Contact mechanics and three-dimensional alignment of normal dog elbows.

ABSTRACT To evaluate the effects of antebrachial rotation at 3 elbow flexion angles on contact mechanics and 3-dimensional (3D) alignment of normal dog elbows.
Ex vivo biomechanical study.
Unpaired thoracic limbs from 18 dogs (mean ± SD weight, 27 ± 4 kg).
With the limb under 200 N axial load, digital pressure sensors measured contact area (CA), mean contact pressure (MCP), peak contact pressure (PCP), and PCP location in the medial and lateral elbow compartments, and 3D static poses of the elbow were obtained. Each specimen was tested at 115°, 135°, and 155° elbow flexion, with the antebrachium in a neutral position, in 28° supination, and in 16° pronation. Repeated measure ANOVAs with post-hoc Bonferroni (P ≤ .0167) were performed.
Both pronation and supination decreased CA by 16% and 8% and increased PCP by 5% and 10% in the medial and lateral compartments, respectively. PCP location moved 2.3 mm (1.8-3.2 mm) closer to the apex of the medial coronoid process in pronation and 2.0 mm (1.8-2.2 mm) farther away in supination. The radial head and medial coronoid process rotated 5.4° and 1.9° internally during pronation and 7.2° and 1.2° externally during supination.

    ABSTRACT: To report thoracic limb alignment values in healthy dogs; to determine if limb alignment values are significantly different when obtained from standing versus recumbent radiographic projections. Prospective cross-sectional study. Labrador Retrievers (n = 45) >15 months of age. Standing and recumbent radiographs were obtained and limb montages were randomized before analysis by a single investigator blinded to dog, limb, and limb position. Twelve limb alignment values were determined using the CORA methodology. Measurements were performed in triplicate and intra-observer variability was evaluated by intra-class correlation coefficient (ICC). Limb alignment values were reported as mean ± SD and 95% confidence intervals. Linear mixed models were used to determine if significant associations existed between limb alignment values and limb, limb position, gender, age, weight, and body condition score. There were significant differences in standing and recumbent limb alignment values for all values except elbow mechanical axis deviation (eMAD). Limb, gender, age, body weight, and body condition score had no effect. ICC values ranged from 0.522 to 0.758, indicating moderate to substantial agreement for repeated measurements by a single investigator. Limb alignment values are significantly different when determined from standing versus recumbent radiographs in healthy Labrador Retrievers.

    ABSTRACT: To evaluate subchondral joint space width measurements (SJSW) and CT-osteoabsorptiometry (CTOAM) measurements in the elbow of dogs with naturally occurring fragmented medial coronoid process (FMCP) preoperatively and 6 months postoperatively, and to compare these to normal dogs. Prospective randomized clinical trial. Large breed dogs <1 year of age (n = 15). Arthroscopic FMCP debridement was performed, followed by randomization to a proximal ulnar ostectomy with IM pin (PUO). CTs were performed pre-operatively and 6 months post-operatively. CTOAM and SJSW measurements were acquired in 7 locations within the joint and compared to an age-matched normal control group. Pearson correlation was performed on SJSW and CTOAM measurements. A linear mixed model determined the effect of disease and treatment on SJSW and CTOAM measurements. Fifteen dogs (28 elbows) with FMCP participated (11 arthroscopic FMCP removal, 17 with additional PUO). Data were normally distributed. Pearson correlation between CTOAM and SJSW measurements showed moderate to strong negative correlation in the control dogs. Preoperatively, affected elbows had lower medial compartment and higher lateral coronoid process CTOAM values than normal elbows. After treatment, CTOAM values of the medial compartment increased to normal. Treatment with PUO did not affect SJSW or CTOAM. Our data agree with these previous studies suggesting lower subchondral plate mineralization in dogs affected by FMCP. Arthroscopy may result in higher CTOAM values secondary to increased loading.

    ABSTRACT: To determine (1) the effect of elbow incongruity on contact mechanics and (2) the effect of treatment of this incongruity with 1 of 2 ulnar ostectomies in the canine elbow. Ex vivo biomechanical study. Unpaired cadaveric canine forelimbs (n = 17). In a servohydraulic testing frame, thin-film pressure sensors were placed into the lateral and medial compartments of the elbow. Specimens were tested in 135° of elbow joint flexion at 200 N of cyclic axial force, followed by a 20 seconds hold. Intra-articular contact area (CA), mean contact pressure (mCP) and peak contact pressure (pCP) were measured in each compartment. After radial shortening, testing was repeated and limbs randomized into proximal ulnar ostectomy with IM pin (PUO) or sequential distal ulnar ostectomy (DUO), interosseous ligament release (DUO-L), and ulnar attachment of the abductor pollicis longus muscle and interosseous membrane release (DUO-ML). Paired t-tests were used to compare each treatment to baseline values. Differences between treatment groups were evaluated with a mixed model with random effect to adjust for the clustering of limbs within dog. P < .05 was considered significant. Radial shortening resulted in shift of mCP and pCP from the lateral to the medial compartment. The PUO group resulted in normalization of medial compartment mCP and decrease of pCP, whereas in the DUO group return to baseline was achieved only after DUO-ML. PUO is effective in unloading medial compartment pCP in an incongruent joint.
