Hautulkus bei Histoplasmose und HIV-Infektion

Abstract

Ein 33-jähriger Brasilianer stellte sich mit einem schmerzhaften Ulkus über der Glabella vor. Der Patient war HIV-positiv.
Mittels dermatohistopathologischer Untersuchung konnte die Diagnose einer Histoplasmose gesichert werden. In Lateinamerika
tritt bei HIV-Patienten mit disseminierter Histoplasmose oft eine Hautbeteiligung auf. Wenn die Haut betroffen ist, kann die
klinische Manifestation vielgestaltig sein. Der vorgestellte Fall unterstreicht, wie wichtig es ist, diese Erkrankung in das
Spektrum der Differenzialdiagnosen bei HIV-Patienten aus Endemiegebieten aufzunehmen.
A 33-year-old Brazilian man presented with a painful ulcer on the glabella region. The patient was HIV-positive. Diagnosis
of histoplasmosis was established by histopathological examination. In Latin America, disseminated histoplasmosis frequently
involves the skin of HIV-positive patients. When affecting the skin, the disease has a wide range of clinical presentations.
Our case highlights the importance of including this disease in the differential diagnosis of cutaneous ulcers in HIV patients
from areas endemic for histoplasmosis.
