Transmission of Hepatitis B Virus From an Orthopedic Surgeon With a High Viral Load

During the evaluation of a needle-stick injury, an orthopedic surgeon was found to be unknowingly infected with hepatitis B virus (HBV) (viral load >17.9 million IU/mL). He had previously completed two 3-dose series of hepatitis B vaccine without achieving a protective level of surface antibody. We investigated whether any surgical patients had acquired HBV infection while under his care.

A retrospective cohort study of all patients who underwent surgery by the surgeon was conducted. Patients were notified of their potential exposure and need for testing, and samples with positive HBV loads underwent DNA sequencing. Characteristics of the surgical procedures for the cohort were evaluated.

A total of 232 (70.7%) of potentially exposed patients consented to testing; 2 were found to have acute infection and 6 had possible transmission (evidence of past exposure without risk factors). Genome sequence analysis of HBV DNA from the infected surgeon and patients with acute infection revealed genetically related virus (>99.9% nucleotide identity). Only age was found to be statistically different between those with confirmed or possible HBV transmission and those who remained susceptible to HBV.

We documented HBV transmission during orthopedic surgery to 2 patients from a surgeon with HBV. This investigation highlights the importance of evaluating individuals who do not respond to 2 series of HBV vaccination, the increased risk of HBV transmission from providers with high viral loads, and the need to evaluate the clinical practice of providers with HBV and implement appropriate procedure-based practice restrictions.

    ABSTRACT: Healthcare personnel (HCP) are at risk for exposure to and transmission of potentially life-threatening vaccine preventable diseases to patients and colleagues. The Centers for Disease Control and Advisory Committee on Immunization Practices (ACIP) recommend routine influenza immunization and maintenance of immunity to hepatitis B and pertussis, among others. In this article, we aim to review recently approved influenza vaccines, as well as address some of the issues regarding hepatitis B and pertussis vaccinations in HCP. Several new formulations of influenza vaccines are now available, including quadrivalent vaccines and non-egg-based vaccines; their use in HCP requires further study. An alarming rise in pertussis rates has led to a revision of ACIP guidelines recommending vaccination for women during each pregnancy. Persistent lack of immunity to hepatitis B after vaccine series remains a problem for many HCP. Inactivated trivalent influenza vaccines remain the safest and most widely studied influenza vaccinations for healthcare workers. A pertussis booster in the form of Tdap is now recommended for most HCP. More studies are needed regarding the issue of nonresponders in HCP who receive the three-dose hepatitis B vaccine series, as there are some promising strategies available that may boost immune responses.

    ABSTRACT: Issues regarding the transmission of blood-borne viruses from infected medical personnel to patients are controversial to both parties. The aim of this study was to evaluate the attitudes towards disclosure of HBV/HCV/HIV-infected surgeons and the possibility of being forced to give up surgical procedures, as well as to assess the prevalence of anti-HBc total, HBsAg, anti-HCV and anti-HIV in surgical staff. Using an anonymous questionnaire a cross-sectional sero-survey was conducted among surgeons and nurses of surgical wards in 16 randomly selected hospitals, Western Pomerania, Poland, from January to June 2009. Serum samples were tested for anti-HBc, HBsAg, anti-HCV and anti-HIV by ELISA tests. In the group of 427 participants (232 nurses, 65 doctors; a median age: 42 years), anti-HBc was found in 16.6%, HBsAg in 0.7%, anti-HCV in 1.4% and anti-HIV in 0%. The risk of a single exposure to HBV was correctly defined by 26.5% of participants, to HCV by 19.7%, to HIV by 18.7%. 16.2% participants stated that infected surgeons should disclose their HBV, HCV, or HIV serostatus, 39.8% and 42.6% that those HBV/HCV-infected and HIV-infected, respectively, should discontinue practicing surgery. Participants who correctly assessed the risk of contracting HIV/HBV/HCV after a single exposure were significantly (p = 0.0001; p = 0.03; p = 0.01, respectively) less likely to favor infected staff being forced to discontinue surgical procedures. A fraction of surgical staff showed detectable markers of HBV/HCV infection, they may be a source of infection for operated patients. Surgical staff's knowledge about occupational blood exposure risk was not satisfactory, which might have influenced the restrictive attitudes to force those infected with HBV/HCV/HIV to give up surgical procedures and a willingness to disclose their serological status.
