This article will systematically review radiographic assessment of hip arthroplasty including classifications based on different types and techniques of hip arthroplasty, terminology for prosthetic designs and materials, surgical techniques, and initial and follow-up radiographic assessments.

Assessment of postoperative hip arthroplasty radiographs is extremely important. It is well known that patients with complications may be asymptomatic, and for this reason, routine radiographic follow-up is recommended for all patients with hip arthroplasty. The foundation of radiologic interpretation of hip arthroplasty is knowledge of the normal appearance of the many different types of prostheses. A standard approach to radiologic reporting should be undertaken.

    ABSTRACT: Aim: The clinical utility of digital linear tomosynthesis in musculoskeletal applications has been validated in only a few reports. Technical performance and utility in hip prosthesis imaging have been discussed in technical reports, but no clinical evaluation has been reported. The purpose of the current study was to assess the added clinical utility of digital linear tomosynthesis compared to radiography in loosening of total hip joint arthroplasty. Materials and methods: In a prospective study, radiography and digital tomosynthesis were performed in 40 consecutive patients with total hip arthroplasty referred for suspect prosthesis loosening. Tomosynthesis images were compared to anterior-posterior (AP) and cross-table lateral radiographs regarding demarcation and extent of demineralization and osteolysis. Further noted were skeletal fractures, cement fractures, fragmentation, and artifacts interfering with the diagnosis. Results: Tomosynthesis was superior to radiography with sharper delineation of demineralization and osteolysis in the AP projection. A limitation was the inability to generate lateral tomosynthesis images, with inferior assessment of the area anterior and posterior to the acetabular cup compared to cross-table radiographs. Artifacts interfering with diagnosis were found in one hip. Conclusion: Tomosynthesis improved evaluation of total hip arthroplasty in the AP projection but was limited by the lack of lateral projections.

    ABSTRACT: Objective: This article reviews the imaging features of revisions and complications of hip replacement arthroplasty and relates these features to the current understanding about how and why these failures occur. Conclusion: Short-term failures of hip replacements are most commonly the result of instability and dislocation. Complications ranging from osteolysis caused by granulomatous reaction to particulate wear debris lead to many long-term failures. Attempts to reduce wear debris through changes in design and materials have reduced the rate of some complications but have resulted in new ones. Infection remains a devastating complication that is difficult to resolve.

    ABSTRACT: There is no consensus among hip surgeons in Denmark on how to follow up patients after total hip arthroplasty (THA). Agreement on the need for radiographic examinations is also lacking. The purpose of this study was to evaluate if routine outpatient post-operative radiographs, obtained three and 12 months after uncomplicated cementless primary THA influenced patient treatment. A retrospective chart review was performed for 249 cases who had received THA during a four-month period. Patient data, indication for surgery and type of prosthesis were noted. The radiographic descriptions in the medical record made by the operating surgeon at the three- and 12-month outpatient follow-up visits were examined. At three months, the radiograph showed subsidence ranging from "barely detectable" to ten millimetres in eight of 216 cases. One patient was treated with crutches. The remaining three patients were given another follow-up visit. At 12 months, two cases had signs of stress shielding with cortical thickening. This had no consequence in one patient and the other was given additional follow-up. We conclude that routine radiographs can be omitted from the outpatient follow-up within the first year following primary elective cementless THA as it does not affect treatment. We believe radiographs should be reserved for patients with subjective complaints such as pain and to those who are referred outside the routine follow-up plan due to complications. not relevant. not relevant.
