Clinical comparison of gluma and Er:YAG laser treatment of cervically exposed hypersensitive dentin.

ABSTRACT To compare the desensitizing effects of a glutaraldehyde-based desensitizing system (Gluma) and an Er:YAG laser treatment on cervically exposed hypersensitive dentin.
A total of 22 subjects (mean age: 39 +/- 13.7 years; 15 females, 7 males) suffering from cervical dentin hypersensitivity was included in a prospective, split-mouth clinical study. The teeth were treated on one side of the mouth with the glutaraldehyde-based desensitizing system and on the other side with the Er:YAG laser. Sensitivity perception was recorded before treatment (baseline), during and immediately after treatment, after 1 week, 1 month, 3 months and 6 months. The subjects were asked to rate the sensitivity experienced during air stimulation by placing a mark on a visual analogue scale (VAS).
Both techniques showed an effective reduction of cervical dentin hypersensitivity. The subjects experienced equal improvements compared to their status before and 6 months after treatment with both methods (P<0.001).

    ABSTRACT: Background and Objectives During laser vaporization of benign prostate hyperplasia (BPH), high precision of optical fiber handling is pivotal to minimize any post-operative complications. The aim of the study was to evaluate the feasible applications of a bundled fiber to treat BPH by directionally and selectively manipulating laser light onto the targeted tissue.MethodsA bundled optical fiber, consisting of four side-firing fibers, was fabricated to selectively emit laser beams in from one to four directions. Both transmission efficiency and light distribution were qualitatively and quantitatively characterized on the bundled fiber. In terms of interstitial application of the proposed fiber with 1064 nm on porcine liver tissue, the extent of thermal denaturation was estimated and compared at various laser parameterizations and for different directions of light.ResultsFrom the laser source to the fiber tip, the fabricated fiber device demonstrated a total light transmission of 52%. Due to internal light reflection, a secondary beam was emitted backward from the fiber tip and was responsible for 25% of the transmission loss. According to tissue testing, the extent of tissue denaturation generally increased with laser power, irradiation time, and number of light directions. The geometrical shape of thermal coagulation correlated well with the direction of light emission. Thermal damage to the glass tube occurred during excessive heat accumulation generated by continuous irradiation.Conclusions The proposed fiber can be beneficial for laser vaporization of BPH by providing a selective light direction irradiation along with minimal thermal damage.     ABSTRACT: Erbium:yttrium-aluminum-garnet (Er:YAG) laser preparation of tooth cavities for restoration is an increasingly popular method, but its compatibility with existing composite material bonding protocols has not been fully defined. This study evaluated the effect of laser and etchant pretreatments on the performance of one-bottle self-etch adhesives in Er:YAG laser-prepared dentin. Eight groups of 20 extracted teeth were established to investigate bonding in tested dentin disks. Various combinations of laser preparation (with/without), pretreatment (none/acid-etch/low-fluence Er:YAG irradiation), and self-etching adhesive (G-Bond Plus or Xeno V) were tested. Samples were then restored with composite resin and subjected to a tensile bond strength (TBS) test. We also performed scanning electron microscopy (SEM) on dentin disks from some of these groups before and after adhesive application to evaluate their microscopic morphological appearance. Statistical analysis (Dunnett T3 test coupled with the general linear model at 5 % significance level) revealed that the laser preparation of dentin did not impact on TBS (p = 0.914), whereas pretreatment with either phosphoric acid (p < 0.0001) or low-fluence Er:YAG laser irradiation (p < 0.0001) significantly increased TBS, although there was no difference between them in their respective elevation of TBS. SEM analysis demonstrated that both acid and laser pretreatments reduced irregularities and produced a more homogeneous surface. Er:YAG laser preparation does not compromise the efficacy of one-step self-etch dentin adhesives, and pretreatment with phosphoric acid or low-fluence Er:YAG laser can significantly increase the TBS of adhesion to this irradiated dentin.

    ABSTRACT: To evaluate the desensitizing efficacy of 2% sodium fluoride solution (NaF), diode laser (DL), a DL and NaF association and a solution of hydroxyl-ethyl-methacrylate and glutaraldehyde (HEMA-G: Gluma desensitizer) in cervical dentin hypersensitivity (CDH). 262 teeth of 24 subjects (16 females and eight males; age 21 to 64 years, mean 38 years), each having at least two CHD teeth for each quadrant, were included in this prospective, split mouth, clinical study. Teeth of each oral quadrant were randomized in four groups (SG) to study the effectiveness of NaF (SG-1), of DL (SG-2) NaF-DL combination (SG-3) and HEMA-G (SG-4). The subjects were asked to rate the sensitivity experienced during air stimulation by placing a mark on a visual analogue scale (VAS) before treatment (baseline), immediately after treatment, and after 1, and 6 months. The outcomes showed a significant reduction of discomfort compared to baseline values for teeth of SG-3 immediately post treatment (82.6%) (P < 0.001), after 1 month (69.5%) (P < 0.001) and after 6 months (60.8%) (P < 0.001), respectively, compared with the reduction scores of 51.6% (P < 0.001), 29.7% (P < 0.05) and 4.7% (P > 0.05), recorded for SG-1; 72.2%, (P < 0.001), 62.5% (P < 0.001), and 47.2% (P < 0.05), recorded for SG-2; 77.4% (P < 0.001), 56.1% (P < 0.001), and 27.3% (P < 0.05), recorded for SG-4.
