Beverage consumption, appetite, and energy intake: what did you expect?

ABSTRACT Beverage consumption is implicated in the overweight/obesity epidemic through the weaker energy compensation response it elicits compared with solid food forms. However, plausible mechanisms are not documented.
This study assessed the cognitive and sensory contributions of differential postingestive responses to energy- and macronutrient-matched liquid (in beverage form) and solid food forms and identifies physiologic processes that may account for them.
Fifty-two healthy adults [mean ± SD age: 24.7 ± 5.5 y; BMI (in kg/m(2)): 26.3 ± 6.3] completed this randomized, 4-arm crossover study. Participants consumed oral liquid and solid preloads that they perceived, through cognitive manipulation, to be liquid or solid in their stomach (ie, oral liquid/perceived gastric liquid, oral liquid/perceived gastric solid, oral solid/perceived gastric liquid, or oral solid/perceived gastric solid). However, all preloads were designed to present a liquid gastric challenge. Appetite, gastric-emptying and orocecal transit times, and selected endocrine responses were monitored for the following 4 h; total energy intake was also recorded.
Oral-liquid and perceived gastric-liquid preloads elicited greater postprandial hunger and lower fullness sensations, more rapid gastric-emptying and orocecal transit times, attenuated insulin and glucagon-like peptide 1 release, and lower ghrelin suppression than did responses after oral-solid and perceived gastric-solid treatments (all P < 0.05). Faster gastric-emptying times were significantly associated with greater energy intake after consumption of perceived gastric-liquid preloads (P < 0.05). Energy intake was greater on days when perceived gastric-liquid preloads were consumed than when perceived gastric solids were consumed (2311 ± 95 compared with 1897 ± 72 kcal, P = 0.007).
These data document sensory and cognitive effects of food form on ingestive behavior and identify physical and endocrine variables that may account for the low satiety value of beverages. They are consistent with findings that clear, energy-yielding beverages pose a particular risk for positive energy balance. This study was registered at clinicaltrials.gov as NCT01070199.

    ABSTRACT: The expected impact of a food or drink on appetite can influence decisions around eating and the actual experience of satiation and satiety post-consumption. This study explored the relationship between a product’s anticipated sensory characteristics and its expected impact on feelings of hunger, fullness and thirst. Female participants (n = 118) evaluated 40 widely available food and beverage products (varying in physical characteristics, packaging, serving size and total energy content) for anticipated sensory characteristics, pleasantness and familiarity, alongside expected impact on immediate fullness, hunger after one hour and thirst both immediately and after one hour. Correlations revealed that the most caloric products and those anticipated to be creamier were expected to be more filling and hunger suppressing than the products with lower energy value and expected to be less creamy. Total energy was the best predictor of expected satiation and satiety. We observed that beverage products were expected to be similarly satiating as food products (including liquid, solid and semi-solids) with a similar total energy content and expected creaminess. On the other hand, products expected to be less salty and thick were expected to be most thirst-quenching, and these tended to be beverage products, regardless of their total energy content. These results are in line with emerging evidence suggesting that certain sensory cues associated with nutrients can be used to estimate the satiating power of other foods with similar sensory characteristics. Beverages are expected to be uniquely thirst-quenching, but may be expected to be similarly satiating as solid and semi-solid food products of comparable energy content.

    ABSTRACT: Objective To provide an overview of beverage consumption patterns using the first nationally representative survey of dietary intake in Brazil.Design Beverage consumption data were obtained by 1 d food records in an individual dietary survey.Setting Nationwide cross-sectional survey, 2008-2009.Subjects Nationally representative sample of individuals aged ≥10 years (n 34 003).Results Beverages contributed 17·1 % of total energy consumption. Caloric coffee beverages provided the greatest level of energy overall (464 kJ (111 kcal)/d). Individuals aged 10-18 years (243 kJ (58 kcal)/d) and 19-39 years (230 kJ (55 kcal)/d) consumed a higher proportion of energy from sugar-sweetened soft drinks than individuals over this age (142 kJ (34 kcal)/d for those aged 40-59 years and 79 kJ (19 kcal)/d for those aged ≥60 years).Conclusions Overall, the contribution of beverages, particularly sugary beverages, to total energy consumption in Brazil represents an important public health challenge and is comparable with those from other countries.

    ABSTRACT: The goal of this review was to critically evaluate the scientific evidence in humans on the potential effect of sweetened beverages on weight gain and risk of obesity in youth and adults. Two categories of these beverages were reviewed. Sugar-sweetened beverages (SSBs) include soft drinks, colas, other sweetened carbonated beverages, and fruit drinks with added sugar. Artificially sweetened beverages (ASBs), also referred to as non-nutritive sweetened beverages, are marketed and used as a replacement for SSBs for those who want to reduce sugar and caloric intake. The totality of evidence to date demonstrates a pattern across observational and experimental studies of an increased risk of weight gain and obesity with higher intake of SSBs. However, it remains difficult to establish the strength of the association and the independence from other potentially confounding factors. The primary reason for unclear conclusions regarding the robustness of any effect of SSBs is due to the heterogeneity and methodologic limitations of both observational and experimental studies on this topic. Although some observational studies have suggested that ASBs may cause increased risk of obesity and cardiometabolic diseases, there is no clear mechanism for this pathway, and the epidemiologic studies are highly inconsistent. An important issue with the observational studies on ASBs and obesity or disease risk is reverse causality bias, with higher-quality studies demonstrating this possibility. The field needs higher-quality experimental studies in humans, with relevant direct comparisons between sweetened beverages and their sweetened solid-food alternatives.
