Comparison of 2 Hyaluronic Acid Drugs for the Treatment of Temporomandibular Joint Osteoarthritis

To compare the effectiveness of 2 treatment protocols providing 5 weekly temporomandibular joint (TMJ) arthrocenteses immediately followed by injections of 2 different molecular weight hyaluronic acid (HA) drugs to manage symptoms in patients with inflammatory-degenerative TMJ disease.
Patients with a Research Diagnostic Criteria for Temporomandibular Disorders diagnosis of osteoarthritis were randomly assigned to 1 of 2 study groups receiving either low- or medium-molecular weight HA after arthrocentesis. The level of maximum pain at chewing was the primary outcome variable, and maximum pain at rest, subjective chewing efficiency, functional limitation, treatment tolerability, perceived treatment effectiveness, and jaw range-of-motion function in millimeters were the secondary outcomes. All variables were assessed and compared between groups at baseline, at the end of treatment, and 3 months later.
Forty subjects entered the study. At the end of the follow-up period, all the outcome variables improved in both groups of patients. A between-group comparison of changes over time showed that differences were not significant for any of the outcome variables, that is, pain at chewing (F = 0.056, P = .815), pain at rest (F = 0.383, P = .541), chewing efficiency (F = 0.050, P = .825), functional limitation (F = 0.268, P = .609), and mouth opening (F = 0.003, P = .954). In addition, no between-group differences were shown for perceived treatment effectiveness and treatment tolerability.
Similar positive effectiveness was shown for 2 treatment protocols for TMJ osteoarthritis (ie, 5-session single-needle arthrocentesis plus low- or medium-molecular weight HA).

    ABSTRACT: JUSTIFICATIVA E OBJETIVOS: Realizar uma revisão a cerca das diferentes técnicas de artrocentese empregadas nas disfunções artrogênicas da articulação temporomandibular (ATM), bem como das suas indicações, seu possível mecanismo de ação e complicações. CONTEÚDO: Pesquisas têm sido realizadas, nesses últimos anos, na tentativa de padronizar diferentes protocolos de artrocentese empregadas nas disfunções temporomandibulares (DTM), no intuito de definir quando e como empregá-las. Essas técnicas são pouco invasivas, de baixo custo, podendo ser realizadas sob anestesia local, com ou sem sedação, replicáveis e de baixa morbidade. São indicadas para aliviar ou eliminar o quadro de dor e disfunção artrogênica presente.CONCLUSÃO: Várias técnicas de artrocentese combinadas ou não com anti-inflamatórios, opioides, substâncias viscoelásticas, produzem resultados adequados nas DTM artrogênicas, porém o sucesso terapêutico depende da cronicidade da doença, de suas características clínicas e de imagem, do diagnóstico correto, da colaboração do paciente, da experiência do profissional e da técnica empregada.

    ABSTRACT: Data sourcesThe Cochrane Oral Health Group Trials Register, Cochrane Central Register of Controlled Trials (CENTRAL), Medline, Embase and PEDro databases were searched with no language restrictions. Handsearching of a number of relevant dental journals and the reference lists of identified articles was also conducted.Study selectionRandomised controlled trials (RCTs) comparing any form of non-surgical or surgical therapy for TMJ OA in adults were included. Only studies with clinical and/or radiological diagnosis of TMJ OA according to the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) guideline or compatible criteria were considered. Pain/tenderness/discomfort in the TMJs or jaw muscles, self assessed range of mandibular movement and TMJ sounds were the primary outcomes considered. Secondary outcomes included the measurement of quality of life or patient satisfaction, morphological changes of the TMJ and TMJ soundsData extraction and synthesisStudy selection, data abstraction and risk of bias assessment were carried out independently by two reviewers. Meta-analysis was not conducted owing to clinical heterogeneity.ResultsAlthough three RCTs were included in this review, pooling of data in a meta-analysis was not possible due to wide clinical diversity between the studies. The reports indicate a not dissimilar degree of effectiveness with intra-articular injections consisting of either sodium hyaluronate or corticosteroid preparations, and an equivalent pain reduction with diclofenac sodium as compared with occlusal splints. Glucosamine appeared to be just as effective as ibuprofen for the management of TMJ OA.Conclusions In view of the paucity of high level evidence for the effectiveness of interventions for the management of TMJ OA, small parallel group RCTs which include participants with a clear diagnosis of TMJ OA should be encouraged and especially studies evaluating some of the possible surgical interventions.

    ABSTRACT: The temporomandibular joint (TMJ) consists in the glenoid fossa arising from the otic capsule through intramembranous ossification, the fibrocartilaginous disc and the condyle, which is derived from the secondary cartilage by endochondral ossification. We have reported previously that cranial neural-crest-specific inactivation of the homeobox gene Shox2, which is expressed in the mesenchymal cells of the maxilla-mandibular junction and later in the progenitor cells and perichondrium of the developing chondyle, leads to dysplasia and ankylosis of the TMJ and that replacement of the mouse Shox2 with the human SHOX gene rescues the dysplastic and ankylosis phenotypes but results in a prematurely worn out articular disc. In this study, we investigate the molecular and cellular bases for the prematurely worn out articular disc in the TMJ of mice carrying the human SHOX replacement allele in the Shox2 locus (termed Shox2 (SHOX-KI/KI)). We find that the developmental process and expression of several key genes in the TMJ of Shox2 (SHOX-KI/KI) mice are similar to that of controls. However, the disc of the Shox2 (SHOX-KI/KI) TMJ exhibits a reduced level of Collagen I and Aggrecan, accompanied by increased activities of matrix metalloproteinases and a down-regulation of Ihh expression. Dramatically increased cell apoptosis in the disc was also observed. These combinatory cellular and molecular defects appear to contribute to the observed disc phenotype, suggesting that, although human SHOX can exert similar functions to mouse Shox2 in regulating early TMJ development, it apparently has a distinct function in the regulation of those molecules that are involved in tissue homeostasis.
