ABSTRACT The dialysis disequilibrium syndrome is a rare but serious complication of hemodialysis. Despite the fact that maintenance hemodialysis has been a routine procedure for over 50 years, this syndrome remains poorly understood. The signs and symptoms vary widely from restlessness and headache to coma and death. While cerebral edema and increased intracranial pressure are the primary contributing factors to this syndrome and are the target of therapy, the precise mechanisms for their development remain elusive. Treatment of this syndrome once it has developed is rarely successful. Thus, measures to avoid its development are crucial. In this review, we will examine the pathophysiology of this syndrome and discuss the factors to consider in avoiding its development.

    ABSTRACT: We aim to raise awareness for the potential for rapid brain edema and herniation in acutely brain-injured patients undergoing renal replacement therapy (RRT), including one case undergoing continuous veno-venous hemofiltration. Dialysis disequilibrium syndrome (DDS) may have been a possible cause for the brain edema.

    ABSTRACT: Aim: The study aimed to evaluate verbal memory performance in end stage renal disease (ESRD) patients on maintenance haemodialysis (HD) and determine the relationship between the socio-demographic, clinical and biochemical characteristics of patients with verbal memory performance. Methods: A cross-sectional study of 91 ESRD patients on HD from December 2012 to May 2013 was conducted in nephrology unit, Hospital Sultanah Bahiyah and 3 private dialysis centres in Alor Setar, Kedah during the study period. Verbal memory performance and depression were assessed using the Malay version of the Auditory Verbal Learning Test (MVAVLT) and the Beck Depression Inventory (BDI) respectively during the first hour of HD procedures. Results: Older age, male gender and none/primary education were independently associated with worse verbal memory performance. In contrast, longer duration of HD and anti-diabetic treatment were associated with better verbal memory performance. Other factors, including depression, were not significantly associated with verbal memory performance. Conclusion: Anti-diabetic and HD treatment are associated with better cognitive function. Hence, this study suggests the importance of good diabetic control and regular maintenance HD to prevent cognitive impairment in ESRD patients.

    ABSTRACT: Children with chronic kidney disease stage 5 requiring dialysis can be treated by peritoneal or hemodialysis. In the United Kingdom nearly twice as many children receive peritoneal dialysis compared with hemodialysis. Technical aspects of pediatric hemodialysis are challenging and include the relative size of extracorporeal circuit and child's blood volume, assessment of adequacy,technical and complications of vascular access. Alternatives to standard hospital-based hemodialysis are also increasingly available. Optimizing nutritional status with the support of specialist pediatric dietitians is key to the management of children receiving hemodialysis. The effects of chronic illness on growth and school achievement, as well as the psychological, emotional, and social development of the child should not be underestimated. This review focuses on the above elements and highlights common pediatric practice in the United Kingdom.
