0.10 mmol per liter, P less than 0.001). High-density lipoprotein (HDL) cholesterol, usually low in this population, increased by 31 percent, from 32 +/- 2 to 42 +/- 3 mg per deciliter (0.83 +/- 0.05 to 1.09 +/- 0.08 mmol per liter). Consequently, the ratio of LDL to HDL levels changed little (2.25 with the base-line diet and 2.38 with the affluent diet). Plasma triglyceride levels increased by 18 percent, from 91 +/- 8 to 108 +/- 11 mg per deciliter (1.03 +/- 0.09 to 1.22 +/- 0.12 mmol per liter, P less than 0.05), with a significant increase in the very-low-density lipoprotein triglyceride fraction. All the subjects gained weight, with a mean increase of 3.8 kg (7 percent). When Tarahumara Indians from a population with virtually no coronary risk factors consumed for a short time a hypercaloric diet typical of a more affluent society, they had dramatic increases in plasma lipid and lipoprotein levels and body weight. If sustained, such changes might increase their risk of coronary heart disease.

    ABSTRACT: Background.Atherosclerosis begins in childhood and progresses into adulthood. The reduction of cardiovascular risk factors, such as elevated serum total cholesterol and low-density lipoprotein cholesterol (LDL-C) levels, in childhood may reduce cardiovascular morbidity and mortality in adulthood. Lipid distributions among children and adolescents were examined using the most recent nationally representative data.Methods.Data from 7,499 examinees in NHANES III (1988–1994) were used to estimate mean and percentile distributions of serum total cholesterol, LDL-C, high-density lipoprotein cholesterol (HDL-C), and triglycerides in children and adolescents aged 4 to 19 years. The estimates were analyzed by age, sex, and race/ethnic groups. Trends in mean total cholesterol were examined for 12- to 17-year-olds using data from NHES III (1966–1970), NHANES I (1971–1974), and NHANES III (1988–1994).Results.For children and adolescents 4 to 19 years of age, the 95th percentile for serum total cholesterol was 216 mg/dL and the 75th percentile was 181 mg/dL. Mean age-specific total cholesterol levels peaked at 171 mg/dL at 9–11 years of age and fell thereafter. Females had significantly higher mean total cholesterol and LDL-C levels than did males (P< 0.005). Non-Hispanic black children and adolescents had significantly higher mean total cholesterol, LDL-C, and HDL-C levels compared to non-Hispanic white and Mexican American children and adolescents. The mean total cholesterol level among 12- to 17-year-olds decreased by 7 mg/dL from 1966–1970 to 1988–1994 and is consistent with, but less than, observed trends in adults. Black females have experienced the smallest decline between surveys.Conclusions.The findings provide a picture of the lipid distribution among U.S. children and adolescents and indicate that, like adults, adolescents have experienced a fall in total cholesterol levels. Total cholesterol levels in U.S. adolescents declined from the late 1960s to the early 1990s by an average of 7 mg/dL. This information is useful for planning programs targeting the prevention of cardiovascular disease beginning with the development of heathy lifestyles in childhood.

    ABSTRACT: The aim of this study was to investigate the association between participation in vigorous physical activities (VPA) and indicators of adiposity and fat distribution in male adolescents. Subjects were classified on the basis of the number of reported 15-min periods over 3 d during which VPA were performed. Body weight, fat mass, body mass index, sum of six subcutaneous skinfolds, trunk and extremity skinfolds, and trunk to extremity skinfolds ratio (T/E ratio) were all significantly and inversely related to a higher VPA participation. Moreover, T/E ratio was significantly lower in the subgroup reporting a mean of 7.8 15-min periods of activity for 3 d (equivalent to a mean of 39 min x d(-1)) compared with subjects reporting no participation. This decrease in T/E ratio was more pronounced in subjects reporting a mean of 13.6 15-min periods of VPA for 3 d. These observations reinforce the idea that excess abdominal fat accumulation can be prevented by regular participation in vigorous physical activities.
