Informed Consent: Advising Patients and Parents About Complementary and Alternative Medicine Therapies

ABSTRACT Although research on complementary and alternative medicine (CAM) therapies is still limited, systematic reviews have revealed sufficient evidence to conclude that CAM can be effective for certain conditions. In this article we discuss clinicians' responsibilities to inform parents/patients about CAM alternatives and use the example of acupuncture for chemotherapy-induced nausea and vomiting. Chemotherapy-induced nausea and vomiting remain significant adverse effects of cancer therapy, and some patients cannot find relief with standard therapies. When making decisions for a child with a life-threatening illness, parents must consider all reasonable options and decide what is in the child's best interests. A physician's failure to provide parents with relevant information regarding therapies with the prospect of therapeutic benefit impedes their ability to make an informed decision. Physicians have the ethical duty of beneficence; they must be aware of current research in pain and symptom management and other aspects of care. A physician's duty of care does not necessarily include the obligation to provide information about therapies outside the range of conventional treatment or those not yet supported in the medical literature. However, as CAM therapies such as acupuncture become better studied and their safety and efficacy are established, the scope of disclosure required may expand to include them. The legal and ethical obligation to obtain informed consent to treatment requires disclosure and discussion of therapies when there is reliable evidence of potential therapeutic benefit. At the same time, the more limited state of knowledge regarding effects of a particular therapy in the pediatric population must be factored into decision-making when treating a child.

    ABSTRACT: Introduction Pediatric use of natural health products (NHPs) is common, although conventional health care providers frequently do not attempt to manage them due to limited knowledge and lack of confidence. The aim of this review is to synthesize available guidance given to pediatric health care providers on how to manage NHPs in clinical practice. Methods An integrative review of the literature was conducted. Key search terms were NHPs, dietary supplements, herbal medicines, CAM, pediatrics, decision-making, guides, management, safety, parents, and medical providers. Expert organizations and databases from CINAHL, PubMed, Embase, and the Cochrane Collaboration were searched. Fifty-two articles were chosen for inclusion based on appraisal using the Johns Hopkins Nursing Evidence-Based practice tool for research and non-research. Themes for NHP management were identified through integrating repeated expert guidance. Results: Three themes emerged from the literature regarding clinical management of NHPs: product regulation and its impact on safety, communication deficits, and limited provider knowledge. Despite guidance on NHP management from well-known organizations such as the American Academy of Pediatrics and the NIH National Center for Complementary and Alternative Medicine, considerable heterogeneity was discovered in management guidelines. Discussion This is the first known review to synthesize NHP clinical management guidance. Identified themes led to development of six key principles to help guide clinician NHP management. More research is needed to evaluate if this guidance is effective in promoting clinician confidence and competency with NHP management.     ABSTRACT: Objective To describe the chiropractic care of a pediatric patient with complaints of spinal pain, headaches, motor dysfunction and left leg and left arm twitching concomitant with a diagnosis of conversion disorder.Clinical featuresThree and half months following a fall on her head from the monkey bars, the patient presented for chiropractic care with complaints of neck pain, dizziness, severe headaches, significant left leg and left arm twitching (i.e., similar to patellar and biceps deep tendon reflex testing), and poor motor control of the lower extremities. Medical testing (i.e., neurological and imaging studies) to find an organic cause in her twitching was unsuccessful. The patient was placed on Montelukast, Naproxen, Valium, Prednisone and Ibuprofen medications. The patient's activities of daily living were severely affected as well as her ability to attend school.Intervention and outcomeChiropractic spinal manipulative therapy and adjunctive therapy consisting of rehabilitative exercises were provided to the patient. Following 30 patient visits, the patient no longer suffered from headaches or neck pain and resolution of her left leg and left arm twitching. The patient was able to resume her school and various activities of daily living.Conclusion This case report provides supporting evidence that pediatric patients with a diagnosis of conversion disorder may benefit from chiropractic care.

    ABSTRACT: Many cancer patients use cancer diets. We listed 13 cancer diets simulating an internet search for which we systematically reviewed clinical data. In the next step we derived recommendations on counseling patients using a Delphi process. We evaluated the following diets: raw vegetables and fruits, alkaline diet, macrobiotics, Gerson's regime, Budwig's and low carbohydrate or ketogenic diet. We did not find clinical evidence supporting any of the diets. Furthermore, case reports and pre-clinical data point to the potential harm of some of these diets. From published recommendations on counseling on complementary and alternative medicine, we were able to derive 14 recommendations for counseling on cancer diets. Considering the lack of evidence of benefits from cancer diets and potential harm by malnutrition, oncologists should engage more in counseling cancer patients on such diets. Our recommendations could be helpful in this process.
