Article: Intraoperative donor cholangiography.

ABSTRACT: Biliary drainage has long been called the Achilles' heel of liver transplantation, and biliary complications compromise the success of liver transplantation by increasing graft loss and the rates of a required second operation, morbidity, and mortality. One cause of complications is unrecognized anomalous biliary anatomy. We examined 73 intraoperative donor duct cholangiograms (IODDCs) to assess our ability to identify biliary anomalies intraoperatively. Normal anatomy was seen in 42% (31/73); some part of the right-sided biliary system drained into the left bile duct in 22% (16/73); trifurcated systems with a single branch point for the right posterior, right anterior, and left ducts appeared in 16% (12/73); low insertion of a right segmental duct to the hepatic duct was seen in 11% (8/73); and drainage of a right segmental duct into the cystic duct or into the hepatic duct at the cystic duct origin was noted in 8% (6/73). It was believed that the last group represented a condition that dictated extra caution in biliary reconstruction. The incidence of radiographic recognition of these anomalies was more than twice the clinical recognition in our patient population, implying that many such "problem" ducts usually go unrecognized. IODDCs facilitate training of transplant fellows. Article: Intrahepatic biliary anatomy of living adult liver donors: correlation of mangafodipir trisodium-enhanced MR cholangiography and intraoperative cholangiography.

ABSTRACT: OBJECTIVE: The purpose of our study was to assess the usefulness of mangafodipir trisodium-enhanced MR cholangiography for evaluating intrahepatic biliary anatomy of adult living liver donors and to correlate the results with intraoperative cholangiography. CONCLUSION: Mangafodipir trisodium-enhanced MR cholangiography accurately shows the biliary anatomy in the livers of donors. Noninvasive preoperative evaluation of the biliary anatomy in donor candidates is important for the detection of common anatomic variants that may require alternative graft-harvesting surgery.American Journal of Roentgenology 12/2002; 179(5):1281-6. · 2.78 Impact Factor -
Article: Variations of intrahepatic and proximal extrahepatic bile ducts.

ABSTRACT: The modern segmental concept of the liver with a segmental bile duct distribution is of practical importance for biliary specialists and should include information on variations of intrahepatic and extrahepatic bile ducts. The aim of this study was to find biliary variations, especially on a segmental and sectorial level, and to try to arrange them into types. We analyzed 51 corrosion casts of the human liver, which enabled the three-dimensional study of extrahepatic, sectorial, segmental and subsegmental bile ducts, their variations in course and confluencing, and the relationship between the structures of the portal pedicle. The types of confluence and their frequency were determined separately for the left, right, right anterior sectorial, right posterior sectorial and common hepatic ducts. "Normal" left confluence was formed in 82% and three variations in 18%. "Normal" right confluence was formed in 75% and four variations in 25%. A complete ("normal") anterior sectorial duct was present in 35% and four variations in 65%. A complete ("normal") posterior sectorial duct was present in 61% and four variations in 39%. The study showed that the biliary tree variations are quite frequent, and therefore clinically important, and that they could be arranged into several types.Hepato-gastroenterology 50(50):342-8. · 0.66 Impact Factor
